In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
Mid -roof inspections may be called in at the same time, 100 percent of the sheathing must
be complete and 25-75 percent of the midroof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. A ladder extending 3
feet above the roof eave and secured in place is required to be provided for all roof
inspections. Roof ventilation is required to comply with applicable codes and/or
manufacturer installation instructions, whichever is more stringent.

This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the original permit fee.
If this permit expires, a new permit may be required to be obtained. issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
No work of an manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any
applicable code or any ordinance o1 regulation of this jurisdiction. Approval of work is subject to field inspection.

The Fire•Lite MS-9200 is a compact, cost-effeclive, ad-
dressable fire alarm control panel with a capacity of 198
Fire•Llte 300 Series devices. A single Signaling Line Cir-
cuit (SLC) loop supports up to 99 smoke detectors and 99
control or monitor modules. The panel uses suAace-mount
technology and is designed for ease of installation and
programming. It features the latest in fire proteclion tech-
nology, including ma(ntenance alert and automatic detector
test. Its new, larger enclosure is capable of housing 12.0
AH batleries.
New as of September 1, 2001, the MS-9200 will include on
the motherboard (Rev. PC-D) built-in intedaces for a printer
and remote annunciators.

The SLC requires use of a speci(ic wire type to ensure proper circuit operation. It is recom-
mended ihat all SLC wiring be twisted-pair shielded to minimize the effects of electrical
interference. Wire size should be no smaller than 18 AWG (0.75 mm') and no larger Ihan 12
AWG (3.25 mm') wire.
The wire size depends on the length of the SLC circuit. Use the table below to determine the
specific wiring requirements for the SLC.

GENERAL
The BG-10LX is an addressable manual pull slation with
a key-lock rese[ feature. Il provides the MS-9200 with one
addressable alarm initialing input.
NOTE: The BG-fOLX has been replaced with the BG-12LX,
described in data sheet DF52013.

(models manutaclured after 10-1-96, identified by an A
under station spring).

• Easily operated (dual-aclion).
• Attractive shape and lextured finish.
• Semi-tlush, mounts to a standard single-gang (2.125'
minimum depth), double-gang, or 4' square electrical
box.
• Operales with or without a crush tube.
• Handle latches in the down position to clearly indicate
the station has been operated.
• Key reset.
• Screw terminals for easy connection to SLC Loop Chan-
nel.

CONSTRUCTION
• Shell, door, and handle are molded of durable LEXANS
(or polycarbonate equivalent) with a textured finish.
• 16 gauge steel back plate.
OPERATION
Pulling the handle down causes it lo latch in the down
position. The handle is restored manually by using a
mechanical key which allows the lop of the case lo pivol
forward and the spring•loaded handle to return to its nor-
mal position. The case can lhen be pivoted back lo its
normal position and secured with the key.
Manual stations connect with lwo wires to the conlrol panel
SLC loop. Each manuai slation, on command from the
control panel, sends dala lo the panel representing the
state oi the manual swilch. A pair of rotary decimal
switches allows two-digit address setting.

The American National Standards Instiwte and the National Fire Alarm
Code require that all horns used for building evacuation installed atter July
t, 1996, must produce Temporal Coded Signals.

This letter is reference our conversation on December 4, 2002, regarding the
revision of the kitchen arrangement and the need to upgrade the &re alann system currendy
installed in your restaurant located at 3240 Young6eld, Wheat Ridge, Colorado. As we discussed,
the revision will entail the following items:
The current fire alarm system needs to be replaced with an addressable fire alarm
panel separated from the burglar alarm system. The fire alazm system must be
upgaded to include manual fire alarm pull stations at all exits with "screamer
covers" over each pull station. The flow switch acid tamper switches on the
sprinkier system must be sepazated and identified. The hood and duct protection
systems for the kitchen hoods must be separated and individually identified. The
fire alarm system must be monitored by an outside agency on a continual 24-hour
basis. Plans must be submitted and approved prior to installation. Fire Pemut &
Building Department pemrit is tequired.
2. The revision of the kitchen will require an upgrade to the Sre suppression system
curremly installed in the hood and duct at this time. Plans must be submitted and
approued prior to revision of the system. Fire Permit & Building Depardnent
permit is required.
3. Issues and or items not addressed will be addressed as need arises.
I have spoken with Brad at Sentty Fire & Safety regazding the above issues and he
understands what needs to be done in order to bring the restaurant into compliance with the
cucrent codes. I am not requiring complete detection of the building due to its' size and layout. I
believe that additional detecrion would cause unnecessary false alazms and an undesirable
imerruption to business that is unnecessary.
