Installation of roof sheathing (new or overlay) requires an approved inspection prior to installation of ANY roof coverings and is require on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is required. Eave and rake metal is required. A'ladder extending 3 feet above the roof eave and secured in place is required to be provided for all roof inspections. Roof ventilation is required to comply with applicable codes and/or manufacturer installation instructions, whichever is more stringent. All damage to the building which occurred while roofing was in progress will be covered by the contractor. Mountain States Roofing, Inc. requires no down payment, however if one is given, Mountain States Roofing, Inc, shall hold in trust any payment received from you until Mountain States Roofing, Inc. has delivered roofing materials at the site or has performed a majority of the roofing work on your property. Mountain States Roofing, Inc. carries worker's compensation, automobile, and commercial general liability (bodily injury and property damage insurance). A certificate of insurance identifying our insurance carrier and the types and amounts of coverage and our insurance contact information can be provided to Customer and is incorporated herein by reference and should be considered as part of this contract. You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering the contract. If you plan to use the proceeds of a property and casualty insurance policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive written notice from the property and the casualty insurer that your claim has been denied in whole or in part. However, Mountain States Roofing, Inc. is entitled to retain payments or deposits to compensate Mountain States Roofing, Inc. for roofing work actually performed in a workmanlike manner consistent with standard roofing industry practices. Mountain States Roofing, Inc. cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on your property. Mountain States Roofing, Inc. will not begin any work and/or ordering material until after the 72 hour right of rescission period(s) have expired. All material is guaranteed to be as specified. All work is to be completed in a workmanlike manner according to standard practices. All interior damage to buildings incurred 90 days after completion is excluded and the owner is notified that nay damage to contents will be the responsibility of the owner and their insurance company. All damage to the building which occurred due to roofing process will be covered by the contractor. All agreements are contingent upon strikes, accidents, or delays beyond our control. Owner is to carry fire, tornado, and other necessary insurance. Scheduling will be done based on date signed but limited to availability of materials and crews, and weather conditions. The home owner will be informed. 1 1/2 percent per month interest will be added onto all accounts 30 days past due. In addition, costs and reasonable attorney's fees for collections may be charged on any account past due more than 30 days. Payment is due upon completion. Cancellation of the contract (after 72 hours) may be achieved by paying a cancellation fee in the amount of 10 percent of the contract price unless it is due to claim being denied.
