A jobsite observation of the condition of the existing framing system was performed by an audit team of Blue Raven Solar as a
request from Domus Structural Engineering. All review is based on these observations and the design criteria listed below and
only deemed valid if provided information is true and accurate.
On the above referenced project, the roof structural framing has been reviewed for additional loading due to the installation of
the solar PV addition to the roof. The structural review only applies to the section of the roof that is directly supporting the
solar PV system and its supporting elements. The observed roof framing is described below. If field conditions differ, contractor
to notify engineer prior to starting construction.
The roof structure of (MPI) consists of composition shingle on roof plywood that is supported by 2x6 rafters @ 24"o.c. paired
with 2x6 ceiling joists @ 24"o.c.. The rafters have a max projected horizontal span of 9'-9", with a slope of 19 degrees. The
rafters are connected at the ridge to a ridge board and are supported at the eave by a load bearing wall.
The existing roof framing system of (MPI) is judged to be adequate to withstand the loading imposed by the installation of the
solar panels. No reinforcement is necessary.
The spacing of the solar standoffs should be kept at 72" o.c. for landscape and 48" o.c. for portrait orientation, with a
staggered pattern to ensure proper distribution of loads.
The scope of this report is strictly limited to an evaluation of the fastener attachment, underlying framing and supporting
structure only. The attachment's to the existing structure are required to be in a staggered pattern to ensure proper
distribution of loading. All panels, racking and hardware shall be installed per manufacturer specifications and within specified
design limitations. All waterproofing shall be provided by the manufacturer. Domus Structural Engineering assumes no
responsibility for misuse or improper installation of the solar PV panels or racking.
Note: Seismic check is not required since Ss<.4g and Seismic Design Category (SDC) < B

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

CONTRACT AGREEMENT
TERMS AND CONDITIONS
1. General. This is a contractual agreement ("Agreement") by and between
O'Connor Roofing, Company (hereinafter called "Contractor'), and the Owner(s), as
reflected on the reverse side, whereby, upon Contractor's acceptance, Contractor
agrees to supply and furnish the materials and labor at the location(s) set forth on the
reverse side, to repair, restore, refurbish or build the improvements to the real property,
and whereby (jwners) agree to pay to contractor all invoices within ten (10) days after
job completion.
2. Materials and Workmanship. Contractor agrees to supply and furnish and use
materials of a value and quality reasonably equivalent to those previously existing (if
any), if reasonably available, provided, however, Owner may require materials of a
different value or quality than those previously existing (if any) if specified in writing
and agreed to by Contractor. Unless otherwise specified by Owner in writing and
agreed to by Contractor, Contractor, in furnishing materials and labor, agrees to match
reasonable existing textures, colors, plans and materials but does not guarantee exact
duplication.
3. Access to Work. The Owner shall at all times have access to the work wherever
it is in preparation and progress.
4. Zoning and Modification. Owner agrees to furnish all information necessary to
secure plans and permits, if required or necessary, unless otherwise specified, and
Owner warrants the work contracted for to be in compliance with zoning, classification
and building codes. In covenant -controlled communities or neighborhoods that have
an HOA, it is the responsibility of the property owner to make sure that such governing
body approves selection of product. Owner agrees that Contractor may employ an
architect, on behalf of Owner, to submit plans to the property authorities for approval,
inasmuch as this Agreement may be based upon an estimate of what will be required
to restore property to its equivalent status before loss, if any modification in the original
plans and/or of the contents of the property before loss is desired by Owner, or if any
additional work required by lawful authorities to bring the plans, contents and/or work
into compliance with code requirements shall entail additional costs. Owner agrees to
pay such additional costs and architect's fees to be covered by the Estimate.
5. Pre -Existing Conditions. Owner agrees that Contractor assumes no
responsibility for pre-existing conditions of inferior workmanship or materials; damage
due to termites, rot or deterioration, or violations of zoning, classification or building
codes or leaks above the roof line!
6. Hazardous Materials. Unless otherwise provided, Contractor assumes no
responsibility for testing for the presence of, removal, or the disposal of asbestos
containing or other hazardous materials. Contractor agrees to promptly notify the
Owner if such materials are encountered and recognized as the work progresses, and
may, at Contractors option, stop work until such time that suspect materials have been
properly tested or arrangements by the Owner have been for the lawful removal and
disposal. Contractor may also at Contractor's option issue to the Owner for all work
completed to the date of work stoppage, with such billing to include all costs including
direct and indirect overhead, taxes and profits on the work completed.
7. Ownership of Materials. All work and materials delivered on the premises by
Contractor to form part of the work, whether actually incorporated therein or not are to
be the property of the Contractor until the same have been paid for in accordance with
the terms hereof. All surplus and salvage materials remain the property of Contractor.
S. Acceptance. This Agreement, having been signed by Owner as accepted,
constitutes an irrevocable offer to be accepted or rejected by Contractor within thirty
(30) days from date of its delivery from Owner.
9. Performance. Performance of this Agreement is made subject to weather, labor
strikes, fires, wars, acts of God and the ability of Contractor to obtain materials.
10. Changes During Process. If Owner chooses to have additional work done
during the re -roofing process, charges for that work will be added to the contract
amount and all specifications of this contract will apply to that work.

11. Insurance and Waiver of Subrogation. The Owner shall maintain prope
insurance upon the entire structure including `all work to be performed under tl
contract to the full insurable value thereof. This insurance shall insure against t
perils of fire, extended coverage, vandalism and malicious mischief. The Owner a
Contractor waive all rights against each other for damage caused by insured perils
the event they are not covered under the otheeLparty's liability insurance whether or r
such damage is caused by the fault or negligence of one of the parties hereto, and
the extent necessary all contracts of insurance shall be appropriately endors
evidencing this waiver of subrogation.
12. PaymEnts. Unless otherwise provided, Owner agrees to pay Contractor within t
days of completion. In the event payment is not made within ten days, then Contrac
will deem that Owner is imbreach of contract. If payment is made with a credit or de
card, a 2% fee of the contract amount will be added to the balance.
13. Severability. Should any part of this Agreement be void, unenforceable,
contrary to public policy, only such portion of this Agreement shall be deem,
eliminated herefrom, and the remainder of this Agreement shall remain valid ai
subsisting.
14. Collections. Should collection of payment under this Agreement be placed
Contractor in the hands of an attorney after payment is due, Owner agrees to p
Contractor's attorney fees, court costs, and expenses of collection. If the balance
not paid within thirty (30) days, Owner agrees to pay a late charge on any outstandii
balance from the date of completion at the rate of eighteen percent (18%) per annul
compounded monthly until paid. Any collection or legal fees will be added to tl
balance and subject to the same interest charges. Owner will be deemed to ha
accepted Contractor's performance under the contract in total unless Owner otherwi:
notifies Contractor in writing within thirty (30) days after completion of project..
15. Ice Damming. Contractor has no responsibility or liability for ice damming issw
or any consequential damages from ice damming that occur after the roofing project
completed. It is the Owners responsibility to take preventive measures to preve
causes of ice damming.
16. Flexing. When existing roofing material is removed from a roof, it is n
uncommon for the roof to "Flex". On occasions this flexing may cause small cracks
sheet rock. Contractor cannot be responsible.
17. Nail Pops. Construction of buildings in many cases involves the use of sheetroi
interiors. At the time of application, a small amount of spackling may be used to cow
the head of the nail for application. Contractor cannot be responsible if spackling
dislodged during roof installation.
18. Disclaimer. Contractor will not responsible for items such as pictures, chin
crystal or anything in the interior of the building that may fall from vibration during fl -
work process. It is the owner's responsibility to secure such items. Contractor will n,
be responsible for exterior items that are not properly secured such as light fixtures 1
any items attached to the building that may fall from the vibration during the woi
process. Contractor will not be responsible for items that could be damaged froi
debris falling from the roof, such as pottery, outdoor furniture, grills. Owner accepts th
responsibility of moving such items away from the building during the work process.
Contractor will not be responsible for any damage to driveways caused by supp
company trucks. Contractor will not be responsible for separation of the sealant of ler
of skylights from the frame. of the skylight.
Contractor will not be responsible for repositioning of satellite dishes.
Owner agrees to keep the driveway clear of any vehicles or possessions that woul
prohibit the supply company from loading the roof. Owner also agrees to keep th
driveway clear of any vehicles or possessions during the installation of the roof.
19. Agreement. Owner and Contractor acknowledge they have read and understan
each of the foregoing provisions and the terms and descriptions contained in th
estimate and other contract documents prior to execution and agree to comply wit
each term and condition without reservation.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.
