Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and_procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a changeof the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

CONSTRUCTION CONTRACT BETWEEN
CONTRACTOR AND OWNER
THIS CONTRACT made and entered into Utis _day of October, 1996 by and between: Mary Bellio
hereinafter referred to as "Owner", and Conner Construction Co. hereinafter refecred to as "Contrador."
FOR AND W CONSIDERATION of the covenants and agreements herein contained, the parties agree as
follows:
1. DESCRIPITON OF WORK: Contractor agrees to fumish all materials and to pedorm all work required
pursuant to the plans and specifications as shown on the attached Description of Work (Attachment A) for the
swcture located at:
4665 Routt Sk, Wheat Ridge, CO. 80033
2. COMMENCEMENT AND COMPLETION: The Contractor agrees to properly and promptly complete all
the work required under this contract no later ihen January 26th,1997, which is 90 days after the issuance of the
Notice to Proceed dated, 28tN October, 1996. Work shall commence not later that 10 days after the Notice to
Proceed date. All work shall be done to the satisfaction of the Owner and the Jefferson County Housing Authoriry
(7CHA), or its agent, and in accordance with all applicable ordinances, codes, and property rehabilitation standards
of the JCHA and local authoriues involved as well as all applicable federal requirements.
No time extensions will be granted and if the Contractor shall fail to complete all the work by that date the Owner
may recover damages for i[s actual and consequential losses as well as attomeys fees. Therefore, time is of the
essence. The Contractor shall not be entiUed to damages for Owner caused delays but may receive time eatenafons.
3. PROHIBITION AGAINST TRANSFER: The Contractor shall not assign, Vansfer, convey, sublet or
othenvise dispose of all or any part of Uus Contract, or any right, fltle or interest to any persoq firtn, or
corporation without the prior written consent of lhe Owner and ihe JCHA.
4. PAYMENTS: In complete satisfaction for all work to be pedormed by Contractor pursuant to this Convact,
Contractor shall be paid the fixed price contrad price of $ 24.580.00
Payments as tollows:
The contract item amount will be paid [o the Contractor on Partial Paymcnt draws as submit[ed on the authorized
Application for PaymenhCertiticate and Release, and as approved by the Owner and authorized by the JCHA upon
completion and acceptance of work in place. Final payment after completion and acceptance of all work by the
Owner and lhe JCHA provided no payment claims exist.
The wntract funds will be placed into an escrow account as selected by the JCHA. ]CHA will pay all costs related
to utilizatlon of the escrow. No paymen[s will be made pursuant to ihis Contract unless approved and authorized
by the JCHA. Under either payment optioq relainage may be used if necessary [o correct any warranty obligations
lhat the Contractor fails or failed to pedorm and for any payment claims. No payment will be approved or
authorized unless satisfactory lien waivers and releases are provided ro the Owner and to the JCHA. Under either
payment option, payment checks will be issued jointly to the Contractor and the Owner.
5. COMPLIANCE WITH PERMITS, CODES AND LICENSES: The Contractor shall at his own expense,
secure all necessary pernuts, licenses, and other authorizadon as may be required any govemmental entity, by any
applicable code compliance, or by the JCHA and pay all fees [herefor, as may be necessary for the work covered by
this Contract.
6. PAYMENT OF TAXES: The Contractor shall pay all sales and use taxes upon all of the materials furnished
pursuant to this Contract as required by appropriate governmental authority.
7. CHANGE ORDERS: Only with the prior written consent of the JCHA, the Owner may at any stage of the
work make changes in the work within the general scape of the Con[ract, by written order to the Contractor signed
by JCHA and the Owner. If any change under this paragraph causcs an increasc or decrcasc in the Contractor's
cost of, or the time required for, the performance of any part of the work under this con[ract, an equitable
adjustment shall be made and mutually agreed upon by the Owner and Contractor with the approval of the JCHA.
No extra work or changes shali be recognized or paid for unless agreed to in advance in writing and approved in
advance by the JCHA in writing.
8. TERMINATION FOR DEFAULT - DAMAGES FOR DELAY: If the Contractor refuses or fails to
prosecute the work in a timely manner so as to insure its completion within the time specified in this Contract, or
any extension thereof, or fails to complete the work within this time, the Owner may, by written notice to the
Contractor, terminate the right to proceed with the work that has been delayed. In this event, the Owner may take
wer the work and complete it, by convact or otherwise, and may take possession of and use any materials,
equipment, and supplies on the work site necessary for wmpleGng the work. The Contractor shall be liable for any
damage to the Owner resulting from the Contractor's refusal or failure to complete the work within the specified
time, whether or not the ConVactor's right to proceed with the work is terminated. This liability includes any
increased costs incurred by the Owner in completing the work.

LEAD-BASED PAINT: Contracror certifies that oNy lead-free paint will be used. ff it is determined t6at
lead-based paint exists in a ha7ardous form or Iceation, contractor/homeowner must abate all areas invoived in
accordance with applicable standards.
20. FINAI, INSPECTION AND PAYMENT: Final payment on the contraM shall be made only after final
inspection and acceptance by the Owner and the JCHA, of all the work ta be done pursuant to this Contract by the
ContraMOr, further upon the Contractor furnishing the Owner and JCHA satisfactory liens waivers from all
subcontractors and suppliers who supplied materials, equipment or labor for this Contract, the ContraMOr's lien
waiver and certificate and release, all manufacturer's warranties, and any other documentation that may be
required by the JCFIA.
21. ADDITIONAL REQUIREMENTS: Attachmen[ B hereto contains addidonal requirements which aze part
of this Contract.
22. BINDING EFFECT OF CONTRACT: This Contract shall be binding upon and shall inure to the benefit of
the parties hereto, their heirs, personal representatives, successors and approved assignees.
