Dear Mr. Farland,
Per your request, I reviewed photos and piers logs provided by CBS. I
also discussed the installation with the CBS foreman. I was out of
town during the installation and was unable to perform a closeout site
visit. The installation was performed by a qualified crew trained by the
manufacturer of the components used in this repair.
The two helical piers were advanced to a depth of 15 -feet before
achieving capacity/refusal. In my opinion, the piers appeared to have
been installed in general conformance with the requirements set forth
in the engineering documents.

Other Items (beyond the scope of the structural foundation repairs)
1) Although beyond the scope of the foundation repairs, it may also be
necessary to provide additional stabilization near the top of the
chimney.
2) If applicable, racked doors and windows should be reset, and
cosmetic repairs can be made three (3) months after the structural
repairs are complete.
3) It is very important to maintain proper drainage away from the
retaining wall. I do not recommend irrigating within 5 -feet of the
foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees, and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home
or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, and sump pumps if
they exist, should function properly.
I observed the structural repairs of the foundation described above.
Other unknown concerns may also exist. The Lepetsos Group, Inc.
makes no representations regarding the suitability of other areas,
including, but not limited to mechanical, electrical, plumbing, previous
foundation repairs, and other systems. The homeowner should
understand additional unforeseen and unforeseeable issues related to
soil heaving and or settlement, and exposure to unwanted water may
cause additional foundation movement in the future. My evaluation
and recommendations are based on my education, experience and
observations made on the date of my visit. The Lepetsos Group, Inc.
makes no representations regarding the ability of the property to meet
current or past building code requirements. Please note it was not
possible to observe every structural detail. Contact this office if
additional information becomes available which could affect our
observations and recommendations.

(2) Resistance piers shall be certified by Pei Eval Services PER -17107 2 7/8" dia (or equiv).
Helical piers shall be UES 391 2 7/8" dia (or equiv).
(3) Minimum required allowable axial load capacity = 24 k per pier, FSu = 2.0,
min pier depth = 30' or to capacity/refusal. Contractor shall ensure each pier achieves
minimum required capacity.
(4) Concrete shall be min fcy = 4,000 psi.
(5) The contractor shall ensure all OSHA requirements
are satisfied during excavation, and pier and counterfort installation.
(6) Reinforcing steel shall be 60 ksi per ASTM A 615

Subject: Visual Structural Foundation Evaluation for 5125 Swadley St.,
Wheat Ridge, CO.
Dear Mr. Farland,
Per your request, I visited the home at the above referenced address
on November 3, 2020. The purposes of my visit were to observe the
foundation damage reported and provide repair recommendations as
required.
Please note: My investigation was based on observation of accessible,
visible items. Issues/concerns concealed by finishes, if any exist, were
not able to be observed, and were therefore not evaluated as part of
this investigation. Issues and repairs, if required, regarding other parts
of the home, were not part of the scope of this investigation, and are
specifically excluded from this report.
Observations
For purposes of this investigation, the front of the house faced east.
The brick chimney on the south side of the house was separated and
rotated away from the exterior. Previous attempts to seal the opening
appeared to have been made.

Recommendations
1) 1 recommend stabilizing the south chimney with two (2) helical
piers per attached engineering drawings. The components used in
this repair shall be certified by the manufacturer for corrosion
resistance per the manufacturer's technical report.
2) Although beyond the scope of the foundation repairs, it may also be
necessary to provide additional stabilization near the top of the
chimney.
3) If applicable, racked doors and windows should be reset, and
cosmetic repairs can be made three (3) months after the structural
repairs are complete.

(1) This permit was issued in accordance with the pravisions set forth in yopur application and is subject to the laws of the State of Colorado and to the Zoning
Regulations and Building Code of W heat Ridge, Colorado or any other applicable ordinances of the City.
(2) This permit shall ezpire if (A) the work authorized is not commenced within sixty (60) days from issua date or (B) the building authorized is suspended or
abandoned for a Denod oi 120 days.
(3) If this permit expires, a new permit may be acquired for a fee of one-half the amount normally requiretl, pmvided no changes have been or will be made in the
original plans and specificalions and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paitl for a new permit.
(4) No work of any manner shall be done that will change the natural Flow o( water causing a drainage pmblem.
(5) Conlrector shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive writlen approval on inspection card before
proceediing with successive phases of the job.
(6) The i5"nce o( a permit or the approval of drawings and specifcations shall not be constmed to be a permit for, nor an approval of, any violation of the
