Dear Mr. Rodriguez,
Per your request, I visited the house at the above referenced address on
October 23, 2020. The purpose of my visit was to observe the foundation
repairs.
The final two internal wall braces had not been completely installed at the
time of my site visit. Colorado Structural Repair has since confirmed the
installation of the five internal wall braces has been completed. In my
opinion, with the items noted above, the five (5) internal wall braces and
the sixteen (16) lateral wall anchors appeared to have been installed in
general conformance with the requirements set forth in the engineering
documents.

Other Items (beyond the scope of the structural foundation repairs
1) If applicable, racked doors and windows should be reset, and
cosmetic repairs can be made three (3) months after the structural
repairs are complete.
2) It is very important to maintain proper drainage away from the
garage and house. I do not recommend irrigating within 5 -feet of
the foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees, and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home,
or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, if they exist, should
function properly. Perimeter drains, and sump pumps if they exist,
should function properly. Proper separation of grade to top of
foundation should be maintained.

Dear Mr. Rodriguez,
Per your request, I visited the house at the above referenced address on
August 20, 2020. The purposes of my visits were to observe the
foundation damage reported and provide repair recommendations as
required.
Please note: My investigation was based on observation of accessible,
visible items. Issues/concerns concealed by finishes, if any exist, were
not able to be observed, and were therefore not evaluated as part of this
investigation. Issues and repairs, if required, regarding other parts of the
home were not part of the scope of this investigation.
I understand Colorado Structural Repair previously visited the property
before some of the interior walls were removed.
Observations
For purposes of this report, the front of the home faced east. The east
foundation wall was cracked and rotated and bowed. The horizontal crack
in this wall was significant. Exterior lateral relative movement at the brick -
to -top of foundation consistent with bowing was also observed. Most of

the north (short) foundation wall was concealed by finishes, however
drywall bucking was observed on this wall. A horizontal indication
appeared to coincide with the location of horizontal rebar in this wall. In
my opinion, this wall was slightly rotated. The rear (west) foundation wall
was also cracked and in my opinion, was rotated and bowed, however the
damage was Tess severe than the front (east) wall.
Recommendations
Recommendations for foundation wall repairs to the long east and west
walls and the short north foundation wall are provided below. It is not
necessary to perform all of these repairs immediately. I recommend at
least proceeding with repair of the east foundation wall at this time. If
desired, repair of the west and north foundation walls can be delayed. If
the west and north walls are not repaired, they should be monitored for
significant changes. Significant changes/movement will be an indication
that additional repairs to these walls should be considered.
1) 1 recommend stabilizing the east foundation wall with eight (8)
lateral wall anchors per the attached engineering drawings.
2) 1 recommend stabilizing the west foundation wall with eight (8)
lateral wall anchors per the attached engineering drawings.
3) Due to exterior space limitations I recommend stabilizing the north
foundation wall with five (5) internal wall braces per the attached
engineering drawings.
4) If applicable, racked doors and windows should be reset, and
cosmetic repairs can be made three (3) months after the structural
repairs are complete.
5) It is very important to maintain proper drainage away from the
garage and house. I do not recommend irrigating within 5 -feet of
the foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home, CRY of

or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, if they exist, should
function properly. Perimeter drains, and sump pumps, if they exist,
should function properly. Proper separation of grade to top of
foundation should be maintained.
I observed the structural condition of the visible portions of the
foundation described above. Other unknown concerns may also exist.

suitability of other areas, including, but not limited to mechanical,
electrical, plumbing, previous foundation repairs, and other systems.
The homeowner should understand additional unforeseen and
unforeseeable issues related to soil heaving and or settlement, and
exposure to unwanted water may cause additional foundation
movement in the future. My evaluation and recommendations are
based on my education, experience and observations made on the
date of my visit. The Lepetsos Group, Inc. makes no representations
regarding the ability of the property to meet current or past building
code requirements. Please note it was not possible to observe every

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire -roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspect.ion., conducted wiiei 23-75 percent of the root covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufactureriel"Ts installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.
