This letter is to advise that the modified bitumen roof installation completed at: 4625 Reed St (permit #
201705975) was installed as per Mule Hide Roofing Products, manufacturer specifications for modified bitumen
base and cap sheet. Product data sheets are included for your convenience.
Manufacturer installation instructions:
• Apply over clean, dry, dust and debris free substrates.
• Concrete decks must first be primed with Mule -Hide 121 asphalt primer
• When re -roofing, remove all existing roofing materials down to a clean, dry substrate. Ensure that all abandoned
roof projections are properly closed off.
• Concrete and steel roof decks are to be designed with proper expansion joints.
• Wood decks should have all joints blocked and properly supported.
• Verify the fire rating of the roofing assembly over any combustible substrate.
• Ensure that the installation of APP Torch 5 does not prevent the ventilation of the existing construction.
• Do not apply over shingles or any granulated surface.
• When installing APP Torch S
o Start at the low point of the roof.
o Unroll the material and allow to relax in the sun for at least 15 minutes.
o Install with traditional torch roofing techniques ensuring proper heating of the roofing material as not to
expose the reinforcement.
o Do not heat the substrate.
o Position successive rolls providing a minimum 6" end lap and 3" side lap. Asphalt bleed out shall be Y" to
3/8" on all seams.
o It is suggested but not mandatory that laps shall be rolled with a 6" wide roller immediately after heat -
welding.
• Details and flashings may be installed using APP Torch S with torch applied techniques. Do not use cold adhesives or
hot asphalt. Check project details for proper installation requirements.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Any Material Warranty is non -transferable unless otherwise stated by the manufacturer.

sheeting. A to Z Roofing is not responsible for exposed caves that nails penetrate.

no additional cost to you, except for your deductible and any upgrades or additional work authorized by you that are not part of your approved insurance claim. Should your insurance
company deny a full roof replacement, you will not be obligated to this contract and may enter into an acceptable repair agreement with A TO Z ROOFING.
ACCEPTANCE OF PROPOSAL— This proposal becomes a legal binding contract 72 hours from date of
acceptance. 1 'A% per month interest will be added on to all accounts 30 days past due. In addition, costs and
reasonable attontey's fee for collections may be charged on any account past due over 30 days. Payment will
be made as outlined above. Failure to pay can constitute a mechanics lien filed against property. Cancellation
of the contract may be achieved by paying a cancellation fee in the amount of 5 % of the contract price. All
returned checks are subject to a returned check charge fee.
Customer is responsible for payment as outlined in this contract and payment to A to Z Roofing is not
contingent upon government inspection and / or payment from his or her Insurance company.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
