Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, po Flans

2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if /
your contractor's license or insurance has expired, and you may update those (�
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS AND CONDITIONS. This Contract agreement is between Mixteriors
Roofing, Ilc. ("MR") and the Customer identified on page 1 of this Agreement.
1.DELIVERY AND PAYMENT. An initial deposit is due upon acceptance of
Agreement. All remaining balances are due upon MR's final inspection of the work
("substantial completion"). On commercial jobs, sales taxes are owed by Customer
and will be added to the total price of the Contract or Final Invoice. On residential
jobs, however, taxes are included in the price of the Contract. All building permit
fees shall be paid by Customer which will be reflected in the Final Invoice. On
outstanding balances existing 7 days after substantial completion Customer shall
be charged 18% interest or the maximum amount allowed by law, whichever is
lower, on all amounts owed. However, if Customer submits an insurance claim, th
interest shall not accrue and be charged in until 30 days after substantial
completion. The parties agree that there shall be no retainage withheld on the
project_ The Customer may not retain, delay, offset or hold back any monies owed
MR for any reason. In the event Customer submits an insurance claim, payment by
Customer to MR is not conditioned upon payment from the insurance company.
All amounts not paid by Customers insurance carrier are due by Customer upon
substantial completion.
2.DAMAGE. In the event damage to materials (the "Product") caused by MR's
crews occurs during delivery or installation, MR shall repair or replace the
damaged materials at its discretion and as necessary to comply with the
warranties. MR is not liable for any damage to the Product once installed at the
home or job site if such damage is not caused by MR.
3-CODES/ORDINANCES/PERMITS. MR is not responsible to meet any ordinances,
building or fire codes, or homeowner association regulations.
4 -ACCEPTANCE OF ORDER.. Cashing a down payment check does not constitute
acceptance of an order. Notwithstanding, Customer or MR may cancel this
agreement at any time within three business days of all parties acceptance by
notifying the other party in writing by hand delivery or certified return receipt
mail.
S.INSURANCE AND RISK OF LOSS. Customer shall carry insurance that covers fire,
wind, wind -driven rain, tornado, hail and water damage.
6JOB SITE CONDITIONS. Customer shall provide a clear worksite. All expenses
related to delays to installation caused by un -cleared obstructions shall be
paid/charged to Customer. Customer shall furnish electricity and water to the
worksite at no expense to MR. MR is not responsible for hidden site conditions not
identified by the Customer.
7.WARRANTY/EXCLUSIVE REMEDY. Unless otherwise stated in this Agreement,
MR
warrants its workmanship as follows: On modified Bitumen roofing -1 year; TPO -
3 Yrs. On shingle roofs with a 2:12 pitch or greater- 3 years (if the slope is lower
than a 2:12 pitch there is workmanship warranty); (c) On PVC roofing systems -5
years; (d) On Standing seam metal roofs -5 Years above 3:12 pitch; (e) On standing
seam double lock metal roof with a 2:12 pitch or greater -5 years (If the slope is
lower than a 2:12 pitch there is no workmanship warranty); (f) On standing seam
single lock or snap lock metal roof with a 3:12 pitch or greater- 5 years ( If the pitch
is lower than 3:12 pitch there is no warranty). All other complete systems not
mentioned herein are one year. Repairs have a 90 day warranty on the specific
repair only and do not include any warranty around the repair. The means,
methods and materials in workmanship warranty repairs shall be MR's choice. If
any amounts owed under this agreement are not paid, the warranties in this
agreement are void. The above workmanship warranties are transferable for
$250.00 if a request in writing is received by MR within 30 days of new ownership.
MR does not warrant materials. All material warranty claims shall be made by the
Customer directly to the manufacturer. The warranties in this Agreement shall not
apply if MR advises Customer that flashing, trim, caulking, masonry, siding and or
any other exterior component should be replaced and such advice is rejected by
Customer.
THE ABOVE WARRANTIES ARE IN LIEU OF ALL OTHER WARRANTIES, EXPRESS,
IMPLIED, COMMON LAW OR STATUTORY INCLUDING THE IMPLIED WARRANTY
OF MERCHANTABILITY AND FITNESS FOR A PARTICULAR PURPOSE, WHICH ARE
HEREBY EXPRESSLY DISCLAIMED.
8.CONSTRUCTION/SERVICE CALLS. All work shall be constructed using MR's
standard methods unless otherwise noted in this Agreement. Customer
acknowledges that with installation of Standing Seam Roofing, oil canning is
inherent and shall not be a cause for rejection. Customer understands and agrees
that environmental conditions including temperature and humidity may cause
construction materials to warp, expand, crack, split, wrinkle or loosen and such
conditions are not considered a defect. Work directed by Customer not covered by
applicable warranty in this Agreement shall be done at $75.00 per man hour plus

materials as an extra charge to Customer. Customer further agrees that any request for
service not covered by the workmanship warranty shall require Customer to pay MR all
currently applicable service call and diagnosis fees.
9.LIMITATIONS OF LIABILITY. MR is not liable for any incidental, special or consequential
damages, including loss of revenue, loss of use of equipment, loss of use of facilities, or
economic damages, based on any theory of law to include but not limited to strict liability,
breach of contract or warranty, failure to follow applicable statute/ordinance/code, or
negligence. MR's maximum liability for any reason for any and all damages to Customer shall
consist of the refunding of all money paid by Customer to MR under this Agreement. Further,
should Customer become aware of damage to the Customers property by MR, its agents or
e employee's, during the course of the job, said damage shall be brought to the attention of
MR prior to substantial completion. If Customer fails to notify MR in writing within ten
working days of occurrence, Customer waives all rights against MR concerning said damage.
Customer further agrees that MR shall have no liability for damages of any kind, including
ensuing loss damages to persons or property located below the installed roof, windows or
siding whether or not such damages are a result from (a) water leaks or other weather
oriented sources or (b) mold growth (c) puncture to electrical, A/C lines or anything else
damaged by fasteners; (d) vibrations; (e) ACTS OF GOD to include lightning, wind speeds of
40 mph or greater lasting for 1 hour or longer or winds of 58 mph or greater for any
duration, hurricane, tornado, hail, ice, or wind driven rain. MR is not liable for the
adjustment/alignment of satellite dishes or any other products or materials on Customer's
roof. MR is also not liable for damages to concrete driveways, whatever the source or reason
or for damages.
IMENTIRE AGREEMENT. This Agreement constitutes the entire understanding between the
parties. No other provisions, alterations, or additions, shall be binding on either party unless
in writing and signed by the Customer and an officer or authorized agent of MR. No oral
agreements or understanding shall be binding or deemed to exist.
113NDEPENDENT CONTRACTOR. Customer understands and agrees that MR may engage an
independent contractor to perform the work pursuant to this Contract. Such independent
contractors are not authorized to bind MR in any manner.
12.0ISPUTE RESOLUTION. MR and Customer agree that any and all claims, including but not
limited to: (a) relating to, this agreement and any amendments thereto; (b) any controversy,
dispute or claim arising by virtue of any representations, omissions, promises or warranties
alleged to have been made by MR or its agents; and (c) any personal injury or property
damage claim against MR or its agents, shall be decided by a court of general jurisdiction in
Boulder County, Colorado. The costs and attorney fees shall be awarded to the prevailing
party in any suit. The parties agree venue is proper in Boulder County, Colorado, and agree
the laws of the state of Colorado shall govern this contract.
13 -WAIVER OF CONSUMER RIGHTS. I WAIVE MY RIGHTS UNDER THE DECEPTIVE TRADE -
PRACTICES ACT, SECTION 17.41 ET SEQ, BUSINESS AND COMMERCE CODE, A LAW THAT
GIVES CONSUMERS SPECIAL RIGHTS AND PROTECTIONS. AFTER CONSULTATION WITH AN
ATTORNEY OF MY OWN SELECTION, 1 VOLUNTARILY CONSENT TO THIS WAIVER.

TRIAL FOR ANY AND ALL CLAIMS ARISING OUT OF OR IN CONNECTION WITH THIS CONTRACT
