2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

Farmer Environmental Group, LLC (Farmer) performed the limited asbestos -containing building
material survey of the residential site (Site) located at 2915 Sheridan Boulevard in Denver,
Colorado for asbestos -containing building materials (ACBM) on June 13, 2019. Colorado
Department of Public Health and Environment (CDPHE)-licensed Asbestos Inspector Kurt A.
Pierce (License #14063) conducted this survey under an agreement between Farmer and Mr.
Hamm.
This limited asbestos containing material survey is in response to a possible spill at this
residence. Textured (Smooth) Plaster (Rough and Skim Coats) was the suspect asbestos
containing material.
The purpose of the survey was twofold: 1) determine locations and conditions of
asbestos -containing products at the Site; and 2) assist in obtaining the necessary building permits
with the State of Colorado in accordance with Colorado Regulation #8.
Based on the laboratory analysis of the bulk samples collected, no products sampled were
identified as containing asbestos at the Site
All accessible areas within the Site were viewed and suspect materials touched by the inspector
to determine the location, condition, and friability of observed asbestos material. All bulk
samples were submitted for PLM analysis. When suspect material was initially sighted, bulk
samples were usually collected. By definition, a homogeneous area means an area of surfacing
material, thermal system insulation material, or miscellaneous material that is uniform in color
and texture, and is installed within the same period. ON of

Farmer has had extensive experience with older buildings that have been abated during
demolition or renovation projects. Occasionally, additional asbestos is discovered during
demolition of concealing ceiling, wall, or flooring materials.
Sample analysis reported as "None Detected" should be interpreted as meaning no asbestos was
observed in the suspect material above the reliable limit of detection, (I% by visual estimation)
for the PLM method.
The following materials did not contain asbestos:
• Texture (Smooth) on Plaster (Rough and Skim) located on ceilings and wall on the main
level and basement level
Any product containing more than I% fibrous asbestos, when analyzed by PLM, is considered a
potential hazard by the Environmental Protection Agency (EPA). PLM is the EPA -recognized
method for determining fibrous bulk asbestos content. Additionally, the EPA indicates that further
analysis by point counting be performed to confirm asbestos content for friable materials found to
contain less than ten percent asbestos by visual area estimation. Point counting analysis may be
omitted if the material is assumed to contain more than one percent asbestos and is then handled
accordingly. All samples collected by Farmer were analyzed by EMSL Analytical, Inc.'s
NVLAP-accredited laboratory in Denver, Colorado.
Sample locations are presented in Appendix I. The PLM Report in Appendix II indicate the
results of PLM analysis for bulk samples collected in the scheduled demolition areas. The PLM
Report shows the room or area in which the sample was collected and provides a description of
the sampled material.
Based on observations made at the Site, and results of the analysis of bulk samples collected
during the survey, Farmer recommends the following:
Collect additional bulk samples from previously concealed materials that may be
revealed during demolition activity, or from materials outside the original survey
scope of work. "Suspect" material includes any material serving as a sprayed -on or
troweled -on acoustic or fireproofing surface; floor and ceiling tiles; transite panels,
siding or shingles; thermal insulation or any material associated with mechanical
systems; or any binding agent such as tar sealant, mastic adhesive, roofing tar,
caulking, et cetera. Colorado Regulation #8 requires bulk samples in public
buildings be collected by a licensed Asbestos Inspector.
2. If any material cannot be positively identified as non -asbestos -containing by the
analytical results of samples collected during this inspection, it should be treated as
asbestos until a sample of the newly identified suspect material can be collected and
analyzed by PLM.

3. The building owner should be aware that removing ACBM does not discharge
ownership. During transport or after burial in an EPA -approved landfill, any
contamination throughout the lifetime of the material remains the responsibility of
the building owner.
Farmer has endeavored to inspect the existing conditions within the affected areas using CDPHE
inspection protocols. Regardless of the thoroughness of a survey, it is possible that some areas
containing asbestos were overlooked or inaccessible. This report presents general descriptions of
various construction materials and the general locations where these materials were encountered. If
questions arise during the planning for renovation, demolition or other construction, Farmer should
be notified to permit us to review the situation and present recommendations.
This report has been prepared on behalf of and exclusively for the use of Eric Hamm. The
conclusions expressed by Farmer regarding the conditions of the site are based solely on the
observations made on June 13, 2019, the data collected during this inspection, and the laboratory
results of the samples collected and analyzed. The beneficiaries are hereby advised that
conditions observed are subject to change. This report and the findings contained herein shall not,
in whole or in part, be disseminated or conveyed to any other party or be used or relied upon by any
other party, in whole or in part, without Eric Hamm' or Farmer's prior written consent.

The undersigned was requested by Owner Marco Ramirez to evaluate the adequacy
of two existing headers installed at the above referenced address. The header in the
living room spanned 9'-4" and the header in the basement spanned 8'-8". Both
composite headers were constructed of (2)-2x8 and supported by 2x4 trim studs.
The header in the living room should be sized to support a 30 psf snow load (live
load) and 15 psf dead load. The header in the basement should be sized to support
a 40 psf live load for residential house. It is recommended that both existing
composite headers should be changed at a minimum to (2)-2x 10. Alternatively the
Owner could use a 31/2"x71/2" microlam.

The undersigned was requested by Owner Marco Ramirez to look at the new
window installed in the basement of the residential house located at the above
referenced address. This window is to be served as an emergency exit for a future
bedroom upon remodeling.
The subject window is a 48" x 48" horizontal slider. The bottom of the window is
approximately 32 -inch above the basement floor. Building Codes 2015 IRC
310.2.1 requires a minimum 5.7 SF openable egress -able window and not to be
higher than 42 -inch from finished floor. It is my opinion the installed window
meets the code. However, it is recommended grating and safety steps should be
added to the window well for emergency exit.

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.
