Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent- of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

THIS AGREEMENT made August 22, 2017 by and between FRANK MONTOYA hereinafter referred to as
"Owner", and COLORADO CUSTOM TRIM, INC hereinafter referred to as "Contractor."
WITNESSETH, That the Owner and Contractor for the consideration stated herein, mutually agree as follows:
1. Statement of Work. The Contractor agrees to furnish all labor, materials, equipment and services, and to perform
and complete all work required for the rehabilitation of the property located at and known as:
2905 Webster Street, Wheat Ridge, CO 80033
in strict accordance with the Description of Work, and any plans and specifications attached as Exhibit A, which are
incorporated herein by reference and made a part hereof. The work is to be performed within the requirements of the
Homeowner Housing Rehab Program as set forth below and as administered by the Jefferson County Housing
Authority, hereinafter referred to as "JCHA". The Agent of the JCHA shall be the Housing Rehab Specialist or Project
Manager as assigned.
2. The Contract Price. The Owner shall pay the Contractor for the performance of the contract, subject to additions
and deductions as provided for in the Contract, the total sum of :

3. Commencement and Completion. The Contractor agrees to properly and promptly complete all the work required
under this contract no later than
Sunday, November 26, 2017, which is 90 days after the Notice to Proceed.
The above Contract date shall suffice as the Notice to Proceed date unless a different date and Notice to Proceed is
issued in writing. Work shall commence not later than 10 calendar days after the Notice to Proceed date. Awritten
schedule of work that is agreed to by the Owner maybe substituted for actual start of work within the 10 days. No
time extensions will be granted unless delay arises from unforeseen causes beyond the control and without the fault
or negligence of the Contractor and as approved by written Change Order. Therefore, time is of the essence. The
work being done, or any part thereof, shall not be deemed completed until the same has been accepted as
satisfactory to the Contract by the Owner and the JCHA. The Contractor shall not be entitled to damages for Owner
caused delays but may receive time extensions.
4. Standard of Work. The Contractor does hereby agree to perform the work diligently and in a workmanlike manner,
using the materials specified or of equal quality approved by the Owner and JCHA. All equipment, material and
articles furnished under this Contract shall be new and of standard grade or better, unless otherwise specifically
provided in this Contract. The Contractor shall at his own expense, secure all necessary permits, licenses, and other
authorization as may be required by any governmental entity, by any applicable code compliance, or by the JCHA and
pay all fees therefore, as may be necessary for the work covered by this Contract. All work shall be done in
accordance with all applicable ordinances, codes, and property rehabilitation standards of the JCHA and local
authorities involved as well as all applicable federal and/or state requirements (see Attachment B). The Contractor
shall pay all sales and use taxes upon all of the materials furnished pursuant to this Contract as required by
appropriate governmental authority.

Item Description Total Cost
1 Complete Roof Replacement (including re -decking): Remove the existing roofing on the house
and carport down to the decking board (was original 3 tab asphalt shingles) and re -deck the roof
(existing deck is spaced 1" x 6" boards). Haul from site and properly dispose of all debris
generated on a daily basis. Install 2 courses of Ice & Water Shield (up from eave fascia) across
bottom roof area of each roof deck. Provide and install synthetic felt (or new 301b ASTM felt base
underlayment) to remainder of roof. Each penetration (pipes, skylights, chimney, etc.) to get new
flashing. Include new drip -edge, roof jacks and new gable flashing and ridge cap as needed.
Satellite dish is to be removed and replaced. Roofing shall be Tarko 30 year dimensional
("shadowed") asphalt shingles (or approved equal). Color as selected by Owner from stock
available. New shingles and attachment shall comply with ASTM standards for wind resistance to
100mph or local requirements, moderate hail exposure requirements. All components shall be
installed according to IRC, local code and manufacture's recommendation.
The two sheds that need to be roofed are to have rolled roofing if possible on the most westerly
shed and dimensional shingles on the other.
Owners preference is for Owens-Corning 30 year dimensional shingles.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
