As requested, a site visit of the aforementioned residential renovation project was conducted on
February 12, 2020 to observe the structural rough framing as it pertains to the referenced design
drawings. For the purpose of this report, the residence east towards Yarrow Street.
All observed items were found to be in general compliance with the stamped design
documents and applicable local and national codes.
Subsequent to our site visit plumbing penetrations were made in the southeast comer of the main floor
framing for the master bathroom plumbing. The two western most penetrations are adequate, the
eastern most penetration required 3/4" OSB reinforcement. No additional remediation required.
We trust this letter meets the requirements for the City of Wheat Ridge Building Department. Please
do not hesitate to contact us if you need additional information on this matter.

INTRODUCTION
This report concludes the professional industrial hygiene services following the minor spill response associated
with the removal of drywall material containing <1% asbestos joint compound as identified through laboratory
analysis. This project is a single-family residential dwelling (SFRD) located at 3995 Yarrow Street in
Wheatridge, Colorado. The structure is currently unoccupied but after renovation will be the homeowner's
primary residence.
PROJECT SUMMARY
A limited asbestos inspection was performed on December 4, 2019 by AVENV which identified <1% asbestos in
the joint compound of the drywall system throughout the main floor of the residence. Since the material was
reported as containing less than 1% asbestos and not considered a regulated material, the homeowner believed
he was authorized to remove and dispose of the material himself. As outlined in Colorado Regulation No. 8
Part B - Asbestos and as regulated by the Colorado Department of Public Health and Environment Air Quality
Control Commission (CDPHE) Section I. B.17. states Asbestos -containing material means material containing
more than 1% asbestos.
After the Mr. Haen (homeowner) had removed the drywall himself, he was later visited by an inspector with the
City of Wheatridge who informed him that he needed an air clearance report before the inspector could enter
the residence for permitting activities. Several conversations occurred between the homeowner and a State
inspector at CDPHE, Asbestos Division, regarding what is required and regulated. By regulations the homeowner
is allowed to remove and dispose of any materials containing 1% or less asbestos. However, OSHA regulations
do not recognize the percentage of asbestos in building materials in which case any amount of asbestos
according to OSHA is still considered an asbestos containing material.
Based on this information, Mr. Haen was prompted to clean the interior areas of his residence in order to
comply with the city inspector. He started by installing two (2) HEPA filtration units, one on the main floor
and the second one in the basement which filtered the air and exhausted to the exterior. He then
proceeded with the Minor Spill protocol as outlined in Section Ill. T.2. of Colorado Regulation No. 8 Part
B - Asbestos and as regulated by the Colorado Department of Public Health and Environment Air Quality
Control Commission (CDPHE).

(PCM), per the National Institute for Safety and Health (NIOSH) 7400 method in general accordance with
the PCM methodology.
Air Quality Analysis, LLC is accredited through the American Industrial Hygiene Association (AIHA) for
Phase Contrast Microscopy (PCM) analysis Lab ID #219321. Air Quality Analysis, LLC successfully
participates in the AIHA Proficient testing (PAT) program.
Air Quality Analysis, LLC Laboratory AQA20-0010 is the lab number assigned to this project. The air
sample results for the project referenced above have been included in your final report and sent to the email
and or address associated with this project. The results described in this report apply only to the samples
submitted and analyzed. This report is considered highly confidential and the sole property of the

FIELD ACTIVITIES
FINAL CLEARANCE
AVENV conducted a visual inspection and final air clearance following the minor spill response within the
residence. Final clearance activities were performed in accordance with the Environmental Protection
Agency (EPA) and Colorado Department of Public Health and Environment (CDPHE) regulations. Final
clearance activities were conducted by a CDPHE certified Air Monitoring Specialist (AMS).
Air Sampling
Five (5) air samples were collected throughout the main floor of the residence using variable high-
volume pumps. Flow rates of each pump were established with a calibrated flow meter using a
rotameter (secondary standard) traceable to a primary standard. Air samples were collected by drawing
air at a rate of 15.44 liters per minute (LPM) through 25mm mixed cellulose ester membrane (Millipore
0.35 MCEF) filter, housed in three-piece cassettes and equipped with 50mm electrically conductive
extension cowls.
Pre -and post -calibrations of each sample pump was documented, and an average flow rate was
calculated and used in the final analysis of each sample collected. Calculations used in the final analysis
combines the average flow rate (LPM), the total sample time and the volume of air collected.
All air samples collected were analyzed by the AMS under our participating laboratory, Air Quality
Analysis, LLC using the Phase Contrast Microscopy (PCM) NIOSH 7400 Method. Samples were analyzed
for airborne fibers using a positive Phase -Contrast microscope equipped with a Walton -Beckett graticule
(type G-22 for "A counting Rules"). Only fibers greater than 5 microns in length with a length -width
(aspect) ratio equal to or greater than 3:1 were counted and resulting concentrations expressed in terms
of total fibers per cubic centimeter of air (f/cc).

Final Clearance Criteria
An abatement action is considered complete when the results of samples collected within the abatement
work area and analyzed by PCM using the NIOSH Method 7400 show that the concentration of fibers for
each of the five samples collected is equal to or less than 0.01 f/cc. The is the level the EPA and the Air
Quality Control Commission (CDPHE) requires all samples to be equal to or less than prior to releasing an
area to the public.
CONCLUSION
Based on the visual inspection; documentation review; and air sample results it is the professional
opinion of American Veteran Environmental that the interior of the residence has met the clearance
criteria and is in compliance with all regulatory and industry standards.

As requested, a site visit of the aforementioned residence was conducted on November 191, 2019 to
review the proposed renovations to be completed at the subject site. The proposed renovation requires
the removal of a sections of walls between the Kitchen/Living Room and Stair Corridors (Beams 1-3),
removal of section of walls for the proposed Master Bedroom (Beams 4 & 5), and 3 new egress
windows in the basement foundation.
The existing residence is a single family, one-story house with attached garage and a
basement/crawlspace. The residence was reportedly constructed in 1957. Physical observation of the
structure verified the cut framed hip roof. In addition, the floor framing was verified to be 2x8s
spanning north to south over a bearing wall and crawlspace foundation. For the purpose of this report,
the residence faces east towards Yarrow Street. Based upon the existing conditions, proposed
renovations, and applied design loads the following design shall be followed.

garage wall (min. 4-2x4 stud pack) and install HHUS5.50/14 hanger at north end into beam #2.
Beam #2 (Approx. 17'-0")
New beam in the ceiling between the kitchen and living room shall be 3-16" LVL. At west end, post
beam in exterior wall (min. 5-2x4 stud pack) and install HGUS5.50/14 hanger at east end into beam #3.
If LVL needs to be ripped, a minimum height of 8.5" is required.

INTRODUCTION
As requested, American Veteran Environmental, LLC (AVENV) performed a limited asbestos
inspection at 3995 Yarrow Street, Wheat Ridge, Colorado. This asbestos inspection was
limited to the attic insulation, kitchen area drywall and basement ceiling tiles. This inspection
was conducted on Wednesday December 4, 2019 by Colorado State Certified, Asbestos
Building Inspector, Samuel Rolf, ABI #921.
Colorado Regulation No. 8 requires that all suspect Asbestos -Containing Building Materials
(ACBM) be properly sampled and analyzed prior to any renovation or demolition activities. The
purpose of this inspection was to identify and sample suspect flooring and surfacing material.

ASBESTOS INSPECTION
This inspection involved identification and sampling of suspect ACBM in the area to be impacted
by renovation/demolition activities. As stated above, sampling was limited, but all sampling was
performed in general compliance with Colorado Regulation No. 8. Suspect ACBM includes
nearly all building materials except glass, metal, wood and ceramic. Bulk samples of suspect
ACBM were collected in general conformance with AHERA protocols as outlined in Colorado
Regulation No. 8. Random samples of suspect building materials were collected throughout
each homogeneous area. Samples were placed in sealable containers and labeled with unique
sample numbers using an indelible marker.
2.1 ASBESTOS -CONTAINING MATERIALS
Asbestos -Containing Materials (ACM) contains asbestos in concentrations greater than 1
percent, (> 1% asbestos) as confirmed by an accredited laboratory. ACM is categorized as
Non -friable or Friable asbestos. Non -friable asbestos is any material that cannot be crumbled,
pulverized or reduced to a powder by hand pressure when dry. Friable asbestos can be
crumbled, pulverized, or reduced to a powder by hand pressure when dry.
2.2 SUSPECT ASBESTOS CONTAINING MATERIALS
Suspect ACBM are building materials historically known to contain asbestos and therefore
suspected to contain asbestos. The EPA and the Occupational Safety and Health
Administration (OSHA) maintain comprehensive lists of suspect ACBM frequently encountered
during inspections.
The following suspect friable and non -friable materials were observed during the inspection:
• Miscellaneous Blown in attic insulation (BL101-1 and 2)
• Miscellaneous Composite drywall and tape joint compound (CDW01-1 and 2)
• Miscellaneous Ceiling tiles (CT01-1 and 2)

Six (6) samples were collected and submitted under chain of custody procedures to an
accredited laboratory for analysis by Polarized Light Microscopy (PLM) with dispersion
staining techniques per EPA 600 method. Microscopic visual estimation was used in
obtaining the percentage of asbestos in bulk samples. Appendix B contains these analytical

2.4 GENERAL COMMENTS
This asbestos inspection was conducted in a manner consistent with the level of care and skill
ordinarily exercised by members of the profession currently practicing under similar conditions
in the same locale. The information contained in this report is relevant to the date on which this
inspection was performed, and should not be relied upon to represent conditions at a later date.
This report has been prepared on behalf of and exclusively for use by the owner for specific
application to the project as discussed. Contractors or consultants reviewing this report must
draw their own conclusions regarding further investigation or remediation deemed necessary.
AVENV does not warrant the work of regulatory agencies, laboratories or other third parties
supplying information which may have been used in the preparation of this report. No warranty,
express or implied is made. In the event of any reuse or publication of any portion of this report,
AVENV shall not be liable for any damages arising out of such reuse or publication. Any use a

CONCLUSION
AVENV has performed a limited asbestos inspection of the water damaged area of this house.
Reasonable effort was made by AVENV to locate and sample accessible suspect building
materials. However, for any structure, the existence of unique or concealed asbestos
containing materials is a possibility. AVENV only collected PLM bulk samples in the
renovation/demolition areas at the direction of Josh with Empirical Construction. This limited
asbestos inspection report is not considered a comprehensive asbestos inspection report.

Electron Microscopy (TEM). Please note that PLM analysis of dust and soil samples for asbestos is
not covered under NVLAP accreditation. Estimated measurement of uncertainty is available on
request.

As requested, a site visit of the aforementioned residence was conducted on November 19h, 2019 to
review the proposed renovations to be completed at the subject site. The proposed renovation requires
the removal of a sections of walls between the Kitchen/Living Room and Stair Corridors (Beams 1-3),
removal of section of walls for the proposed Master Bedroom (Beams 4 & 5), and 3 new egress
windows in the basement foundation.
The existing residence is a single family, one-story house with attached garage and a
basement/crawlspace. The residence was reportedly constructed in 1957. Physical observation of the
structure verified the cut framed hip roof. In addition, the floor framing was verified to be 2x8s
spanning north to south over a bearing wall and crawlspace foundation. For the purpose of this report,
the residence faces east towards Yarrow Street. Based upon the existing conditions, proposed
renovations, and applied design loads the following design shall be followed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

REPLACEMENT COST: $ 15,004.05 per insurance proceeds plus any supplements that may apply to your claim.
Upon approval of your claim, ATO Z ROOFING will file for any and all supplements which may include building code items, unforeseen items and missed or omitted items on the insurance
scope provided to you by your Insurance company. Those items will be paid per your policy provisions and will become part of this contract as if contained herein. A TO Z ROOFING will
negotiate a fair and equitable settlement agreeable by both your insurance company and A TO Z ROOFING based on competitive market rates for labor & materials with no additional cost
to you except for your deductible and any upgrades or additional work authorized by you that are not part of your approved Insurance claim. Should your insurance company deny a full
roof replacement, you will not be obligated to this contract and may enter into an acceptable repair agreement with A TO Z ROOFING.
ACCEPTANCE OF PROPOSAL—This proposal becomes a legal binding contract 72 hours from date of
acceptance. 1 %%per month interest will be added onto all accounts 30 days past due. In addition, costs
and reasonable attorneys fee for collections may be charged on any account past due over 30 days.
