1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3, If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or grantigg of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
apploab�e code or any �rdinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

In order to facilitate this the basement will be gutted and rebuilt from the foundation up. Currently there are two
bedroom areas that will be rebuilt with a slightly different layout. These rooms will also have code compliant egress
windows in place of the current windows.
The bathroom will be relocated and a half bathroom added. In addition to the sub floor work the plumbing contractor
will correct years of modification providing all copper plumbing in an efficient and organized manner. When complete
there will be 1.5 bathrooms in total.
The laundry room will be relocated closer to the utility closet as a part of a new utility space.
The existing bar will be has water run to it but no drain. The bar will be relocated and given all the necessary
connections for a fully functional wet bar. In the bar and living space will bean added wide egress window to provide
both natural light and an additional means of egress if necessary.
During the renovation electrical will be readdress and relocated throughout. To facilitate this a sub panel will be placed
in the utility room to run all of the new lighting, switches and outlets off of. The electrical contractor has surveyed the
current power panel and has verified it has sufficient room to support the additional circuits desired.
Lastly the existing boiler and baseboard heat will be removed and replace will a full HVAC system. This system will
provide both heat and AC for the entire house.

EXECUTIVE SUMMARY
Alta Environmental LLC (Alta) provided project management and air -monitoring services for
Excel Environmental Inc. (Excel) at the property located at 3292 Vivian Drive in Wheat Ridge,
Colorado on August, 22nd 2019. Work was performed in conjunction with removal of asbestos -
containing materials from the structure. Asbestos removal, as part of a scheduled demolition of
the building, was completed by Excel of Aurora, Colorado. Excel is a Colorado Department of
Public Health and Environment (CDPHE) licensed asbestos removal contractor.
Alta's services included final visual inspections and clearance air monitoring as required in the
abatement areas. Upon completion of a visual inspection of containment after asbestos removal,
Alta collected final clearance air samples and released the work area when laboratory results
indicated airborne fiber concentrations were within acceptable limits.
Based on Alta's observations on air monitoring results, asbestos removal was completed in
accordance with applicable federal, state and local regulations. At project completion, air
monitoring results indicated that concentrations of airborne asbestos fibers within the established
containment areas were below the level of 0.01 fibers per cubic centimeter (f/cc) of air as defined
in CDPHE Regulation No. 8.

1 INTRODUCTION
Alta Environmental LLC (Alta) was contracted by the Excel Environmental Inc.to provide final
visual inspections and clearance air monitoring as required in the abatement areas at the property
located at 3292 Vivian Drive in Wheat Ridge, Colorado on August, 22°d 2019
Friable and non -friable asbestos -containing materials (ACM) were removed from the building
prior to a planned demolition of the structure in accordance with the National Emission Standards
for Hazardous Air Pollutants (NESHAP) and the CDPHE Regulation 8 Asbestos Requirements
(Regulation 8). Asbestos removal was completed by Excel Environmental Inc. of Aurora,
Colorado. Excel Environmental Inc. is a CDPHE licensed asbestos removal contractor.
2 Final Clearance Air Monitoring Sampling
Air samples were collected on 3 -piece 25 -millimeter mixed -cellulose ester membrane filters (0.8
micron pore size) designed for phase -contrast microscopy (PCM) analysis. The filters were
preassembled by the manufacturer in conductive, three -stage cassettes with extension cowls.
Rates were recorded at the beginning and at the end of the sampling period using a rotometer
calibrated with a DrycalTM primary flow calibrator.
Air samples were collected on open-faced cassettes that were positioned at breathing zone height
(approximately five feet above the floor) with the exposed portion of the cassette facing downward.
Asbestos air monitoring records were completed at the site each day that air samples were
collected. These records are designed to be the primary working document, containing most
information about air sampling activities and are attached to this letter.
Analysis of air monitoring samples under PCM was performed by Reservoirs Environmental
Laboratory of Denver, CO, in accordance with the NIOSH 7400 method. The method may be
found in the NIOSH Manual of Analytical Methods.
PCM samples are reported as fibers per cubic centimeter of air sampled (f/cc). Acceptable
clearance air samples are those at or below 0.01 f/cc for all PCM analyses. Regulation, Part B,

Section III.P.3.b explains the Colorado requirements for air sampling and acceptable
concentrations.
2.1 Air Sampling Summary
The following summaries in Table 1 provide an overview of air monitoring conducted by Alta for
the work area. Additional information such as final clearance monitoring worksheets and daily air
sampling records are included in Appendix B. In accordance with CDPHE regulations the
materials form the out building required only a visual inspection.

3 INSPECTION AND AIR MONITORING RESULTS
The work area abated was cleared for re -occupancy subsequent to final visual inspection and
clearance air monitoring. Results of final clearance sampling were at or below the EPA monitoring
guideline of 0.01 fibers per cubic centimeter of air for PCM analyses. A visual inspection of the
out building identified that all materials were removed in accordance to applicable regulations
prior to demolition.
4 CONCLUSIONS
Based on visual inspections and final clearance air monitoring results, the work area passed the
clearance level of 0.01 fibers per cubic centimeter of air for PCM analyses as set by the USEPA
and the CDPHE. At project completion, air -monitoring results indicated that concentrations of
airborne fibers were less than the concentration recommended by the CDPHE for re -occupancy of
the abated area.

5 LIMITATIONS
Alta cannot document or comment on all day-to-day activities and events with respect to
compliance with regulations and specifications. However, based on inspections conducted during
asbestos removal operations, work appeared to conform to the specifications and applicable
regulations.
This report describes the data collected and observations made by Alta during the final clearance
sampling in the above referenced building. Alta represents that its services were performed within
the limits prescribed by applicable regulations, within scope of work as approved by the EPA and
the State of Colorado and in a manner consistent with the level of care and skill ordinarily exercised
by other professional consultants under similar circumstances.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Alh-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€I"'s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
