Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at.(project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.

party inspection report will be collected at final roof inspection performed by City of

to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process

You, the buyer, may cancel this transaction at any time prior to midnight of the third business day after the date of this transaction. See the
attached notice of cancellation form for an explanation of this right.
1. Any prices quoted will remain valid for sixty (60) days from the date of this Agreement. Specified work and quoted prices are subject
to change upon discovery of hidden defects or conditions. The Customer shall: procure any documentation and third -party endorsements(s)
such as lender's approval for the repairs or replacement: comply with any and all requirements of the lender or other lien holder that are
necessary for the insurance claim payments to be endorsed and available to Company, including providing relevant lender and insurance
carrier contact and claim information to Company in a timely and cooperative manner: and participate in any applicable third party appraisal
and/or arbitration process to finalize the proper claim valuation. Customer agrees to reasonably assist Company in communicating with
insurance carrier to reach agreement on cost and scope of repair. Customer will be responsible for the costs of any requested upgrades or
work outside the scope of the claim, Standard payment terms are 50% of the amount quoted in the Agreement as a deposit or upon delivery
of materials, with the balance due upon completion (or release of withheld insurance proceeds to the Customer when applicable), After receipt
of insurance claim payments for the work completed by Company, Customer shall immediately pay these funds to Company. All checks for
payment shall be made payable to CB Construction Inc. with no payments made in cash.
2. The Company will perform the specified repairs or replacements in a workmanlike manner consistent with standard industry
practices, obtain the necessary permit(s), comply with the local building code, and ensure that its work will pass inspection, The Customer
shall not withhold any payments to Company while waiting for the building departments inspection or for Company to perform any additional
work to pass inspection. Repair or replacement of gutters, fascia boards, decking, roof jacks, ventilators, or any other work not specified in
this Agreement will be charged on a time and material basis, if approval of products and colors is required by a homeowners association
("HOA ) or anyone else, Customer shall obtain such approvals prior to commencement of any repairs or replacement. If Customer does not
obtain such approvals, then Customer is responsible for any HOA actions due to the lack of approvals, including fines. All excess materials
will remain the property of Company.
3. The Company provides a limited workmanship warranty for five (5) years for complete roof replacements and for one (1) year for all
other work, which will commence upon completion of the work, If Company is not paid in full for all monies owed under this Agreement, the
Company's limited workmanship warranty is null and void. The Company does not provide any warranty on materials, which are warranted by
their manufacturer. The Company's limited warranty does not cover damages caused by extreme weather or events, including, but not limited
to: high winds: lightning; ice; hail; flooding; mold; fire; or the settlement, failure or cracking of the roof deck, wails or foundation, In the event
there is any dispute about the Company's work, Customer agrees to allow Company the opportunity to do additional work to resolve such
dispute. If Company is not allowed the opportunity to do additional work to resolve the dispute, then Customer agrees that Company is not
liable for the cost of any repairs or damages resulting from the disputed work.
4. The Company disclaims and Customer releases Company, its agents, employees, and officers from any liability for any failure of
performance due to: (a) delays, omissions, or errors by Customer, their insurance carrier, or their mortgage company; (b) weather; (c) acts of
God; (d) the difficulty in procuring materials from usual sources; (e) labor strikes or controversies; (f) lack of access to or additional damage
to the property identified in this Agreement; and (g) any other circumstance beyond the control of Company.
5. The Company disclaims and Customer releases Company, its agents, employees, and officers from any liability for damages caused
by: improper original construction, including but not limited to, faulty framing and roof decks; pre-existing damage or deterioration; rotten,
brittle, or improperly maintained or installed siding, trim, or stucco; nail pops, cracks in plaster, drywall or paint, and other minor interior
damage; scratching and denting of gutters, scratching of paint, landscaping damage, cracking or chipping of concrete, and other minor
exterior damage; and negligence that may cause or contribute to mold, mildew, and its abatement, removal, or cost of repairs and
replacements. The Company disclaims and Customer releases Company, its agents, employees, and officers from any liability for damage
to: electrical, cable, air conditioning, or plumbing lines that are installed within 6 inches of the roofline; evaporative coolers; HVAC units; solar
systems; satellite dishes; skylights; and ventilators.
6. The Customer agrees to indemnify, hold harmless, and defend Company (with counsel reasonable acceptable to Company) from
and against any and all claims other than for willful and wanton conduct, damages, costs and expenses, including reasonable attorney's fees,
arising from any action or proceeding based upon or related to any aspect of the transaction in connection with this Agreement, or any
document executed or delivered in connection with such transaction. The Customer agrees that Company's maximum liability under this
Agreement shall be limited to the Funds Company received for the repairs or replacement. The Company disclaims and Customer releases
Company from any liability for incidental or consequential damages. The Customer further waives any right to recover punitive damages.
7. If Customer does not pay all sums due under this Agreement within thirty (30) days after completion of the repairs or replacements,
then Customer shall pay Company eighteen percent (18%) per annum interest on all amounts due and all collection costs, including
reasonable attorney's fees and other fees incurred by Company in collection of debt, if a payment is canceled or returned due to insufficient
funds (NSF) Customer additionally agrees to pay Company all bank fees and $ 100 per occurrence for recovery,
s. The Customer acknowledges Company is a general contractor, and is entitled to receive all overhead and profit (O&P and/or GC
OHP) in accordance with industry standard practices.
9. If the Customer plans to use the proceeds of a property and casualty insurance policy to pay for roofing work, then pursuant to
C.R.S. § 6-22-104 Customer may rescind this Agreement within 72 hours after receiving written notice from the property and casualty insurer
that the insurance claim has been denied in whole or in part. The Customer's right of rescission does not apply when the properly and casualty
insurer denies, in whole or in part, a claim related to a request for supplemental services, if the damage requiring the supplemental roofing
services could not have been reasonably foreseen as a necessary and related service at the time of the initial inspection or the execution of
the initial contract. The Company may retain payments or deposits by the Customer to compensate the Company for roofing work or other
services actually performed in a workmanlike manner consistent with standard industry practices. To rescind this Agreement, Customer must
give written notice to the Company's office at 355 S. Teller St. Suite 200 Lakewood, CO 80226 within seventy-two (72) hours after Customer
was notified of the denial. If the Customer cancels this Agreement more than three (3) business days after its execution, but prior to
commencement of any work by Company, then Customer agrees to pay Company 15% of the contract price including any supplements
secured in relation to the loss identified in this agreement, not as a penalty, but as fair and just compensation for the services the Company
has provided,
10. The Company is insured by Lightship Insurance, 1650 Franklin St. Denver, CO 80218, telephone (303) 292-9995.
11. This Agreement and all attachments hereto represent the entire agreement between the parties, and supersede all prior written or
oral agreements. No modification, amendment, or waiver of any provision of this Agreement shall be binding upon the parties unless made in
writing and signed by both parties. This Agreement and all claims or controversies arising out of or relating to this Agreement shall be governed
by and construed in accordance with the laws of the State of Colorado, without regard to conflicts of law rules. If any provision of this
Agreement is found to be invalid, illegal or unenforceable, it shall not affect the rest of this Agreement, which shall continue in full force and
effect as if said provision was not included herein. Venue for any legal action, proceedings, or alternative dispute resolution brought pursuant
to or involving this Agreement shall be in the City and County of Denver, Colorado, or any other location mutually agreeable to the parties.
The parties waive the right to a jury trial in any action or proceeding based upon or related to any aspect of the transaction in connection with
this Agreement, or any document executed or delivered in connection with such transaction. This Agreement shall be binding upon the parties
and their respective successors, heirs, legal representatives and permitted assigns.
