We witnessed the final installation ofthe repairs on January 19, 1996 and this report is based on that inspection and the repair design provided by our office, dated November 2, 1995. The subject property suffered from localized settlement of the foundation system due to loss ofload bearing capacity ofthe local soils, located on the south side ofthe home (gazage), and at the northwest corner of the home. Evidence of tlus condition included earterior cracking ofthe brick veneer and a loss ofrelative elevation. A repair design was performed to install steel piers at the affected azeas. These piers were installed at engineered locations such that their load bearing capacity exceeded the combined live and dead loads of the structure. Our office monitored this installation. The repairs were performed with regazd to the repair design in a sound and competent inanuer. All piers were installed to specification and in locations satisfactory for proper stabilization and support. There were no exceptions to our requirements for craftsmanship or quality of materials. Summarily, the engineer Snds that the repairs to the subject building are in accordance to the repair design, and that the repair is satisfactory to preserve the safety ofthe inhabitants and the welfare ofthe building owner. Principal i~ DC7/ attachments 0 STRUCTiJRAL INSPECTION AND INVESTIGATION Homeawner: Don van Winkle Address: 3070 Reed Street Wheatridge, CO ORIENTATION The subject residence is a one story home located in Wheatridge, Colorado. The home is of solid masonry construction, including load bearing perimeter of the home. There is an attached garage which appears to be part ofthe original constmction ofthe home. The home is built on a relatively flat lot in which little if any cut and Sll was required at the time of original constnxction. For the purposes ofthis report, the home is considered to west. The foundation of this residence is a cast in place concrete foundation wall that creates a fixll height basement. The garage is buik on a continuous cast in place concrete grade beam when not common to the residence. The floor of the garage, as well as the basement, is a cast in place concrete slab-on-grade wluch are separate pours fromthe foundation system. The basement is finished. It is reported to us that the garage foundation wall is of fiill depth as the basement, however, no excavation was performed to verify this. It is reported to us by the homeowner that this home was built over a former landfill. There are no records of the type of landfill tlus was, such as the type of debris that may be found under the home. Contractors should note this when planviug for repair methods. DISTRESSES The call to our office was with regard to settlement in the garage azea of the home. The inspection of the subject building revealed the following distresses: Exterior 1. Diagonal and vertical cracks in brick wall - 3/4" 2. Horizontal crack in brick wall - 1/16" 3. Vertical crack in brick wall - 1/16" 4. Diagonal crack in brick wall - hairline 5. Diagonal crack in brick wall, with lateral roll - previously patched 6. Horizontal crack in brick wall - 1/16" Interior 1. Horizontal and vertical cracks in foundation wall - previously patched 2. Diagonal crack in foundation wall - previously patched 3. Horizontal crack in foundation wall - previously patched 4. Horizontal crack in foundation wall - previously patched 5. Horizontal compression cracks in basement staircase partition walls LJ An illustration ofthe location ofthese distresses is shown on Drawing 95-1289A. DISCUSSION The distresses in the subject residence are common and typical for a home suffering from a vertical displacement of the foundation systems. In order to verify and quantify Uus condition, a relative elevation survey was performed. The relative elevation survey was done with the use of a laser transit, which is ideally suited to tlus application. Measurements were taken in the basement with an inverted rod, and adjusted to a common datum. The results of the relative elevation survey indicate that there is some vertical movement of separate portions of the home. The area of largest vertical movement is the south, or gazage, end of the residence. Here; it was found that the outboard wall of the garage has settled up to 4 inches relative to the inboard (common) wall ofthe garage, and 7 inches from the opposing corner ofthe garage. This was consistent from &ont to back in the garage, and at the soffit of the garage. The portion ofthe foundation wall north ofthe garage and at the front ofthe home is also experiencing some vertical displacement. Relative elevations were taken from inside the basement, as well as the exterior ofthe home. Here, displacement is radically occurring at the front ofthe home, gradually decreasing to the north. The northwest comer of the home is also suffering from negative vertical displacement. At this comer, the total loss of elevation is 3/4 inch. It is reported to us that the home has had some foundation repairs made in the past. These include 3 each cast in place concrete caissons angled at the front of the home near the garage. Also, the sole plate far the wall framing has been shimmed in an attempt to minimize the visible damage from the foundation settlement. CONCLUSIONS It is our considered opinion that the subject residence foundation is suffering from vertical settlement due to an inability ofthe load bearing soil to support the structure. The physical evidence indicates that the condition is dynamic, and it is expected to propagate with time. The extent of the affected area is the south side of the residence, including the gazage area, the southwest comer ofthe residence, and the northwest comer ofthe home. We recommend remedial action to preserve the value and livability of the residence. i,J (V REMEDIES To stabilize the foundation of this home, and to restore as much elevation as practicable, we recommend that the distress be remedied through the use of steel piers. These piers shall be placed adjacent to the foundation system ofthe building and driven hydraulically into the soil until load bearing strata is aclueved. The steel piers shall be located such that their total load bearing capacity exceeds the total of dead and live loads for the structure. The piers shall be located at all affected areas. REPAIR SPECIFICATIONS Repair specifications for the correction of tlus building or building component include the installauon of steel piering and subsequent stabilization of the gazage grade beam, the common wall between the garage and the residence, the foundation wall adjacent to the residence, and the northwest foundation wall corner. Tnstallation of steel piering shall occur outboard of the residence foundation wall as indicated by Drawing 95-1289B. Contractor shall jack against the foundation ofthe home and garage to install steel piers per GENERAI. NOTES. Contractor shall field verify which type of foundation support is used. If caisson and void system is used under foundation, contractor shall clip and abandon caissons after preloading steel piers. Contractor shall use as wall loading as follows: TOTAL LOAD = 2,061 PLF when applying proprietary vertical jacking system. Calculations are based upon solid masonry walls, 8 foot foundation walls, single story, and a snow load appropriate for this region. Calculations are the property of the client and the Engineer, and can be requested only through the Engineer. With regard to pier spacing, a Safety Factor is increased by 51 % in order to provide for substantial shear strength for the foundation. Recommended locations for the steel piering is indicated on Drawing 95-1289B. Steel piers shall be installed on the exterior of the home in order to use normal bracketry and therefore minimi~.e customer cost. Contractor may field located some piers to avoid previous foundation repair systems, as applicable. Unless otherwise noted, all conditions of the attached GENERAL NOTES shall be observed. Contractor shall adhere to the GENERAL NOTES and the Work Description except by written and prior notice ofthe Engineer. Contractor is responsible for job safety. Temporary shoring, bracing, and lifting or jacking devices shall also be provided by the cantractoc All materials and workmanship aze subject to the approval of the representative of Criterium - Johanson Engineers, hereafter known as Engineer. 2. All laws of the State of Colorado, including but not limited to Workman's Compensation laws, shall be adhered to in the course of work on this project. Excavation support, scaffolding lifting, and the use of power movement devices shall be performed in accordance with OSHA Safety and Health Standards for We Construction Industry (29 CFR Part 1926, Amended June 1, 1989) and applicable local building codes. 3. The Uniform Building Code, with any local admendments, shall be adhered to in the course of new cons[ruction when applicable in the described project. 4. While data, information, condition descriptions, material conditions and/or quantities, and structural member sizes contained in this repaidconstruction plan may be used to describe the scope of repairs and provide general information to the contractor for bid putposes, the contractor shall verify and perform an evaluation of conditions and material conditions prior to submitting bid(s). The entire extent of the work required and necessary equipment and materials for completion of the work are the responsibility of the contractor. 5. The contractor shall exercise due caze while lceating and auoiding problems with buried utilities when excavating, if necessary in the course of this project. 6. When bacldill of and excavation is necessary, the bacldill shall be performed in no more than 6" lifts, with compaction at each lift. Backf'ill material shall be of non-expansive soils, unless otherwise noted. Z Concrete shall be provided by the contractor or the contractor's supplier. Concrete shall cylinder test at 3500 psi compressive strength at 28 days. Compressive strength that achieves 3500 psi at less that 28 days shall be considered acceptable. Grout used for securing of structural members shall be of non- shrinking varieTy. Where concrete or grout is placed onto exis[ing concrete, the existing surface shall be prepazed and a bonding agent shall be used prior to placement of the new material. Reinforcing bazs for cast in place concrete shall conform to ASTM A615, Grade 60 or better. Welding of reinforcing bars is strictly prohibited without the consent and supervision of the Engineer. In this case, welding shall be performed by a welding operator certified on the procedure. All welding shall be performed by ceRified welding operators,and in accordance with the American Welding Society D11 - 94. Welding procedure specifications, procedure qualification records, and operator quati5cation and continuity records shall be provided to the engineer upon request. All welding shall incorporate the use of filler materiat. The use of short-cirwit metal inert gas welding is sMc[ly prohibited. 9. Foundation cracks of a size 1/16" and larger shall be repaired by epoxy injection. Cracks shall be completely filled with Sikadur 35 or equal, as approved by the Engineer. Resulting surface shall be grinded to original surface level. 10. Repair of damaged exterior landscaping shall be the responsibility of the contractor. All exterior landscaping shall be replaced to original condition, or to the satisfaction of the homeowner. Original slope away from residences shall be maintained, unless othenvise noted. 11. Repair of damaged paint, textures, finishes, equipment, and fixtures, shall be the responsibility of the contractor unless otherwise noted. Replacement for these shall be of equal or better quality as those in ~ existence. ~ ~ PROFESSIONAL QUALIFICATIONSAND EXPERIENCE CHRISTIAN JOHANSON, P.E. Area of Expertise Christian Johanson is a registered Professional Engineer with excellent experience for your home and building engineering needs, including diagnostics, inspections, analysis, maintenance planning, and design. During the past fifteen yeazs, his experience has ranged from construction, inspection, and structural repair of individual farruty dwellings to operation and maintenance of one-half billion dollaz industrial facilities and to construction and quality assurance in commercial fabrication. As Principal of Criterium - Johanson Engineers, he is responsible for the developmen[ and management of the firm. He is actively involved in serving clients in need of building evaluation, investigative engineering, site planning, structural repairs and design, and maintenance planning. He also coordinates the activities of the Srm with Criterium Engineers, a nationwide building specialty engineering firm that has been listed as a Top 500 firm on a national basis for the last three yeazs. Qualitications Since establishing the firm, Mr. Johanson has personally inspected and investigated hundreds of homes and commercial buildings wer the entire Colorado front range. Typical investigations include pre- purchase inspections, foundation failure analysis, foundation repair design, insurance invesUgations, expert testimony and arbitraGon, and design consultapon. Prior to establishing his own firm, Mr. Johanson was among the senior technical staff at City Public Service, a 3500 person electric and gas uGlity in San Mronio, Texas. In that capacity, he was the lead engineer for construction and overall quality of a new $444,000,000 power plant, as well as other projects. Eazlier in his cazeer, Mc Johanson served as Maintenance Supervisor with Texas Utilities at various powEr facilities throughout the southwest. There, he was responsible for staff and operating personnel, yearty budgets exceeding $20,000,000 and design upgrades. Throughout his adult life, Mr. Johanson has never been too faz from the building trades, however. At vazious times, he has had hands-on experience as a framing and trim carpenter, roofer, plumber, concrete installer, brick mason, and finalty as a general contractor. He has been involved with virtually all types of construction, including conventional or stick-built, metal frame, tilt wall, and conventional masonry. Education Mr. Johanson earned dual Bachelor of Science degrees in Bicengineering (engineering sciences relating to human physiology) and Mechanical Engineering from Texas A&M University in 1979 and 1980, respec[ively. He is a registered Professional Engineer in Colorado (29671) and Texas (65818).
