Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

1. This permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those

For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

II. Consumer Protection
1. Turner Morris, Inc shall hold in trust any payment received from you until Turner Morris, Inc has delivered roofing
materials at the site or has performed a majority of the roofing work on your property -
2. Turner Morris, Inc carries worker's compensation, automobile and commercial general liability (bodily injury and
property damage) insurance. A Certificate of Insurance identifying our insurance carrier and the types and amounts
of coverage and our insurance contact information is attached or will be provided to Customer and is incorporated
herein by reference and should be considered as part of this contract.
3, You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering the
contract. If you plan to use the proceeds of a property and casualty insurance policy to pay for the roofing work,
you may rescind this contract within 72 hours after you receive written notice frorla the property and casualty insurer
that your claim has been denied in whole or in part, However, Turner Moms, Inc., is entitled to retain payments or
deposits to compensate Turner Morris, Inc for roofing work actually performed in a workmanlike mariner consistent
with standard roofing industry practices, Turner Morris, Inc cannot pay, waive, rebate, or promise to pay, waive or
rebate all or part of atay insurance deductible applicable to the insurance claim for payment for roofing work on your
prop erty-
4. Property Owner is / is not intending to make payment from the proceeds of a property and casualty
insurance policy.
III. Terms and Conditions
Payment terms; Standard Payment Terms- Projects under $5000: Net 30. Projects greater than $5000; 50% upon materials
delivery, 40% upon substantial completion and 100/0 30 days after completion. For projects lasting over 1 month- monthly
progress payments for materials stored on or off site and installation completed including overhead and profit. Invoices are
payable 10 days after receipt. Late balances due will accrue 1% interest per month. A $250 lien filing fee will be charged on
payments 60 days overdue. Failure of Customer to make proper payment to Turner Morris, Inc., (TMI) when due shall, in
addition to all other rights, constitutes a material breach of contract and shall entitle TMI, ar its discretion, to suspend all work
and shipments, including furnishing of any warranty, until full payment is made. The time period for TMI to perform the Work
shall be extended for a period equal to the period during which the Work was suopmded, and the contract sum to be paid TMl
shall be increased by the amount of TMI's reasonable costs of shut -down, delay and start-up. Should it be ncceasary to engage
the services of an attorney to collect monies overdue or enforce or defend our rights under this agreement, customer agrees to pay
all costs of collection, including prejudgment and postjudgment costs, arbitration fees and reasonable attorney fees.
2. Exclusigns: Snow removal, structural repairs, skylights, carpentry, plumbing, electrical, mechanical, masonry, or code upgrades
unless specifically mentioned in the above scope of work. TMI neither assumes not accepts any responsibility for the proper
construction, slope or moisture content ofthe roof deck _TMI is not responsible for leakage through the existing roof or other
portions ofthe building that have not yet been re -roofed by TMI. You agree to inform TMI in advance about any hazardous or
regulated substances, including but not limited to asbestos or mold, that may exist or be encountered anywhere in or around the
work area. If TMI encounters any such substance during the course of performance of this proposal, TMI will immediately stop
work until you have arranged for the containment, removal and cleanup ofthe substance as required by any applicable law_ TIVII
is not responsible for any costs associated with containment, disposing of or cleaning up any substances encountered by us, or
directly or indirectly affected by our work, unless that is specifically agreed to in the proposal, Further, you will be responsible
for any increased costs of performance caused to TNII by the existence of any such substance in and around the work area.
3. Disclain jU TMI is an installer only of roofing and waterproofing materials intended to keep exterior moisture from penetrating
through roofing material. We are not a designer, architect or engineer. TMI makes no warranties or guarantees as to the adequacy
of existing or new roof venting to prevent excess humidity and consequential problems of condensation and mold, or any other
interior climatic problems. We recommend that owner consults with an archirect, engineer or other interior air quality
professional concerning ventilation concerns. Owner acknowledges that noise, fumes and odors will be generated as pan of

normal roofing operations. Owner will indemnify and hold harmless TMI from claims relating to noise, fumes and odors
released during normal roofing process. Customer shall be responsible for interior air quality, including controlling mechanical
equipment, HVAC units, intake vents; wall vents, windows, doors and other openings to prevent iutrmes and odors from entering
the building" TMI's price is based upon there not being electrical or other conduit or other materials embedded within the roof
assembly or directly affixed to the underside of the roof deck unless expressly identified on the faco of this proposal. Customer
will indemnify TMI from any personal injury, damage, claim, loss or expense resulting from the presence of conduit, shall render
the conduit harmless so as to avoid injury to TMI's personnel, and shall compensate TMI for additional time, labor and expense
resulting from the presence of such materials. Customer acknowledges that re -roofing of an existing building may cause
disturbances, dust or debris to fall into the interior. Customer agrees to remove or protect property directly below the roof in
order to minimize potential interior damage. TMI shall not be responsible for disturbance, damage, clean-up or loss to interior
property that Customer did not remove or protect prior to the commencement of roofing operations. Unless otherwise agreed, it is
understood That the work shall be performed during regular work hours of regular working days. If overtime work is mutually
agreed upon and performed, an additional chargee therefore, at our published rates for such work, shall be added to the contract
amount. Neither you nor TMI shall be liable to the other party hereto for any loss, damage or delay arising out of any cause
beyond the reasonable control of either party, including but not limited to: Acts of government, strikes, lookouts, fire explosion,
theft, floods, riot, civil commotion, wax, malicious mischief or force majenre; provided however, that, should loss or damage to
our material or work occur at the installation site, you shall compensate TMI therefore, unless such loss or damage results from
Cie acts or omissions of TML TMI's liability with respect to any claim by you of any nature pursuant to this agreement shall be
limited to the dollar amount of this proposal. Under no circumstances shall either of us be liable to the other for any
consequential damages from whatever cause arising"
4. Dispute : Any controversy or claim arising out of or in relating to this proposal or the breach thereof shall be settled by
arbitration in accordance with the rules then obtaining of the American Arbitration Association" The decision of a single
arbitrator shall be final as to law and fact, and judgment upon the award may be entered in any court having jurisdiction thereof.
The arbitration proceedings shall be beld at such place as the parties may mutually agree upon or in the event that their failure to
agree, it shall be held in the City & County of Denver.
