Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding 51% -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to 1: obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

THE CONTRACTOR ON THE FACE HEREOF AND ANY AGREEMENT MADE PURSUANT THERETO BETWEEN
REROOF AMERICA CONTRACTORS CO, LLC (THE "COMPANY") AND THE CUSTOMER(S) WILL BE SUBJECT TO ALL APPROPRIATE
LAWS, REGULATIONS AND ORDINANCES,AND TO THE FOLLOWING SPECIALTERMS AND CONDITIONS.
1. All contracts subject to approval of our Credit Department & Management.
2. Payments shall be made upon the following terms. Net cash on or before the tenth (10th) day following the completion of work. if
work is of such nature that it will exceed one calendar month, partial payments shall be made on the tenth (10th) of each calendar
month, based on the work completed and mate-rial on the job as of the last day of the preceding month as evidenced by our statement. In
the event there needs to be an inspection then the maximum allowable holdback will be 10% for a maximum of 30 days from completion.
3. Should default be made in payment of this contract, charges shall be added from date thereof at a rate of one and one half percent per month (18% PER
ANNUM) with a minimum charge of $2.00 per month, and if placed in the .hands of an attorney for collection, all attorney's fees, and
and legal filing fees shall be paid by Customer accepting said contract.
4. The Company shall have no responsibility for damages from fire, windstorm, torrential rain, lightning, hail, tornado, hurricane, tropical
storm, tropical depression, microburst. or any named storm, resulting in torrential rains or excessive winds, mold, fungus, or other hazard.
5. The quotation on the face hereof does not include expenses or charges for additional bond or insurance premiums or costs beyond
normal bond and insurance coverage. and any such additional expenses.premiums or costs shall be added to the amount of the contract.
6. Replacement of deteriorated decking, roof jacks, ventilators, flashing or other materials, unless otherwise stated in this contract,
are not included and will be charged as an extra charge on a time and material basis. Improperly placed gas lines, freon lines, electrical
wires, or air ducts. that are attached directly beneath decking boards, and are thereby punctured by roofing nails during normal roof
installation, are not the responsibility of the company, and all subsequent repairs or damages shall be the responsibility of the Customer.
7. This contract, if not signed, will expire 10 days from date unless extended in writing by the Company. After 10 days, we reserve the right to
revise our price in accordance with costs in effect at that time.
8. The Company shall not be liable for failure or performances due to labor controversies, strikes, fires, weather, inability to obtain
materials from usual sources, or any other circumstances beyond the control of the Company whether of similar or dissimilar nature.
9. The Company is not responsible for interior damage, mold or fungus below the roof due to excessive rain, wind, ice dams, hail. tropical storm, tropical
depression, microburst, tornado, hurricane, or rain or winds related to any named storm. The company will not be responsible for damage below the roof, if not
notified in writing, within 48 hours of the first occurrence of any leak. If the company determines that it is responsible for any interior damage below the roof due
to improper installation. including sheetrock. painting, or carpet; the customer hereby agrees to a settlement amount up to and not to exceed three hundred dollars.
10. If roofing and sheet metal. gutters, or siding is involved, it is understood and homeowner agrees that our standard one year Limited Labor Warranty shall be
acceptable and that all terms and provisions therein shall prevail. Our standard labor warranty period shall he for one year unless
otherwise stated in writing prior to commencement of the work. If the Limited Labor Warranty is extended in writing beyond one year, then
homeowner agrees that the Company is not responsible for any interior damage below the roof after one year from job completion. The
warranty excludes leaks or damages from any named storm, tropical storm, tropical depression, tomado, hurricane, microburst, lightning, hail.
torrential rains in excess of one inch per hour or winds in excess of sixty miles per hour, construction defects; or any roof vents, pipe boots, or roof
penetrations not supplied by the Company. The only purpose of the Limited Labor Warranty is to insure the correct installment of materials by the Company.
11. If material has to be reordered or restocked because of a cancellation by the customer, there will be a restocking fee equal to fifteen percent (15%) of
the contract price.
12. This contract and warranty shall not be assigned and is nontransferable.
13. If this Contract is cancelled by the Customer later than three (3) days from execution, customer shall pay to the Company twenty percent (20%) of the
contract price as liquidated damages, not as a penalty, and the Company agrees to accept such as a reasonable and just compensation for said cancellation.
14. This Contract cannot be cancelled once insurance negotiations begin or work is commenced except by mutual written agreement of the parties.
15. This Agreement constitutes the entire agreement between the parties. It may be changed only by written instrument signed by both parties.
16. If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the remaining
provisions of this agreement shall not be affected thereby.
17. Any representation, statements, or other communications not written in this Contract are agreed to be immaterial, and not relied on by either party, and do not
survive the execution of this Contract.
18. The contract must be paid in full or the warranty is null and void. If the contract is not paid in full, then the Customer agrees there is no warranty and
there is no recourse against the Company for damages above or below the roof.
19. REROOF AMERICA CONTRACTORS CO, LLC will have the right to supplement the insurance company in the event material and labor increases over
five percent (590) from the date of the damage or if labor and materials exceed the original scope of loss.
20. The Company has the right to order excess materials. All excess materials belong to the Company.
21. Supplement paid by the insurance company for additional labor and/or materials needed beyond the original scope of repairs shall be paid directly
to REROOF AMERICA CONTRACTORS CO, LLC.
22. These conditions shall be considered a part of any contract entered into or authorized to proceed, the same as if they were included therein.
23. Full scope of Insurance proceeds shall be defined as the full price for repairs allowed by the insurance company before any deduction for deductible or
depreciation are subtracted.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your C

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

The parties that (aside from deductibles and upgrades) the Work may be paid for by Customer's Insurer, but Customer acknowledges the need for help in making
a claim. Customer requests and authorizes TTHS to expend its time and expertise to assist Customer with getting an insurance claim (hereinafter 'Claim') for the
Work approved by Customer's Insurer. Customer hereby appoints TTHS as Customer's exclusive agent top communicate with and obtain approval from Insurer
regarding the Claim and the Work. Customer hereby acknowledges that a material and significant part of the value and consideration to be received by Customer
under this agreement is TTHS's assistance in getting Customer's insurance claim approved by Insurer. Customer hereby authorizes and directs Insurer to
make all payments directly to TTHS for the Work and the Insurance Claim to which It appertains, including "Total Negotiated Insurance Proceeds"; in
the event Insurer payment Is instead made directly to Customer, Customer shall endorse said payment directly over to TTHS.

This contract and any agreement made pursuant thereto is behveen Tru -Tek
Home Solutions, hereinafter referred to as the 'Co' or Company.' And the
customer(s) named herein on the Agreement page 1 will be subject to all
appropriate laws. regulations, and ordinances in the state of record
1. All contracts are subject to approval of our cred t department and office
without exception. The person executing this contract must obtain the
approval of the officer of the Co for this contract to be effective under any
conditions in the state of record.
2- SHOULD DEFAULT BE MADE IN PAYMENT OF THIS CONTRACT,
CHARGES SHALL BE ADDED FROM THE DATE THEREOF AT A RATE OF

from mold.
12. IF THIS CONTRACT IS CANCELLED BY THE CUSTOMER LATER
THAN 5 DAYS from the execution, customer shall pay to the Company
twenty percent (20%) of the insurance proceeds or $2000 00 whichever is
greater as liquidated damages, not as a penalty, and the Company agrees to

16 The maximum liability for the Co. shall be the original cost of labor and
materials for the repair which customer agrees shall be a liquidated sum,
under any event of default of Co herein.
17 During the duration of the work the customer's homeowner's insurance
will be responsible for any interior damage as long as the Company has
taken appropriate action to protect the roof during the repair of the roof.
18 If there are any solar panels on the rood, Co. wrC not be responsible for
any damage during the repair so homeowner agrees to have solar panel
company take the appropriate action to protect it if necessary
19 The Company ;s not responsible for construction problems of your
home If pointed out and notified to our Co., we wili try and assist you on
correcting them on time and material basis
20 The Company s not responsible for any damage on or below the roof
due to leaks from sky lights unless we the Company completed the skylight
replacement
21. Warranty is for 5 year on roof replacement 1 year on siding
