1. This permit was issued based on the information provided in the permit application and accompanying pplans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

CONSTRUCTION OBSERVATION REPORT
Dear Louann;
On August 11, 2017 Gary Cantrell, one our engineers, inspected the installation of the helical
pier that is part of the structural repairs at the above referenced property. The purpose of the
inspection was to determine if the pier was installed in compliance with the repair design of this
firm, dated May 23, 2017.
COMPLIANT WORK:
• The helical pier had been installed and attached to the foundation below the masonry
"pillar" located between the two garage doors.
• Pier depth and pressure charts were provided to the engineer, on Jan 11, 2018,
demonstrating installation depth and capacity.
CONCLUSION:
• Based on the observations made and information provided, it is my opinion that the work
completed to date is in substantial compliance with the repair design.
LIMITATIONS:
• This inspection was visual in nature but represents the professional opinions of this firm based
on visible components, observations made, and assumed accuracy of information provided.
• As indicated in the repair design, no warranties are made or implied, as soil/material condi-
tions are unpredictable and unknown/unforeseeable conditions could exist.

inspection, conducted when 25-75 percent of the roof covering is installed, prior to inal

required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice ana
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

STRUCTURAL REPAIR SPECIFICATIONS:
CITY OF WHEAT RIDGE CODE DESIGN CRITERIA:

UTILITY LOCATES:
• All utilities to be located by the contractor, before commencing with any excavation, includ-
ing but not limited to underground electric, gas, water, sewer, cable, phone.
• Contractor to notify the engineer of any utility conflicts with the proposed repairs, to allow
for modification to the design as needed.
• Contractor assumes all risk and responsibility for protecting all utilities. Coyle Structural In-
spection Engineers is not responsible for any damages to any utilities caused by the contrac-
tor.
STEEL HELICAL PIERS:
• FOUNDATION SUPPORTWORKS: Model 288 (round shaft) Helical Pile with FS 288B
Retrofit Bracket. Alternates - Model H150 (1.5" solid square steel shaft) with 8" Helix and
FS 288B Retrofit Bracket. Model H175 (1.75" solid square steel shaft) with 8" Helix and FS
288B Retrofit Bracket.
• AB CHANCE (Hubbell Power Systems): Model RS 2875 (round shaft) Helical Pile with
Chance Standard Remedial Repair Bracket. Alternate - Model SS 175 (square shaft) Helical
Pile
• Piers shall be 8" (minimum) HELICAL PIERS, installed at locations shown on the diagrams.
• Piers to be installed to a minimum safe working capacity of 20,000 lbs (20 KIPS) where safe
working capacity shall be no less than 50% of installed resistance (i.e. 40,000 lbs (40 KIPS)
for a 2:1 factor of safety). Typical torque to resistance relationships result in a torque installa-
tion requirement of 4,000 FT -LBS.
• Installation torque serves as emperical verification of pier capacity, yielding an onsite load
test for each pier. Therefore, soils testing is not necessary for generating a theoretical ca-
pacity.
• It is assumed that each new pier will achieve penetration into bedrock or competent bearing
soil, and can be installed to the specified capacity. Should bedrock or competent bearing soil
(specified capacity) not be reached within reasonable depth/distance, this office should be no-
tified for further instruction.
• Footings shall be clipped flush to the foundation wall plane, at the pier locations.
• Any encountered caissons are to be reported to the engineer for further instruction and possi-
ble need for abandonment.
• Contractor to maintain accurate pier depth and pressure/torque charts for each pier, showing
depth and pressure/torque at each pier shaft section. Charts shall be filled out in real time dur-
ing installation, and maintained onsite for engineer review during inspection. Charts shall be
provided to engineer in final form upon completion.

STRUCTURAL REPAIR NOTES FOR THE HOMEOWNER:
• CONTRACTOR SELECTION and VERIFICATION: Structural repairs should only be per-
formed by qualified contractors with significant experience. The homeowner is solely responsible for
bidding, contractor selection, verification of contractor's experience, licensing, insurance, warranty
program, etc. This contractor selection and verification process is not part of the scope of services
provided by Coyle Structural Inspection Engineers.
• FOUNDATION STABILIZATION: It should be understood that underpinning/stabilization of part
of the foundation will minimize future movement in the area stabilized, but will not necessarily pre-

• FOUNDATION ADJUSTMENT: Some incremental adjustments (lifting/lowering/realignment) of
the affected area may be attempted with the owner's approval. It must be understood that adjust-
ments could cause additional damage to finished surfaces and foundation walls. The owner is encour-
aged to be present at the time of adjustments in order to authorize the amount of adjustment to be at-
tempted. Coyle Structural Inspection Engineers shall not be responsible for adjustment authorization
or for any damages caused by attempted adjustments.
• GOOD DRAINAGE PRACTICE: Poor drainage has the potential to cause structural damage and
water intrusion. The owner is advised to improve and/or maintain drainage per the guidelines below,
in order to promote long term structural integrity for the property.
GOOD DRAINAGE PRACTICE (for the homeowner):
P Providing and/or maintaining good drainage is recommended in Colorado, in order to promote stabili-
zation of moisture content in the foundation bearing soil. Stability of moisture conditions at founda-
tion depth is considered essential toward promoting long term structural integrity of all homes.
These drainage remarks are general in nature to all homes in the area, and not necessarily site specific,
as each site has unique constraints. In some cases, consultation with a land surveyor may be necessary
in order to determine a detailed scope of drainage improvement, based on site constraints.
P Provide/maintain positive drainage away from the foundation. Building Codes typically require a min-
iinum of 2% slope (1/4" per foot) and should be considered an absolute minimum. Ideally, 6" of fall
in the first 5' is recommended by most soils engineers in the Colorado Front Range area with expan-
sive soil conditions, for landscape areas, and then 2% (1/4" per foot thereafter). However, care must
be taken not to raise the soil too high relative to siding/framing. Several inches of clearance should be
maintained to deter moisture and pest access. If clearance cannot be maintained, then cutting the soil
down and away from the foundation may be necessary in lieu of adding soil. In this case landscape
drainage swales and/or french drains may need to be created in order to convey the drainage in a con-
tinuous manner. In some cases, addition of sidewalk "skirts" or hardscape patios may be an alterna-
tive.
Hardscape areas such as driveways, patios, sidewalks, etc. should slope away from the foundation, at
a minimum of 1% slope (1/8" per foot) and should be installed at an elevation that does not act as a
barrier to drainage. Where abutting the foundation, the hardscape should be well sealed to prevent
moisture penetration. It is not recommended to attach hardscapes to the foundation via rebar pinning.
Construct or improve landscape swales, as needed, to route the drainage around the foundation. The
flowlines of the swales should have a slope of at least 2% (1/4" per foot). In areas where 2% cannot
be attained, a minimum slope of 1 % is acceptable if a French drain is incorporated in the flow

• Determine the locations of any and all active sprinkler heads, and move as needed to a distance of 5
feet from the foundation as a minimum, and orient so that spray does not fall within 5 feet of the foun-
dation. Drip irrigation and/or "misting" heads are generally considered to be acceptable as long as
they are used sparingly. Maintain or create a 5 foot wide non -irrigated landscape buffer around the
foundation.
• Replace or modify any solid landscape edging to allow drainage to pass thru the edging. Remove any
plastic weed barriers adjacent to the foundation (within 5 feet). If a weed barrier is desired, use a geo-
textile mesh landscape fabric that allows the soil to evaporate its excess moisture.

Dear Louann:
On April 21, 2017, I inspected the above referenced property at your request. The purpose of the inspec-
tion was to visually observe the foundation of the garage to determine my professional opinion of the
structural condition thereof, in order to determine whether structural repairs are needed at this time, and
to recommend a plan of repair as needed. The inspection did not involve any other system or component
of the residence. No soil or other material sampling, testing, or analysis was performed by Coyle Struc-
tural Inspection Engineers.
General Information:
• The home is a 1 story single family home structure, assumed to be facing East. The home was re-
portedly constructed in 1958.
• The foundation is of poured concrete construction.
• The perimeter walls of the upper structure are of brick and block construction.
• No soils report or other architectural/engineering documents were available for review.
• I understand that you are concerned with brick cracking at the garage. This report and repair design is
relative to the garage only, as the remainder of the house was not inspected.
Observations, Opinions and Recommendations:
• Structural settlement has occurred to the brick and block masonry "pillar" between the two garage
doors. Both garage door openings slope downward toward the center pillar.
• Settlement of the foundation below this area is responsible for the distress observed. The condition of
the foundation of below -grade could not be determined.
• All other walls of the garage appeared to be in good condition.
• The garage floor slab and the driveway have experienced minor settlement and cracking, but nothing
unusual or excessive.
• It is recommended to install a single steel, helical pier on the foundation at the pillar between the two
garage doors. This will require concrete removal and excavation at the pillar location, at which time
the condition of the foundation below grade should be determined. It is assumed that the foundation
will be competent to accommodate the pier. However, in the event that the foundation is deemed in-
competent at this location, a second pier may be necessary, with crossbeam between piers.
• At your discretion, though not structurally required, the garage floor slab and the driveway could be��
mudjacked or foam injected to lift the settled areas. ��cv

• I understand that you are concerned with brick cracking at the garage. This report and repair design is
relative to the garage only, as the remainder of the house was not inspected.
Observations, Opinions and Recommendations:
• Structural settlement has occurred to the brick and block masonry "pillar" between the two garage
doors. Both garage door openings slope downward toward the center pillar.
• Settlement of the foundation below this area is responsible for the distress observed. The condition of
the foundation of below -grade could not be determined.
• All other walls of the garage appeared to be in good condition.
• The garage floor slab and the driveway have experienced minor settlement and cracking, but nothing
unusual or excessive.
• It is recommended to install a single steel, helical pier on the foundation at the pillar between the two
garage doors. This will require concrete removal and excavation at the pillar location, at which time
the condition of the foundation below grade should be determined. It is assumed that the foundation
will be competent to accommodate the pier. However, in the event that the foundation is deemed in-
competent at this location, a second pier may be necessary, with crossbeam between piers.
• At your discretion, though not structurally required, the garage floor slab and the driveway could b%q

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and

?Sc By approving this agreement, the Homeowner authorizes Gen 3 to represent the best interests of the Horrlner for restoration
of storm damage. This contract does not obligate the Homeowner or Gen 3 unless payment for damage is approved by the insurance
company and accepted by Gen 3 following approval of claim. Gen 3 will complete specified work for a price agreeable to the
insurance company and to Gen 3 with no additional cost to the Homeowner except for the deductible. This agreed upon amount shall
become the final contract price, and Gen 3 will receive all insurance proceeds for work completed by Gen 3.
RESIDENTIAL ROOFING AGREEMENT TERMS AND CONDITIONS
1. Right of Approval. This Agreement is subject to the review and approval of Exterior
Solution's management. Any prices quote will remain valid for sixty (60) days from the date
set forth above. Homeowner will indemnity and hold Gen 3 harmless for any proposed or
understood changes to the Agreement not written on the signed copy of the Agreement.
Homeowner is responsible for obtaining any and all applicable product and related color
approvals from any relevant Homeowners Association(s) and Historical Societies, if any.
2. Exterior Solution's Additional Obligations. Upon all preconditions being met, Gen 3 shall
start and diligently pursue work through its completion. In addition to completing the work
described in this Agreement, Gen 3 shall: make reasonable efforts to protect the property
from any further damages and remove equipment, debris and all other material relating to
the services provided hereunder.
3. Homeowner Responsibility. In order for Gen 3 to safely gain access to the property and
install the roof, Homeowner will be required to undertake those actions: clear driveway for
delivery and install; remove breakable items from walls and shelves (Gen 3 is NOT
responsible for broken items due to vibrations during install); remove all items (to the extent
possible) around the property that may impede Gen 3' access to the property and the roof;
take precautions with animals; and take precautions with small children.
4. Subcontracting. Gen 3 may subcontract portions of this work to properly licensed and
qualified subcontractors.
5. Change Orders. No Change Orders shall be binding on Gen 3 unless Gen 3 has
approved of such Change Order in a writing executed by an authorized Gen 3
representative and approved by Gen 3' management. The purpose of such requirement is
to ensure that both Gen 3 and Homeowner are on the same page as to parties' respective
obligations and the costs and expenses to be incurred in fulfilling the parties' obligations
hereunder.
6. Payments. Gen 3 is acknowledge to be a general contractor and is entitled to receive
all overhead and profit charges applicable to an insurance claim. All payments should be
made payable to Gen 3 Plus, Inc., with no payments to be made in cash.
7. Interest/Lien Rights. In the event Homeowner does not pay all amounts due to Gen 3
within thirty (30) days following the due date as set forth in an invoice issued to Homeowner
in accordance with Gen 3' general practice, the outstanding balance shall accrue simple
interest at a rate of 1.5% per month (18% per annum), with such interest being calculated
from the original due date of the outstanding balance. Additionally, should payments be
more than thirty (30) days past due, Gen 3 shall have the right to place a mechanic's lien
on the property and to pursue the collection of funds from the Homeowner.
8. Indemnification. Homeowner will indemnify and hold Gen 3 harmless for. (a) any
damages to personal property or physical injury as a result of vibrations caused during the
execution of the work described in this Agreement; (b) pre-existing damage and
deterioration; (c) improperly installed concrete and/or soils; (d) minor interior damage (i.e.,
nail pops, cracks in plaster, drywall or paint) and minor exterior damage (i.e., slight
scratching/denting of gutters or paint; (e) wear and tear to driveways, sidewalks, walkways,
lawns, shrubs, bushes, flowers or other vegetation caused by trucks, equipment, materials,
work force or debris; (f) any damage below the roof due to leaks caused by excessive winds,
ice dams, hail, pre-existing or future construction defaults caused by storms or lack of
maintenance; (g) any damage attributable to rotten, brittle or improperly maintained or
installed siding/trim/ stucco; (h) the failure to perform due to: (t) the errors, omissions and
delays of the Homeowner and/or their insurance company or mortgage company, (ii) the
rescission of this Agreement pursuant to this Section 10. Nothing herein shall be deemed
to abrogate, waive or release in any manner any property and casualty insurer for those
duties, responsibilities or liabilities of such insurer pursuant to Section 10-3-1115 and
Section 10-3-1116 of the Colorado Revised Statutes, as may be amended.
11. Cancelation of Agreement. Following the expiration of the Rescission Period,
Homeowner may cancel this Agreement with Gen 3 be delivering written notice of
cancelation to Gen 3 in the manner set forth in Section 10, above. Upon its receipt of the
notice of cancelation, Gen 3 shall cease all work on the property but for those activities Gen
3 believes are reasonably necessary to prevent any immediate damage to the property,
including the covering up of any unfinished portions of the roof with a tarp. Gen 3, and
those subcontractors and third parties engaged by Gen 3 for the purposes of performing
this Agreement, shall be entitled to a reasonable amount of time to remove its materials
and equipment, including any dumpsters, from Homeowner's property. Homeowner shall
be responsible for paying to Gen 3 a sum equal to (i) the labor charges Gen 3 has incurred
in its performance of this Agreement, (ii) material charges Gen 3 has incurred in its
performance of this Agreement, (iii) any penalties or fees imposed upon Gen 3 as a result
of such cancelation, and (iv) reasonable overhead, profit, expenses and damages not
previously accounted for (collectively, the "Cancelation Balance"). Any deposit held by Gen
3 shall be applied against the Cancelation Balance; to the extent the deposit is greater than
the Cancelation Balance, Gen 3 shall refund the outstanding balance to Homeowner within
ten (10) days of its final determination of the Cancelation Balance. Such refund shall be
issued as a check made payable to the Homeowner named herein and shall be mailed to
Homeowner's address, as set forth herein. Should any deposit be insufficient to cover such
Cancelation Balance, Gen 3 shall bill the outstanding balance to Homeowner, which amount
shall be due and paid in accordance with Gen 3' general practice. Such outstanding
balance shall be subject to the terms of this Agreement.
12. Warranty. Gen 3 warrants that the services to be provided pursuant to this Agreement
will be performed in conformity (i) with the terms of this Agreement, (ii) with all laws,
regulations and codes applicable to the work to be performed hereunder, and (iii) in a good
and workman like manner consistent with standard roofing industry practices. Gen 3 shall
warrant the workmanship relating to a finished roof for 7 years beginning from the date of
completion (the "Warranty Period"). No warranty will be provided until Gen 3 has been paid
in full by Homeowner. To the extent Gen 3 is notified of a defect relating to the workmanship
during the Warranty Period, Gen 3 will correct such defect in workmanship in a prompt and
reasonable manner from the time Gen 3 is notified. Homeowner shall promptly notify Gen
3 of the defect in workmanship as soon as such defect is discovered. All notices of a defect
in workmanship shall be in writing and shall state with specificity the defect discovered. No
employee, agent or subcontractor of Gen 3 is authorized to make any representation or
warranty on behalf of Gen 3 other than those contained in this Agreement. Manufacturers'
or vendors' warranties or guarantees (collectively, "Product Warranties"), if any, on
materials, fixtures and components, to the extent assignable, are deemed assigned by Gen
3 to Homeowner. Gen 3 shall deliver to Homeowner all product warranty fortes in its
possession. Homeowner shall be solely responsible for compliance with any notice and
claim procedures set forth therein. Gen 3 does not adopt and is not bound by any such
Product Warranties. Homeowner's rights under the Product Warranties are in addition to
Gen 3' warranties in this Section 12. This warranty is non-transferrable.
13. Peripherals Disclaimer: Gen 3 acknowledges that, during the course of the services to
be provided hereunder, certain fixtures and peripherals, including, without limitation,
heating and air conditioning units and evaporative coolers, may be required to be removed
and relocated. Gen 3 will use its best efforts to reinstall such fixtures and peripherals in a
