Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

ROOFING SYSTEM DESCRIPTION & PROPOSAL
B & M Roofing proposes to do the following work at the above referenced property;
SURFACE PREPARATION
• Remove and dispose ofone layer ofexisting roofing material.
Ifadditional layers of existing roofing material are found an added labor cost for removal will be necessary.
a Inspect plywood roof deck and re -secure any loose sheets.
• Replace rottenhvater damaged plywood after approval by Owner. If this work is required, a separate time
and material price of $65.00 per sheet will be added.
• Detach and reattach de-icing cable.
UNDERLAYMENT
• Install two courses ice and Water shield membrane at the eaves.
• install one layer of 30# felt or synthetic underlaymcnt over entire roof surface.
• Install Tarco valley flash in valley areas, closed style. Install 'open'vall eys with metal valley liner with
some systems as necessary.
FLASHINGS
• Install new 2"x 4" prefinished drip edge flashing at the entire perimeter edge of the roof.
• Install new step flashings along sidewall areas.
Install new head wall flashings where roof slope meets a vertical wall, as necessary.
• Install and paint new vent flashings on pipe penetrations,
Install new pre -painted attic vents to facilitate proper attidroof ventilation. Re -use Power attic vent.
• Replace gutter/downspout per insurance scope. ADD 00.00 to the system price below. ! 0

ROOFING SYSTEM
• Install new HERITAGE DIMENSIONAL SHINGLE,
as manufactured by TAMKO, over underlayment, Including all starter, hip and ridge.
We may be able to install a Class 4 M (Impact Rated) at no added cost.
• Color and manufacturer will be chosen by Owner, prior to material order by B&M Roofing.
• All shingles to be installed with six nails per shingle.
TAXES AND MISCELLANEOUS
• Debris caused by the roofing operation will be properly disposed ofby B&M Roofing.
• Liability insurance from Continental Western Insurance is carried by our firm for your protection.
• Workers compensation insurance is carried by our firm on all of our employees.
• Applicable sales tax on materials included in our System Price on this project
The applicable city or county building permit fee is included in our System Price, unless it is being reimburs(
by insurance. Permit costs Eire additional and Will be billed separately after all costs are in.
• We Will complete the roofing work utilizing the driveway to access the property. A roll -off garbage
container, or trailer will be located in the driveway during the re -roofing project.
Clean roofing debris out of the gutters after the re -roofing process is complete.
• We will provide a 5 -year B&M Roofing of Colorado, Inc,'s roofing guarantee on labor upon completion
of the work and receipt of final payment.
• A standard 30yr manufacturer's Warranty covering material will be provided upon completion ofthe
work and receipt of payment
• Please be &wore that while working around skylights, some dry -wall dust may be disturbed, and protection fo
property underneath of the skylights is recommended.
Among other things, painting, carpentry, and sefiit venting are excluded from System Price on this project.

Re: Build'mg Code Discrepancies at the Patio Homes Condominium Complex.
Recently three Officers of our Patio Homeowne+'s Association have visited your office to present our
discrepancies and complainu, which exist in our azea. These items have been on our agaida to crn'rect and
resolve for a period of almost two yeaz s. We hope that you can help us for a swiR and timely correction.
Many of the problems aze endangering our healffi, safety and wuse damage to our property. The number of
car alignment problems, mosquito populaNon increases, and various water caused demages> could be
controlled by proper action on the individuals responsible fa their cause• We have paid for our homes witli
legal "Amaican Greenbacks" and do demand a legal rehnn. Our "Ameaican Dream of Aomeownership"
is wearing thin•
We the undersigned, have purchased and occupied the above listed unit since 4 November 1997, and can
only say that we had nothing but serious problems fran day one. Gas leaks, bad drainage on part of the
roof causing ice build-up near the main entrance, sprinkla system leaks, bad Stting doors, end dc. Please
see the below listed discrepancies and problems. All were reported prior to our one year occupancy, lwt
many are hanging in the breeze, nor aze they being worked on with any reasanable progress or predicted
completion deadline. There is no communication between us and the builder, and some of the workmen
dispatched to solve some of the problems, are not interested in fixing the discrepancy> do not remrn to
finish a jab, or cause mae damage then existed pria. I1ie "after-the-sale" policy of this canpany is
ZILCH, and is only met by belligemcy. Yes, tha e are good points and the houses aze not constructed too
badly. If we could tum back time, we would say NEVER AGAIN. We have physical or photo proof to
back-up each statement.

4302 QUAIL STREET:
. Ol. Cracks in the brick walls outdoors, and concxete surfaces shifting and breaking up.
• 02. Kiichen side door, poor fit and bad alignment
0 03. Deck Gght broken, 'Ihe wuide lights are installed too low making them a safety Uszsrd.
. 04. All hardwood floors have washboard appearance. Builder says, because we have a swamp cooler,
however, vapcm bazriers were suggested, and this unit has nate. Otha units have vapor barriers on the
perimeter wails This unit has none.
. 05. Door to the garage nceds a new sweep. This was damaged and reported prior to t year.
• 06. Fireplace is not whaz we purcdiased and paid for. Jody says we just gave you the plain vanilla
kind, documentation from closing available. Major gas leak for flvee days.
• 07. Furnace problems, shoddY patchwrn'k on Plenum, drowned out control panel because of humidifia
leak in drain hose. Two repa'vs in fitst year.
. 08. No heating outlet in the upper bathroom. All atha similar uniu have heat outlets in the identical
batFrcoom. Building inspector says not required. Builder and owner agreed on a bathroom and walk-in
closet switch, so, we have plenty of heat m the walk-in closd but none in the bathroom.
• ap. No attic opening, a door in the uppa level. Builda' says, no room to wt in a doa.
• 60: Roof wata dramago proble*. Wata' draining from the roof near the main entrance area causes ice
m form on the steps and the entrance apron. This discrepancy is one for the record. Downspout was
changed to a larger one, displacing the house number from its original place. A dam was construc[ed
over the azea attached to the edge of the gutter, but no help. Ice was chipped out of the gutter, no help.
One of the workmen together with the owner and neighbor observed the water flowing into the valley
par[ly up the roof and appearing at different spots in the roof edge area. The workman never retusned.
That did not help. I called the builder on a Sunday afternoon when the snow melted and caused the
water to nm and freeze. That did not help, but, received a lecture in a very belligerent way about not
calling on Simdays. However, a workman showed up on Monday> inspected the situation, talked to the
owner and the neigitbor as a wihiess. He said when the sun shines he will retum and try a few things.
That was fluee weeks ago, and he never returned a called. Well, you see that did not help. After
seven attempts the problem is still there.
. 11. Swamp cooler aai the back roof is not secure.
. 12. Cancrete in the garage is shifting in the comer where the water flows from the roof.
OVERALL AREA:
. Ol. Ditch in the back of the house retains wata and incaeases the mosquito population. Also makes it
difficult for cutting the lawn
• 02. Missing (shat changed) on Guest Paricing Spaces.
• 03. Water problems in crawl spaces.
• 04. Sprinkler System Problems.
. 05. Pawing of the streets and cul-do-saq bringing them ap to code in quality and dimensian, and
repa'ving all the damaged sidewalks and driveways.
