At your request, on behalf of the homeowners, I arrived at 3244 Taft Court to review and
comment on the plumbing penetrations through the existing floor joists. I understand that the size and
location of the penetrations caused concern regarding the structural integrity of the joists. Each joist
that has a large plumbing penetration has one or two Simpson MSTA 36 steel straps installed with (10)-
10d nails on each side of the penetration, ((20) 10d nails total minimum).
Based on my observations, the plumbing penetrations with the steel strap reinforcement of
each joist is appropriate to meet or exceed all required structural loading and meet all local code
requirements.
The seal below confirms that the observations and considerations associated with this project

A toilet water supply line broke in my house and has caused substantial damage to the

Purpose of Inspection:
On July 1, 2019 L&K Environmental had been contracted to perform a limited asbestos inspection and
bulk sampling for suspect asbestos containing building material (ACBM). f the amount of ACM to be
disturbed exceeds the following trigger levels, then an asbestos abatement contractor must remove the
material:
Single -Family Residential Dwellings ("SFRD") - the trigger levels are: 50 linear feet on pipes; 32
square feet on other surfaces; or the volume equivalent of a 55 -gallon drum.
Public and Commercial Buildings (other than SFRDs) - the trigger levels are: 260 linear feet on
pipes; 160 square feet on other surfaces; or the volume equivalent of a 55 -gallon drum.
The purpose is to identify possible ACBM that is to be disturbed, determine the condition of suspect
materials, and determine homogeneous areas where suspect materials are being tested. When
sampling surfacing material, at least three samples must be collected for <1,OOOsq. ft., at least five
samples for between 1,000 - S,OOOsq. ft., and at least seven samples for >S,OOOsq. ft. When selecting
sample locations, random sampling is done in accordance with the EPA pink book. Once samples have
been collected they must be submitted for analysis.
Exclusions: 1. A certified building inspector shall not be required to perform an inspection if based on
prior records there has been sampling done and determined no ACM is present. 2. An architect or
project engineer who was responsible for the construction of the building signs a statement stating that
no ACM was used in any known building material.
Laboratory Protocol:
All bulk samples will be submitted to an independent NIST accredited NVLAP laboratory. Phase Light
Microscopy will be used to analyze samples. All samples <1% will be point counted to determine exact
asbestos composition. For each homogenous area samples containing <1% asbestos is considered not
to contain asbestos if all samples collected show asbestos in this amount. If any samples within a
homogeneous area are found to contain >1% the whole homogeneous area must be treated as asbestos
containing.

PLM Analysis Methodology
PLM samples were analyzed utilizing the Environmental Protection Agency's Test Method: Method for the Determination
of Asbestos in Building Materials (EPA 600/R-93/116. July, 1993) 40 CFR, Part 763, Appendix E to Subpart E. Reporting
Limit <1 % Asbestos. Additional treatment and tests may be required to accurately define composition (i.e. ashing, extractions,
acetone treatment, and TEM). Unused portions of samples are archived for one year unless client requests special handling.
Asbestos content of mastic/adhesive is separated from total percent asbestos and other materials.
Laboratory Equipment
Laboratory analysis was accomplished utilizing an Olympus BH -2 polarized light microscope. The microscope is
equipped with dispersion staining lenses.
Quality Control
ATC Group Services is accredited by NVLAP Bulk Asbestos Sample Quality Assurance Program (Lab Code 102031).
ATC participates in the NVLAP Bulk Asbestos Sample Quality Assurance Program and maintains an in-house QC/QA
program for bulk samples whereby 10% of all submitted samples are reanalyzed and documented in a Quality Control
Manual. ATC also participates in a quarterly round robin QC/QA program for bulk samples with several accredited
laboratories throughout the United States. Current and past QC/QA program results are available in the laboratory
for inspection.
Laboratory Personnel
Samples were analyzed by Jeff Lomme, Laboratory Director. Mr. Lomme is a professional geologist who has
successfully completed the McCrone Institutes's "Advanced Asbestos Identification" Course.

Purpose of Inspection:
On July 1, 2019 L&K Environmental had been contracted to perform a limited asbestos inspection and
bulk sampling for suspect asbestos containing building material (ACBM). f the amount of ACM to be
disturbed exceeds the following trigger levels, then an asbestos abatement contractor must remove the
material:
Single -Family Residential Dwellings ("SFRD") - the trigger levels are: 50 linear feet on pipes; 32
square feet on other surfaces; or the volume equivalent of a 55 -gallon drum.
Public and Commercial Buildings (other than SFRDs) - the trigger levels are: 260 linear feet on
pipes; 160 square feet on other surfaces; or the volume equivalent of a 55 -gallon drum.
The purpose is to identify possible ACBM that is to be disturbed, determine the condition of suspect
materials, and determine homogeneous areas where suspect materials are being tested. When
sampling surfacing material, at least three samples must be collected for <1,OOOsq. ft., at least five
samples for between 1,000 - S,OOOsq. ft., and at least seven samples for >S,OOOsq. ft. When selecting
sample locations, random sampling is done in accordance with the EPA pink book. Once samples have
been collected they must be submitted for analysis.
Exclusions: 1. A certified building inspector shall not be required to perform an inspection if based on
prior records there has been sampling done and determined no ACM is present. 2. An architect or
project engineer who was responsible for the construction of the building signs a statement stating that
no ACM was used in any known building material.
Laboratory Protocol:
All bulk samples will be submitted to an independent NIST accredited NVLAP laboratory. Phase Light
Microscopy will be used to analyze samples. All samples <1% will be point counted to determine exact
asbestos composition. For each homogenous area samples containing <1% asbestos is considered not
to contain asbestos if all samples collected show asbestos in this amount. If any samples within a
homogeneous area are found to contain >1% the whole homogeneous area must be treated as asbestos
containing.

PLM Analysis Methodology
PLM samples were analyzed utilizing the Environmental Protection Agency's Test Method: Method for the Determination
of Asbestos in Building Materials (EPA 600/R-93/116. July, 1993) 40 CFR, Part 763, Appendix E to Subpart E. Reporting
Limit <1% Asbestos. Additional treatment and tests may be required to accurately define composition (i.e. ashing, extractions,
acetone treatment, and TEM). Unused portions of samples are archived for one year unless client requests special handling.
Asbestos content of mastic/adhesive is separated from total percent asbestos and other materials.
Laboratory Equipment
Laboratory analysis was accomplished utilizing an Olympus BH -2 polarized light microscope. The microscope is
equipped with dispersion staining lenses.
Quality Control
ATC Group Services is accredited by NVLAP Bulk Asbestos Sample Quality Assurance Program (Lab Code 102031).
ATC participates in the NVLAP Bulk Asbestos Sample Quality Assurance Program and maintains an in-house QC/QA
program for bulk samples whereby 10% of all submitted samples are reanalyzed and documented in a Quality Control
Manual. ATC also participates in a quarterly round robin QC/QA program for bulk samples with several accredited
laboratories throughout the United States. Current and past QC/QA program results are available in the laboratory
for inspection.

successfully completed the McCrone Institutes's "Advanced Asbestos Identification" Course.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
