Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Dear Mr. Hudson:
At your request, our office performed a site observation at the above-mentioned residence to assess
structural damage to a small azea of ttte roof caused by lightning strike and provide recommendations for
repair. The conclusions and recommendations presented in this report are based on the visual
observations only, as well as our professional knowledge and experience in the engineering field and
construction practices along the Front Range of Colorado. For the purposes of this report, the front
elevation of the building is assumed to face east.
Our office directly observed the damage from the attic space accessed from the second floor hallway. The
majority of the damage occurred at the north hip point of the main roof. The strike destroyed a small area
of the roofing and sheathing approximately 5 square feet in size. The following damage was noted to the
roof structural framing members:
• The full size truss located at the hip point adjacent to the hole in the roof was damaged. The point
where the top chords and two diagonal web members meet was burned.
• A few 2x4 piece-framing members that span between truss members on the hip portion of the roof
were either burned or destroyed.
• The diagonal member of one of the jack trusses was destroyed.
Our office makes the following recommendations to repair the roof framing members:
• The damaged truss indicated above should be repaired in the following manner: the 2x4 wood
members of the truss indicated above should be sistered with new 2x4's. Attach the new 2x4's to
the side of the existing 2x4 top chord with (4) l Od nails at each end, spaced to avoid splitting, and
IOd nails at 12" o.c, between. The 2x4 sister at each top chord may terminate at the intersection of
the top chord and the diagonal web member. The 2x4 sister members should mirror the truss and
die together at the point in the same manner as the existing truss. Provide a Simpson MP36
mending plate.
• The damaged 2x4 piece-framing members indicated above should be removed and replaced w/
new 2x4's. Toe nail to framing at each end w/ 2- 16D nails.

• The damaged diagonal of the jack truss should be removed and replaced with a new 2x4. Attach to
the vertical and diagonal members with Simpson MP24 mending plates.
• Cut back damaged sheathing along the centerline of the supporting members. Replace with new
sheathing of same grade and size and attach to the supporting members with lOd nails at 6" o.c.
All framing members are to be Hem-Fir No.2 or better.
Upon implementation of the preceding recommendations, the repaired roof structure will be capable of
supporting the code required dead and live loads.
This report should not be considered a warranty or guarantee, implied or expressed, of the structure in
general, the building superstructure, or the foundation system. Structures and foundation systems may be
severely impacted by changes in climate, land-use, etc, and the conclusions presented in this report are
based on conditions noted at the time of the observation. Should questions arise, or if further information
