Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Alh-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€rms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

1. This permit was issued based on the information provided in the permit application and accompanyingplans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

1. Contractor shall complete the Scope of Work based on the greater of $2,500 or AIA, with NO ADDITIONAL OUT OF POCKET EXPENSE TO THE
PROPERTY OWNER EXCEPT FOR THE APPLICABLE INSURANCE DEDUCTIBLE, unless the Property Owner requests upgrades or additional work
in writing that is not approved by the insurer. AIA is valid only if Property Owner has full replacement cost insurance coverage. Infinium Construction is
required by law to NOT waive, pay or rebate property owner's insurance deductible in part or in whole.
2. The undersigned hereby gives consent and authorization for the insurance company named above to discuss the Claim, settlement terms, and supple-
mental charges I fees and release all pertinent claim information related to the applicable Claim to Infinium Construction.
3. The undersigned hereby agrees to collect and provide all documents and information related to the Claim to Infinium Construction in a timely manner.
4. The undersigned hereby assigns all amounts due from the Property Owner's insurers and/or mortgagees for work completed by Contractor to Infinium
Construction. This includes, but is not limited to: all direct costs, overhead & profit and any supplements submitted to and approved by the insurer for

Both sides of this agreement, including the terms and conditions set forth below, is between Infinium Construction and the Property Owner, and are
subject to all applicable rules, regulations, laws, and ordinances in effect as of the date of this agreement in the State of Colorado and the local juris-
diction of the property locale.
ADDITIONAL TERMS AND CONDITIONS
1. In the event that any provision of this agreement is held invalid by a court of competent jurisdiction, the remaining provisions shall nonethe-
less be enforced in accordance with their terms. Further, in the event that any provision is held to be over broad as written, such provision shall
be enforced as amended.
2. Infinium Construction liability coverage insurer is Accord All Lines Insurance Agency 303-841-8970.
3. You have the right to rescind this contract for any reason and obtain a fuil refund of any deposit within 72 hours after entering the contract. You
may rescind this contract immediately after you receive written notice from the property and casualty insurer that your claim has been denied
in whole or in part. However, Infinium Construction is entitled to retain payments or deposits to compensate Infinium Construction for roofing
work actually performed in a workmanlike manner consistent with standard roofing industry practices. Infinium Construction cannot pay, waive,
rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing work
on your property.
4. PLEASE MAKE ALL CHECKS PAYABLE TO INFINIUM CONSTRUCTION. Only payments of good funds made to and received by Infinium
Construction will be considered to satisfy the Property Owner's payment obligations. The payment schedule identified on the front page of this
Agreement may not be modified without the prior written consent of Infinium Construction. The Property Owner is expected to make a good
faith effort to make payments in a timely manner. The Property Owner shall notify Infinium Construction immediately upon becoming aware that
there is or may be a problem receiving good funds from the Property Owner's Insurer or mortgagee. If this Agreement is not subject to Agreed
Insurance Allowance, the Property Owner shall be considered to be in default if payment is not made in full within seven (7) calendar days of
the date work is completed. If this Agreement is subject to Agreed Insurance Allowance, the Property Owner shall be considered in default if
payment is not made to the Contractor within seven (7) calendar days of the Property Owner receiving good funds from the Property Owner's
Insurer or mortgagee. Should default be made in payment of this Agreement, Infinium Construction reserves the right to file a Mechanic's Lien
against the property described on the front page of this agreement. Furthermore, the Property Owner shall be responsible to pay a default fee
in the amount of five percent (5%) of the unpaid balance. If the Property Owner remains in default for more than fifteen (15) calendar days,
the default balance shall be subject to interest as a rate of 1.5% per month, compounded annually, from the date of completion of the repairs
applicable to this Agreement. In addition, the Property Owner shall be liable to pay all additional costs, including but not limited to, reasonable
attorney fees, government filing fees, court costs, and any other expenses associated with the collection of the defaulted amount.
5. If Property Owner cancels this agreement after the third business day of the date of this agreement, for any reason other than the claim being
denied or insufficient to complete the work, Property Owner agrees to pay liquidated damages of 25% of the contract amount or AIA, whichever
is applicable, for the consulting services. In addition, Property Owner may be held responsible for any other damages, including, but not limited
to, a fifteen percent (15%) restocking fee for all the materials delivered. and lost profits which Infinium Construction and/or Project Manager may
incur as a result of Property Owner's failure to perform any of the Property Owner's obligations under this Agreement.
6. Contractor's work is deemed accepted unless the Property Owner gives written notification within ten (1 0) days of the completion of such
work, specifying any and all deficiencies. All claims not made in writing by such time shall be deemed waived and released by Property Owner.
7. Infinium Construction's current written Workmanship Warranty is ten (10) years for total roof replacement and one (1) year for
