(2) This permit shall expire it (A) the work authorized is not commenced within sizry (80) days from issue date or (B) the building authorized is suspended or
abandoned for a period of 120 days.
(3) If this permit expires, a new permit may be acquired for a fee of one-half the amount nortnally required, provided no changes have been or will be made in the
original plans and specifcations and any suspension or abandonment has not ezceeded one (1) year. tt changes are made or if suspension ar abandonment
exceeds one (1) year, full fees shall be paid for a new permit.
(4) No work of any manner shall be done that will change the naWrai flow of water causing a dreinage problem.
(5) Contraclor shall nolify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written aOProval on inspeclion card before
proceediing with successive phases of the lob.
(8) The issuance of a permil or the approval of drawings and specifcations shall mt be consiruetl to be a pertnit for, nor an approval ot, any violation of the
provisions of the building codes or any olher ordinance, law, rule or regulation.

To Whom It May Concem:
I have been living in Minshal] Upham apartments since November 2002- Approximately
May of 2003 my waYer heater broke and had to be replaced. The water soaked my ►iving
room carpei. The management did nothing to dry the carpet. On August 23, 2003 I was
wacuuming my carpet and noticed three maggots. Later that evening I noriced more
maggots_ I inspected underneath the carpet and many more were there. The padding of
the carpet was still wet and was now moldy. I contact my managers, Ray and Cheryl
Chenning on the moming of August 24, 2003 and told them about the maggots. They did
not respond, so I visited them in person at about 11:30am. Soon after they came to my
apartment and sprayed a chemical. They claimed to have calJed an exterminator. On
August 25, 2003 I contacted them by phone several rimes to inyuire about the
exterminator who they called. They said that they haven't heard anything and they were
not sure when someone would be out. I am concerneci that my a}arhnent is not safe for
my three-year-old daughter and myself to live in because of the maggots and the mold
that lives in my carpet_
