inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Insta.l]_ation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY .roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable -:odes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspe(_�tion
of elastomeric or similar type roof coverings, a letter of inspection and approva from tie

subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6, The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, atly violation of any provision of any

Work to Be Done: Remove and Replace Hail Damaged Roof Coverings.
1. Obtain Permits from City of Wheat Ridge. (Contract Price to include Permit Fee).
2. Remove Existing Underlayment, Shingles and Modified Bitumen from Main Dwelling and Two (2) Sheds.
3. Remove Debris and haul away from Site, run magnet over property to ensure nails are collected and removed
from site.
4. Install 30# Synthetic Underlayment, Drip Edge Flashings, Vents/Vent Caps and Ice and Water Shield on all
buildings as required to meet City Code..
5. Install approximately 26 SQ. Owens Coring Duration Storm/Antique Silver on all buildings.0 C j -c55 A��= -YR

7. Remove and Replace two (2) skylights, including framing.
8. Re -mount Swamp Cooler and install required flashing.

Building Department.
10. Job will be completed within 4 weeks of receipt of signed contract and up front material and mobilization
payment.
WORK CONTRACT
.I, the undersigned, hereby request and authorize Rastra Building Solutions to execute
all repairs and/or replacements detailed above. I acknowledge the agreed upon price
for these services as being $14,414. I agree to pay $7,500 upon acceptance of this
contract. I agree to pay $4,250 upon completion of Roof Installations and to pay the
remaining balance ($2,664) upon passed City Roof Inspection. RBS guarantees the
workmanship at the above address against any leaking for a period of Ten years.
