Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, po Flans

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

I . if the client plans to use the proceeds of a property and casualty insurance policy pursuant to part 1 of
article 4 of title 10, C.R.S., to pay for the roofing work, pursuant to section 6-2-105, the rooting contractor
cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance deductible
applicable to the insurance claim for payment for work on the covered residential property.

construction guidelines, while adhering to local codes in effect at the time of contract.

8. Client acknowledges Endeavor Exteriors as a general contractor, as such Endeavor Exteriors will be
entitled to ten percent (10%) overhead, and ten percent (10%) profit, (O&P) if approved by the insurance
company, if this project is part of a claim.
9. An account is defined as past due when 10 days have elapsed from the date of completion of work. Past
due accounts will earn interest at a rate of one and one-half percent (1.5%) per month or eighteen percent
(18%) annually. If Endeavor Exteriors is required to turn the matter over to an attorney for collection, the
client also agrees to pay all costs incurred, including attorney's fees.
10. Should default of payment occur on this contract, Endeavor Exteriors reserves the right to place a
mechanic's lien on the clients' property.
11. Endeavor Exteriors shall not be held liable for failure of performance due to labor controversies, strike,
fire, hail, weather, inability to obtain material from usual source, or any other circumstance beyond the
control of the company.
1 Our standard workmanship warranty for roofing is non-transferable limited lifetime, and one (1) year for all
other trades, and the company will provide a service call for roof inspection, as long as the client owns the
property.
13. Endeavor Exteriors is not responsible for any leaks or water damage below the roofline caused by
excessive wind, ice or hail during the period of the warranty.
1-1. Endeavor Exteriors shall refund any deposit if this contract is cancelled within 72 hours (3 days).
15. If this contract is cancelled by the client, after three (3) or more days of execution, the client agrees to
pay Endeavor Exteriors twenty percent (20%) of the contract price, set by the insurance company, only if
materials have been delivered or work has been performed, as liquidated damages. This payment is not a
penalty, but is fair and just compensation for services rendered to that point.

homeowners claim that relates to this contract.
17. While we make every effort to ensure there are limited consequences of construction, Endeavor Exteriors
will not be held responsible for slight scratching or denting of gutters, droplets of oil in the driveway,
cracks in concrete, or damage to plants and trees. Furthermore, Endeavor Exteriors will not be held
responsible for exterior damages such as deteriorated trim pieces, crown molding, stucco, solar panels,
evaporative coolers, existing skylights, or interior damage such as falling dust and debris, cracked plaster,
sheet rock or nail pops, as a result of normal re-roofing.
18. Replacement or repair of the deteriorated decking, fascia boards, swamp coolers, ventilators, chimneys,
counter flashing, or any other work completed for the homeowner not explicitly stated in this contract,
are not included in quoted price and will be charged as extra on a time and material basis.
19. Endeavor Exteriors holds the right to photograph all projects in which it is invloved, and the right to
puhlish throughout all Endeavor Exteriors' social media outlets.

On June 6, 2001, I contacted the realtor for the above mentioned property, because no one was at
the address. The realtor, Sam Applegate(303) 499-3111, informed me that the property is sold
and the current owner is giving up ownership on June 18, 2001.
Since it appears that the owner is cleaning the property and no habitation of the garage is
currently evident, no direct action will be taken at this time. Another inspection of the property
will be scheduled for June 18'h. 2001.

On 052501, at about 1443 lus, I responded to 4275 Vivian St. to check the welfaze of an infant reported to be
sleeping around moldy condifions. After being dispatched to the address, Off. Weigel asked to meet me before I went
to the call. Officer Weigel and I met and she advised me the following: Officer Weigel told me she and code
enforcement has been trying to look at the house for sometime. I was told there were as many as I 1 people living in the
home and the place was not made to fiC that many. I was also infonned there were a coUple of inean dogs at the
residence. Officer Weigel asked if she could come along with me. I advised Off. Weigel she would be able to come.
As I drove up to the residence I pulled in behind a blue Ford Ranger. The occupant of fhe vehicle stepped out and I
talked with him The occupant identified himself as Dominick Shaffrier. I asked Dominick if he lived there at the
residence. Dominick said yes. I advised Dominick why I had come to the house and wanted to see the infant.
Dominick told me the child was his and his girlfriend, Amazida Selvage. Dominick then led me into the house where
Amanda was with the baby and several other people. I talked with Amanda.
1 advised Amanda why I had come and asked to see the baby. Amanda layed the child down so I could look at
him. I checked the baby for bruises, mazks, sores Eta I found nothing on the chiid and he appeared to be in healthy
condition. Afrer looking at the child I asked to see his living quarters. Amanda looked at me puzzied. I asked Amanda
where the child's sleeping quarters were again. Amanda advised me they were staying in the garage. I asked Dominick
to watch the child while Amanda and I went to the gazage which was beliind the house.
While in the garage I made the followine observation:
- Before going inside of the garage I observed the were wholes in the Lara?e door itself.
- Tlie door closest to the entrance door was open and I was able to view, inside. There was a large amount of trash at7d
discarded materials (carpet, auto parts; clothina, and paint canisters) inside.
- Iwalked inside of the garage and saw the condition of the garage was the same as the azea closest to the entry way.
- The aarage was not a finished one. This was a regular car garaee witli not heatina, lieht or runnine water.
- Along the west wall of Yhe gvaae was stacked clothin, aud various items.
- The was ainakeshift bedroom in room southwest corner of garage. The %vas clothing hanging from the rafters. There
were clothes al] alon, the floor.
- Toward the eenter of the garage was a couch hide-a-bed pulled out. 7he sheets of the bed were partially pulled back
and the mattress was eaposed. The mattress was filthy.
- On the westside of the hide-a-bed was the child bassinet/car seat. The bassinet was sitting on the ground. There was a
coach directlv soutli of the hid-a-bed. Tliere were clothes all around and o» top of tlie coach.
- On the east side of the Iiide-a-bed \aas the earace door. I could see the lioht iiom outside shining in. 1 could also see

- There was a pile of dirt and trash near the whole in the gazage wall
Afrer looking at the garage I talked with Amanda. I advised Amanda the garage was not a fit place for the child to
be sleeping. Amanda started to cry and said "I know." Amanda told me that this was the worst thing ttiat could have
happened to her. I asked Amanda what she meant. Amanda said "you taking my child away." 1 asked Amanda why
slie was living in the garage area. P.inanda told me she and the baby were staying inside of the house but since it was
warming up lately she brought him out. I told Amanda though it might be wanning up outside during the day, the night
is still very chilly especially around the morning times. I asked how long she and the baby had been sleeping outside.
Ainanda told me a week. Amanda told me she stays inside of the house during the day with the baby and then comes to
sleep in the garage at night. I asked Amanda where the baby's food aud diapers were. Amanda pointed to the baby bag
ou the floor next to the bassinet. I asked fvnanda how much food the baby had. Amanda told me she buys more food
and diapers as they need them.
I then went to the house and looked at the conditions there. The house overall was diriy. I advised both Dominick
and Amanda I was going to chazge them both with child abuse. Amanda began to cry and Dominick looked angry. I
advised Amanda that I was not going to take the child away at this pomt because I felt she really loved the baby.
Amanda cried even more and told me how much she loved the child. Dominick was feeding the baby at that hme.
I asked Amanda and Dominick to come outside while I issued them their summonses. Amanda and Dominick
came outside with me to my caz. I began to write Amanda's ticket when she asked'to tell me something. I asked
Amanda what it was Amanda said she wanted me to know how it was all her fault. Amanda went on to say how
Dominick works night at the Denny's at 6" and Simms ST., and was noc home when she would take the baby outside to
the garage. Amanda told me Dominick has told her not to take the child outside and she would disregard his advise. I
asked Dominick if that was true. Dominick said it was true and that he has told her this several times. I called the
Demiy's and checked the schedule to confirm Dominick really worked nights. It was true Dominick works nights.
Amanda told me Dominick usually does not learn of the child s]eeping outside until morning. I issued Amanda a
summons (4103628) for child abuse (M3).
After issuing Amanda the summons, I took digital photographs of the outside of the house, the inside of the house,
the back yazd and the garage. While I was taking pictures of the garage Off. Weigel noticed there were some mattresses
on top of the rafters of the garage. Officer Weigel went up a latter that was set-up and observed it looked as if more
people were sleeping there.

ON 052501, AT APPROXIMATELY 1500 HOURS, OFFICER MENDEZ WAS DISPATCHED TO 4275 VIVIAN STREET ON A POSSIBLE CHILD ABUSE. I
REQUESTED OFFICER MENDEZ MEET WITH ME PRIOR TO HIS RESPONSE TO THE ADDRESS. I ADVISED OFFICER MENDEZ CODE
ENFORCEMENT HAD RECEIVED SEVERAL COMPLAINTS ABOUT THE RESIDENCE INCLUDING TRASH BUILDUP AND TOO MANY PEOPLE LIVWG IN
THE HOUSE. I ASKED OFFICER MENDEZ IF I COULD RESPOND WITH HIM TO THE RESIDENCE TO INVESTIGATE THE COMPLAINTS OUR OFFICE
HAD RECEIVED. OFFICER MENDEZ AND I THEN RESPONDED TO 4275 VIVIAN STREET. UPON ARRIVAL, I CONTACTED A1 SUSAN WILSON. i
ADVISED A1 WILSON OF THE COMPLAINTS OUR OFFICE HAD RECENED. I ASKED A1 WILSON HOW MANY PEOPLE LNED AT THE RESIDENCE. Al
WILSON ADVISED ME SEVERAL PEOPLE WERE LIVING IN THE HOUSE AND ADDITIONAL PEOPLE WERE LIVING IN THE GARAGE. I ASKED Al
WILSON TO EXPLAW TO ME WHO WAS LIVING AT THE RESIDENCE AND WHERE THEY WERE STAYING. A1 WILSON ADVISED ME OF THE
FOLLOW ING:

MORGAN OR CINDY HAGERMAN WITH THE CITY'S BUILDING DEPARTMENT TO HAVE ONE OF THEM RESPOND TO OUR LOCATION. ESS VENABLE
ADVISED ME HE WAS NOT ABLE TO CONTACT EITHER MORGAN OR HAGERMAN. I ADVISED A1 WILSON I WOULD FORWARD A COPY OF THIS
REPORT TO THE CITY'S BUILDING DEPARTMENT FOR FOLLOW-UP ON THE NUMBER OF PEOPLE LIVING AT THE RESIDENCE. i ASKED Al WILSON
IF SHE WOULD EXPLAIN TO ME WHERE EVERYONE LIVING AT THE RESIDENCE WAS SLEEPING. A1 WILSON STATED SOME PEOPLE SLEEP ON
THE FLOOR, OTHERS SLEEP ON THE FURNITURE, AND THE REST SLEEP IN THE BEDS. I OBSERVED THE RESIDENCE TO HAVE TWO BEDROOMS
AND A SINGLE BATHROOM. I OBSERVED SEVERAL EXTENSION CORDS RUNNING FROM THE HOUSE TO THE GARAGE. I OBSERVED THE HOUSE
TO BE IN DISARRAY AND MESSY WITH SEVERAL CLOTHES LAYING ON THE FLOOR, DIRTV DISHES, AND DEBRIS LAYING ABOUT. OFFICER
MENDEZ TOOK DIGITAL PHOTOGRAPHS FOR HIS INVESTIGATION. I REQUESTED A COPY OF THE DIGITAL PHOTOGRAPHS FOR OUR
INVESTIGATION. A1 WILSON AND I THEN WENT OUTSIDE. i ADVISED A1 WILSON THE AMOUNT OF ACCUMULATED TRASH AND GARBAGE ON
THE PROPERTY WAS UNACCEPTABLE. i OBSERVED WHAT APPEARED TO BE A LARGE PICKUP BED TRAILER PARKED ON THE SOUTH SIDE OF
THE RESIDENCE NEXT TO THE DRIVEWAY. THE TRAILER WAS OVERFLOWING WITH FULL GARBAGE BAGS AND I COULD SMELL A FOUL ODORCOMING FROM THE TRAILER. THE TRAILER WAS COVERED W ITH A TARP, BUT THE TARP WAS NOT SECURE ALLOWING THE GARBAGE TO
BLOW AROUND AND THE ODOR TO ESCAPE. 1 OBSERVED ANOTHER BLUE TARP COVERING SOME ITEMS ON THE SOUTH SIDE OF THE
RESIDENCE BETWEEN THE HOUSE AND THE TRAILER. I ASKED A1 WILSON WHAT WAS UNDER THE TARP. Al WILSON VOLUNTARILY SHOWED
ME ADDITIONAL FULL GARBAGE BAGS AND AN OLD CHAIR WERE COVERED BY THE TARP. 1 SHOWED A1 WILSON THE ADDITIONAL DEBRIS,
TRASH AND GARBAGE ON THE WEST SIDE OF THE HOUSE. A1 WILSON STATED THEY USUALLY WAIT UNTIL THE TRAILER IS FULL BEFORE
TAKING IT TO THE DUMP. A1 WILSON STATED THEY PICK THE ADDITIONAL TRASH AND DEBRIS AT THE SAME TIME. I ADVISED Al WILSON
THERE APPEARED TO BE SEVERAL WEEKS OF ACCUMULATED TRASH AND GARBAGE ON THE PROPERTY. Al WILSON STATED SHE THOUGHT
OTHER HOUSEHOLD MEMBERS WERE PUTTING THE TRASH AND GARBAGE OUT FOR COLLECTION ONCE WEEKLY. I ISSUED A1 WILSON
SUMMONS #59089 FOR UNLAWFUL ACTIVITIES (15-25-A). I ADVISED Al WILSON SHE HAD UNTIL 060201 TO REMOVE THE TRASH, GARBAGE AND
DEBRIS FROM THE PROPERN. A COPY OF THIS REPORT WILL BE GIVEN TO THE BUILDING DEPARTMENT. ALL MENTIONED PARTIES WERE
CLEAR CCIC AND NCIC WITH THE EXCEPTION OF P10 LOCKE, WHO HAS AN ACTNE WARRANT FROM DOUGLAS COUNTY.
