Current Warranty fee schedules are as listed. Note: Any applicable taxes will be added to your warranty
fee. Please contact Mule -Hide for pre -approval when applying for any non -published Warranty
timeframes.
' Mule -Hide System Warranties are offered only on new construction and total tear -off projects, or for
recover (retrofit) projects when a moisture survey has been performed and accepted by Mule -Hide.
Acceptable survey types are nuclear, infrared, conductive and core cuts. Results of the moisture
survey must be submitted with the Warranty application for recover applications. All wet roofing
materials found in the survey must be removed.

nance program or agreement. There are various items associated with your root system that are not covered under the warranty. it is the responsibility of the Bulling Owner to regularly inspect
and maintain the roof.
Mule -Hide strongyrecommends the Building Omer Institutes an annual maintenance propramwkh written documentation of any activities on the root Maintain a tog of maintenance procedures
and people accesshhg the root. This aids the building owner In determining the source of airy damage to the roof. Your roping system should be Inspected at least twice a year (once in the spring
and once in the IA and after every maim storm. These ins ieectlans should be performed by a Mule -Hide Warranty Eligible Applicator or by someone specially trained In rooft systems.

6. When it Is necessary for workers to be on the roof to seP&e rooftop equipment, e.g., HVAC units, antennas, etc., workers should be cautioned to use walkways and to exercise care with their
tools and equipment to avoid puncturing the roofing membrane. Mule -Hide recommends that the building owner, of property manager keep a 'Aoof-top Maintenance and Atdlvky LW to rack dates
and activities by personnel or nMher trades.
7. Handprints, lootpAnts, general traffic grime, Industrial pokutanle and environmental dk may be cleaned from the surface of the membrane by scrubbing with detergent and water, men rinsing
with clean water. To maximize and maintain reiectiaky, white membeane(s) should be cleaned once every two years.

as a protective sAckng of a membrane may require peADdic recoadng at specified intervals to maintain the warranty coverage. The Buffing Owner Is responsible for all cogs to perform any

11. Examine all areas adjacent to the roof, parapet wait and ad)oining structures. Damage to items such as masonry, fakhtg mortar pits, loose Of missing sealants, loose gone Or ties, loose and
Improperly sealed counteriashkngs, etc., may be me source of leaks that are inadvertently blamed on the mooing system. These items need to be addressed by property trained personnel to avoid
damage to the roof system.

13. If you have a leak, check kir the obvious such as clogged roof orahns, broken skylights, Noose counteriaghings, broken water pipes, leaking roof units, and storm damage. Note when the leaking
occurs. items such as heavy or light rain, wind direction, temperature and time of day are Important dues for racking suspected leaks. Does me leak start and stop with the rain, or does leaking
continue after the rain has ceased?
It you believe that the leak Is covered underthe Mule-Hkle warranty, please notiy Made -Hide's Warranty Department at (800) 786.1492 as soon as possible, and follow up with written notticatbn len
accordance with the warranty terns. Leaks resulting from the deterioration of failure of building components or physical damage are not covered by the Warranty. The building owner must pay the
Investigation and repair cog t the problem Is found to be outside the scope of the Warranty.
For temporary relish In the Mule -Hide membrane, use a one -part urethane sealant and contact Mule -Hide. Do not use any Asphat Product to make repairs on any single -ply roof as it WILL
degrade the membrane. If any asphalt product Is used On a single -ply rooting membrane, that area will have to be retnwved and replaced at the Owers expense.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those

For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

As the Carlisle SynTec Representative for the state of Colorado, I wish to confirm that the following roof
assembly for a Carlisle Sure-Seal Adhered Roofing System meets Factory Mutual Global (FM) Severe HaiL In
addition, our office has reviewed the submitted information for the above referenced project and confirms that
the following roofing system meets the uplift pressures calculated using the ASCE-7. Local wind speeds for this
building are considered 1 10-mph in an Exposure "C" category.
1. Wood Roof Deck
2. 2.5" thick Carlisle Polyisocyatmrate [nsulation installed over the wood deck.
3. thick Carlisle Dens-Deck Prime fastened to the wood deck with a minimum of 12 Carlisle Fastener
and Insulation Plate for each 4' x 8' size insulation board.
4. .060" thick Sure-Seal EPDM Membrane adhered to the Dens-Deck Prime with Carlisle Bonding
Adhesive
ASCF.,-7 Design Wind Vressure Results in pounds per square foot• Field = 35.8
The above listed Roofing System has tested in accordance with the Factory Mumal (FM) 4450 and (FM) 4470
Standard for wind uplift and this assembly has achieved a FM Class 1-90 rating. This rating indicates the
roofing assembly has resisted 90-pounds per square foot. With this in mind, the Roofing System meets the
ASCE-7 Uplifr Calculations since the tested uplift pressure (minimum 90-psf in fhe field) exceeds the calculated
design pressure (-35.8-psf in the field).
