Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

I hereby authorize Infinity Restoration to discuss the details of the project with my insurance company on my behalf, as the contractor, and proceed with all
repairs authorized by the insurance company at a cost equal to the replacement cost value including overhead and profit allowed on the claim. Property
owner agrees that Infinity Restoration is entitled to the Replacement Cost value plus any and all supplements necessary to complete repairs that are
approved by the insurance company, this includes code upgrade requirements. First payment is due upon completion of the roof replacement, the balance is
due on work that has been completed, (such as the roof work) when the recoverable depreciation plus any approved supplements is received. Any
remaining unfinished work, such as gutters, siding, interior paint, or any other trade, will be due upon completion of each trade and receipt of insurance
funds. The Property owners out of pocket expense win not exceed the insurance deductible unless upgrades are agreed to in writing and added or
depreciation is "non recoverable". Deductible is to be paid directly to the contractor (Infinity Restoration). Any upgrades or downgrades will be in writing.
Property owner agrees to furnish Infinity Restoration with a copy of the insurance estimate and any other documents related to this claim immediately
upon receipt. Infinity and its Authorized representatives are not acting as Public Insurance Adjusters.
If the insurance company agrees to pay for the claim, and the undersigned does not allow Infinity Restoration to proceed with the roof project, a
cancellation fee equal to 15% of this contract amount will be due to cover administrative costs associated with preparing the project. This contract

The Property Owner/Authorized Representative selects the following materials and directs Infinity Restoration to furnish this material and its labor in a
good workmanlike manner according to standard practices in the industry. Any alterations, modifications, changes or additions must be in writing and
agreed to between the parties as evidenced by their signatures. Infinity Restoration is not responsible for damage caused by improper original construction
including faulty framing, masonry bracing and/or roof decks. Infinity Restoration is not responsible for rotted siding, paint and sheetrock damage due to

It is the property owners' responsibility to notify Infinity Restoration of any deed restrictions/H.O.A. requirements. If the undersigned fails to pay Infinity
Restoration any amounts due under this contract within thirty (30) days from the date of completion, and receipt of insurance funds, the undersigned agrees
to pay all costs of collection plus a reasonable attorney's fee, should the same be placed in the hands of an attorney for collection, along with eighteen (18)

ADDITIONAL PROVISIONS
1. All proposals subject to approval by the Management of Infinity Restoration.
2. Should default be made in payment of this contract, charges shall be added from date thereof at a rate of one and one-half percent
(I %%) per month, 18% PER ANNUM or the maximum allowed by law with a minimum charge of $2.00 per month, and if placed
for collection customer agrees to pay all cost of such action as attorney and legal filing fees by accepting said contract.
3. Infinity Restoration is not responsible for any damage below the roof, or liability is limited to the roof repair only.
4. Infinity Restoration cannot be responsible for internal structural strength, so if you have any concerns or doubts about the strength
of your roof substructure to include rafters, beams, etc., it will be the homeowner's responsibility to either notify us or have
weakened conditions repaired before we start our work.
5. The company shall have no responsibility for damages from fire, windstorm or other hazard, as is normally covered by the
homeowner's insurance.
6. If you have exposed decking inside your building i.e. cathedral ceilings, you should be certain that we know, so proper nail length
can be used.
7. Replacement of deteriorated decking, fascia board, roof jacks, ventilators, flashing or other materials, unless otherwise stated in
this contract, are not included and will be charged as an extra on a time and material basis.
8. I/We acknowledge Infinity Restoration is not responsible for any damages caused to us or to our property as a result of mold
and/or mildew resulting from water penetration that may develop as a result of the work performed by Infinity Restoration and
I/We agree to indemnify and hold harmless Infinity Restoration and its agents, contractors, subcontractors, assignees and
employees from any and all claims for damages of any type that may result to us, our heirs, assignees, successors in interest, guests
or invitees as a result of any act by Infinity Restoration, its agents, contractors, assignees and employees, including but not limited
to those of negligence, that cause of contribute to mold/mildew and its abatement, removal or cost of repair/replacement.
9. No painting or gutter work is included unless specifically noted.
10. The company shall not be liable for failure of performances due to labor controversies, strikes, fire, weather, inability to obtain
materials from usual sources, or any other circumstances beyond the control of the company whether of a similar nature.
11. This agreement constitutes the entire agreement between the parties. It may be changed by only written instrument signed by both
parties.
12. If any provisions of this agreement should be held to be invalid or unenforceable, the validity, and the enforceability of the
remaining provisions of this agreement shall not be affected thereby.
13. Any representatives, statements or other communications, not written in this contract are agreed to be immaterial, and not to be
relied on by either party, and do not survive the execution of this contract.
14. Additional materials are generally ordered for each job. Any and all materials delivered to your job site remain the property of
Infinity Restoration.
15. These conditions shall be considered a part of any contract entered into or authorized to proceed, the same as if they were included
therein.
16. In the event that any dispute arises under this contract homeowner agrees to allow Infinity Restoration an opportunity to cure such
defects. In the event that Infinity Restoration is not allowed an opportunity to cure such defects then homeowner agrees that
Infinity Restoration is not liable for cost of repair or damages resulting from such defects. In any event Infinity Restoration shall
not be liable for any consequential damages.
17. Customer acknowledges that no warranty, implied or explicit, is in effect until such time as Infinity Restoration, Inc. is paid in full
according to the terms of this contract.
GENERAL ROOF MAINTENANCE
REQUIRED BY THE BUILDING OWNER
The following conditions are unrelated to defects in the roof and thereof are not covered by the warranty. The maintenance tips provided
below will help you get the best service from your new roof.
1. If damage is caused by fire, lightening, windstorm, hailstorm, etc from notify your insurance company immediately and call
Infinity Restoration.
2. Accumulation of leaves or pine needles in gutters or roof valley produce what is known as a "waterdam." A waterdam can force
water under the roofing and produce leaks through the roof structure. These leaks can travel along roof joints, etc., and produce
interior damage at points far distant from the leak. Therefore, provisions should be made for the removal of the debris.
3. Adequate ventilation will keep attic temperature close to those outside, helping to prevent the deck delamination at the roof and the
formation of ice dams in gutters and valleys. Adequate ventilation is essential to the life of Asphalt Shingles. Adequate
ventilation (defined by FHA standards) consist of one square foot or more of open area for every 300 square feet or attic floor area
50% at or near the peak of the roof, 50% in the eaves or lower part of the roof. Lack of ventilation (or inadequate ventilation) will
result in moisture condensation and can cause rotting of the wood deck and/or blistering or curling of asphalt shingles which is not
a defect in the shingle but is a direct result of moisture being pulled through the shingle by the sun.
4. Installation or relocation of antennas or other equipment on the roof may cause leaks unless proper sealants are used at any point
where screws or other anchoring devices enter the roof. Avoid unnecessary roof traffic, since it can greatly diminish the life of the
roof.
5. Leaking can occur in or around the chimney unless it is periodically Maintained. Normal settling of your house through the years
will cause leaking conditions. In addition, the flashing compound around the chimney and vent pipe areas require normal
maintenance on a year to year basis. These areas should be inspected periodically for cracking or indications of weathering and
should be reflashed accordingly.
6. Roof should be properly maintained to prevent performance issues. It is required that you hire a certified roofer once every 12
months to perform an inspection and any necessary maintenance for your warranty to stay in affect.

approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and

complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required

I hereby authorize Infinity Restoration to discuss the details of the project with my insurance company on my behalf, as the contractor, and proceed with all
repairs authorized by the insurance company at a cost equal to the replacement cost value including overhead and profit allowed on the claim. Property
owner agrees that Infinity Restoration is entitled to the Replacement Cost value plus any and all supplements necessary to complete repairs that are
approved by the insurance company, this includes code upgrade requirements. First payment is due upon completion of the roof replacement, the balance is
due on work that has been completed, (such as the roof work) when the recoverable depreciation plus any approved supplements is received. Any
remaining unfinished work, such as gutters, siding, interior paint, or any other trade, will be due upon completion of each trade and receipt of insurance
funds. The Property owners out of pocket expense win not exceed the insurance deductible unless upgrades are agreed to in writing and added or
depreciation is "non recoverable". Deductible is to be paid directly to the contractor (Infinity Restoration). Any upgrades or downgrades will be in writing.

ADDITIONAL PROVISIONS
1. All proposals subject to approval by the Management of Infinity Restoration.
2. Should default be made in payment of this contract, charges shall be added from date thereof at a rate of one and one-half percent
(I %%) per month, 18% PER ANNUM or the maximum allowed by law with a minimum charge of $2.00 per month, and if placed
for collection customer agrees to pay all cost of such action as attorney and legal filing fees by accepting said contract.
3. Infinity Restoration is not responsible for any damage below the roof, or liability is limited to the roof repair only.
4. Infinity Restoration cannot be responsible for internal structural strength, so if you have any concerns or doubts about the strength
of your roof substructure to include rafters, beams, etc., it will be the homeowner's responsibility to either notify us or have
weakened conditions repaired before we start our work.
5. The company shall have no responsibility for damages from fire, windstorm or other hazard, as is normally covered by the
homeowner's insurance.
6. If you have exposed decking inside your building i.e. cathedral ceilings, you should be certain that we know, so proper nail length
can be used.
7. Replacement of deteriorated decking, fascia board, roof jacks, ventilators, flashing or other materials, unless otherwise stated in
this contract, are not included and will be charged as an extra on a time and material basis.
8. I/We acknowledge Infinity Restoration is not responsible for any damages caused to us or to our property as a result of mold
and/or mildew resulting from water penetration that may develop as a result of the work performed by Infinity Restoration and
I/We agree to indemnify and hold harmless Infinity Restoration and its agents, contractors, subcontractors, assignees and
employees from any and all claims for damages of any type that may result to us, our heirs, assignees, successors in interest, guests
or invitees as a result of any act by Infinity Restoration, its agents, contractors, assignees and employees, including but not limited
to those of negligence, that cause of contribute to mold/mildew and its abatement, removal or cost of repair/replacement.
9. No painting or gutter work is included unless specifically noted.
10. The company shall not be liable for failure of performances due to labor controversies, strikes, fire, weather, inability to obtain
materials from usual sources, or any other circumstances beyond the control of the company whether of a similar nature.
11. This agreement constitutes the entire agreement between the parties. It may be changed by only written instrument signed by both
parties.
12. If any provisions of this agreement should be held to be invalid or unenforceable, the validity, and the enforceability of the
remaining provisions of this agreement shall not be affected thereby.
13. Any representatives, statements or other communications, not written in this contract are agreed to be immaterial, and not to be
relied on by either party, and do not survive the execution of this contract.
14. Additional materials are generally ordered for each job. Any and all materials delivered to your job site remain the property of
Infinity Restoration.
15. These conditions shall be considered a part of any contract entered into or authorized to proceed, the same as if they were included
therein.
16. In the event that any dispute arises under this contract homeowner agrees to allow Infinity Restoration an opportunity to cure such
defects. In the event that Infinity Restoration is not allowed an opportunity to cure such defects then homeowner agrees that
Infinity Restoration is not liable for cost of repair or damages resulting from such defects. In any event Infinity Restoration shall
not be liable for any consequential damages.
17. Customer acknowledges that no warranty, implied or explicit, is in effect until such time as Infinity Restoration, Inc. is paid in full
according to the terms of this contract.
GENERAL ROOF MAINTENANCE
REQUIRED BY THE BUILDING OWNER
The following conditions are unrelated to defects in the roof and thereof are not covered by the warranty. The maintenance tips provided
below will help you get the best service from your new roof.
1. If damage is caused by fire, lightening, windstorm, hailstorm, etc from notify your insurance company immediately and call
Infinity Restoration.
2. Accumulation of leaves or pine needles in gutters or roof valley produce what is known as a "waterdam." A waterdam can force
water under the roofing and produce leaks through the roof structure. These leaks can travel along roof joints, etc., and produce
interior damage at points far distant from the leak. Therefore, provisions should be made for the removal of the debris.
3. Adequate ventilation will keep attic temperature close to those outside, helping to prevent the deck delamination at the roof and the
formation of ice dams in gutters and valleys. Adequate ventilation is essential to the life of Asphalt Shingles. Adequate
ventilation (defined by FHA standards) consist of one square foot or more of open area for every 300 square feet or attic floor area
50% at or near the peak of the roof, 50% in the eaves or lower part of the roof. Lack of ventilation (or inadequate ventilation) will
result in moisture condensation and can cause rotting of the wood deck and/or blistering or curling of asphalt shingles which is not
a defect in the shingle but is a direct result of moisture being pulled through the shingle by the sun.
4. Installation or relocation of antennas or other equipment on the roof may cause leaks unless proper sealants are used at any point
where screws or other anchoring devices enter the roof. Avoid unnecessary roof traffic, since it can greatly diminish the life of the
roof.
5. Leaking can occur in or around the chimney unless it is periodically Maintained. Normal settling of your house through the years
will cause leaking conditions. In addition, the flashing compound around the chimney and vent pipe areas require normal
maintenance on a year to year basis. These areas should be inspected periodically for cracking or indications of weathering and
should be reflashed accordingly.
6. Roof should be properly maintained to prevent performance issues. It is required that you hire a certified roofer once every 12
months to perform an inspection and any necessary maintenance for your warranty to stay in affect.
