Subject: Review of Foundation Repairs at 2965 Wright Court, Wheat Ridge, Colorado 80215
As requested by Olshan Foundation Solutions, our office has reviewed the repair work finalized at the above
referenced address, including portions of the pilings using 3 interior pre -cast concrete and steel pipe Cable Lock TM
beneath the above referenced location and the drill log provided for the piling installations supplied by Olshan
Foundation Solutions. In our opinion, the repair work has been completed in general conformance with the
recommendations set forth in Olshan Foundation Solutions repair plan.
In our opinion, the piling depths in conjunction with the pile driving force as reported by Olshan Foundation Solutions
are generally appropriate for this type of structure and for the area where the work is being performed. The repair
work performed to the subject location has been acceptably completed, based on the information provided by Olshan
Foundation Solutions, in accordance with good industry practice for foundation repair work using pre -cast segmental
piles. The repairs performed on the subject location should be expected to minimize the foundation settlement
observed prior to the foundation repair work. In instances where partial repairs are performed, meaning the entire
foundation has not been underpinned, potential differential movement may occur. It should be noted that partial repairs
modify the design of the foundation and while partial repairs are generally accepted industry practice the possibility
of future movement should be recognized. Non -supported areas are not covered for downward foundation movements
by the contractor's warranty.
The future performance of the foundation system on the subject location should function as generally intended,
provided proper soil moisture is maintained and there is not a loss in the load bearing capacity of the soil beneath the
foundation. Soils should be graded such that there is positive drainage away from the foundation or a drainage system
can be installed to prevent water from ponding around the foundation. A foundation maintenance program is
recommended which can be found at www.foundationperformance.org.

We have reviewed the sketch and the foundation repair system proposed for the subject location.
In our opinion, the proposed foundation repair should minimize the foundation settlement at the
subject location providing proper installation techniques which includes the manufacturer's
technical manual/specifications for pre -cast concrete segmental pilings FPA -SC -08 latest edition
which can be found at www.foundationperfonnance.org industry standards and per pre -cast
attached, pier pile systems specification and in accordance to 2018 International Residential
Code for City of Wheat Ridge.
The areas that are represented by cross -hatching on the attached sketch are the areas, in our
opinion that will be affected by the proposed foundation repair system, provided the foundation
system is of acceptable strength. if the actual strength of the foundation system is found to be
inadequate to support the spacing of the piles as shown on the attached sketch then additional
piles may be required to support the foundation.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

I was recently approached by Steve Becker of National Home Improvements, Inc. concerning a re-roofing
project at the above address. Mr. Becker showed me photographs on a small camera viewing screen that showed
a shed style roof over a patio that rested on a hip style roof on the main house. Mr. Becker requested a letter
stating the City of Wheat Ridge requirements to re-roof the entirety of both structures.
Based on the pictures provided, it appears that because of the method(s) of construction used to install the patio's
shed roof, it would be difficult, if not impossible, to properly re-roof the main house without detaching the shed
roof on the patio. The City of Wheat Ridge would require this action as a part of the installation of the main roof
covering. Additionally, the City of Wheat Ridge would require that, upon re-attachment of the shed roof over the
patio to the main house, the attachment and bearing of the patio roof comply with 2003 International Residential
Code requirements. Documents illustrating the method(s) of re-attachment will be required to be submitted and
reviewed at the time of permit application.
