The purpose of our limited observation was to review the fire damage and to make recommendations concerning its repair. The fire damage is confined to the south side of the second floor of the house, within the mansard roof and in the framing for the second floor deck. Enclosed you will find three drawings; S-1 is a mansard roof framing detail and S-2 is a deck framing plan. S-3 is general notes. We noted charring on several members of the mansard roof framing on the south side of the house. If this charring is more than 1/8" deep on any face of a member (this can be measured by scraping the charring), it must either be replaced, per S-1, or sistered with a same -size member, full length, face nailed together with 16d @?a 12" o.c. nails. We also noted some framing members missing from the mansard roof framing. These members should be replaced per S-1. The second floor deck framing should be replaced entirely; see S-2. It is our professional opinion that the remaining non -charred structural members of the house, including vertical and horizontal elements and foundations, can remain in place. The purpose of this report is to document this remediation project was successfully completed, and indoor asbestos amplification was not occurring in the work area. An Industrial Hygienist for Earth Energy Environment (E3) performed the remediation project completion inspection and testing procedures. BACKGROUND A spill occurred inside of the property located at 3190 Wadsworth Blvd in Wheat Ridge, CO 80033. Asbestos Abatement Inc. was contracted to clean the spill area. E3 was contracted by Asbestos Abatement Inc. to perform the post remediation visual inspection and air clearances. INSPECTION METHODOLOGY The remediation work areas (Interior Containment) were visually inspected in order to confirm the scope of work was properly followed and completed. The containments were also visually inspected to verify breaches and/or defects were not present. Air sampling and TEM analyses were conducted in accordance with the protocol in 40 C.F.R. Part 763, Appendix A to Subpart E (EPA 1995) and National Institute for Occupational Safety and Health (NIOSH) method 7402 using standard 3 piece, 0.45um + 5.Oum MCE filter material cassettes. Once the area passed a final visual inspection and no dust or debris has been found, the certified Air Monitoring Specialist collects air samples using aggressive sampling. Air samples were analyzed by Reservoirs Environmental Laboratory of Denver, Colorado, an independent Colorado registered laboratory. The laboratory reports the number of asbestos fibers detected and the concentration of airborne fibers as fibers per square millimeter (s/mm2). The TEM Air Clearance Testing Page 2 of 2 Asbestos Hazard Emergency Response Act (AHERA) TEM clearance standard is 70 structures per square millimeter (70 S/mm2). TEM analysis is more expensive than PCM analysis. However, TEM analyses provides more accurate information on the actual number of airborne asbestos fibers present. The TEM analysis is beneficial to the remediation contractor and the building owner when a zero tolerance for airborne asbestos fibers in the work area is preferred. FINDINGS AND RESULTS Visual Inspection A visual inspection indicated that the work was completed in accordance with the project Scope of Work and current asbestos abatement industry standards. No visible debris or areas of concern were noted in the work areas at the time of the inspection. Asbestos Air Testing Results Aggressive air sampling methods were used in all the work areas. Aggressive sampling methods is a procedure in which air is deliberately disturbed or agitated by using a leaf blower and fans to purposely dislodge and make airborne any remnant asbestos fibers. A total of five (5) air samples were collected. Samples were collected from the work areas. The leaf blower was used on every surface in the work areas. The sampling results are summarized in Table 1. All air sampling results indicated no asbestos fibers detected. The Reservoirs Environmental laboratory testing report is provided in Attachment B. The sampling results relied upon for this report do not include or apply to any other areas within the structure that were not sampled but may contain asbestos; including materials/areas that may be hidden or inaccessible. I (7-1-91 edition) Pt. 763, Subpart F App. A, pages 293 — 299. This procedure involves examination of the samples with a petrographic microscope utilizing polarized light, a procedure commonly known as Polarized Light Microscopy (PLM). Colorado Regulation NO. 8 Part B Regarding Asbestos Spills The following procedures apply to all areas of public access, except school buildings, in which there has been a release of asbestos fibers due to a breach of the containment barrier on an abatement project, or due to any cause other than abatement of asbestos. III.T.1. Major Asbestos Spills In the event of an asbestos spill involving greater than the trigger levels, the building owner or contractor shall: III.T.1.a. Restrict access to the area and post warning signs to prevent entry to the area by persons other than those necessary to respond to the incident. 111. T. Lb. Shut off or temporarily modify the air handling system to prevent the distribution of asbestos fibers to other areas. III.T. I.e. Immediately contact the Division by telephone, submit a notification in compliance with subsection III.E. (Notifications) and, if in an area of public access, apply for a permit in accordance with subsection III.G. (Permits). Be exempted from the requirements to have a certified Supervisor on-site at all times, until such time as the immediate danger has passed. Any cleanup or asbestos abatement that must occur after the immediate danger has passed shall be supervised by a person certified by the Division. III.T. Le. Using certified Supervisors and certified Workers in accordance with section II. (Certification Requirements) of this Regulation, seal all openings between the contaminated and uncontaminated areas and establish negative air pressure within the contaminated area in accordance with paragraph III.J. (Air Cleaning and Negative Pressure Requirements). This is to be accomplished using polyethylene sheeting to cover areas such as doorways, windows, elevator openings, corridor entrances, grills, drains, grates, diffusers and skylights. III.T. Lf. HEPA vacuum or steam clean all carpets, drapes, upholstery, and other non -clothing fabrics in the contaminated area, or discard these materials. III.T.I.g. Launder or discard contaminated clothing in accordance with subsection III.R. (Waste Handling). III.T.I.h. HEPA vacuum or wet clean all surfaces in the contaminated area. III.T. Li. Discard all materials in accordance with subsection III.R. (Waste Handling). III.T.I .j. Following completion of subparagraph III.T. La. through III.T. Li. above, comply with air monitoring requirements as described in subsection III.P. (Clearing Abatement Projects); air samples shall be collected aggressively as described in 40 C.F.R. Part 763, Appendix A to Subpart E (EPA 1995), except that the air stream of the leaf blower shall not be directed at any friable ACM that remains in the area. III.T.I .k. Comply with any other measures deemed necessary by the Division to protect public health. Lead Sampling: Samples of suspect lead containing paint were collected from the following locations, Third Level (southeast bedroom south wall and ceiling south bedroom ceiling and wall) Entry Level (foyer ceiling, kitchen ceiling and south bar room entry south wall) and were submitted for lead analysis in accordance with (SW 846 3050B/7000B). Lead-based paint is defined as those materials containing lead at/or above 1.0 mg/cm2; 0.5% by weight; or 5000 ppm (parts per million). Lead- based paint is defined as those materials containing lead at/or above 1.0 mg/cm2; 0.5% by weight; or 5000 ppm (parts per million). Sample results are attached and indicate that samples collected from the third level south bedroom were determined to contain lead above 5,000 ppm. Results for other materials sampled indicate that the samples collected did not meet the definition of lead-based paint, however, it is important to note that lower concentrations of lead were detected (see attached laboratory sheet). Therefore, WES recommends that any contractor who disturbs any lead - laden surfaces be notified of the hazard and their requirement to comply with all applicable federal, state and local rules and regulations. Appropriate local, state and federal rules and regulations must be followed when disturbing asbestos and /or lead contain materials. Any suspect building material that was not accessible, identified, or not sampled during this survey and is not identified in this report should be sampled and analyzed for asbestos and/or lead content prior to disturbance.
