Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Off Bial and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services

6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any

This date of service is an estimate. It may change based on the actions of your carrier, weather, or other factors.
This is a contract between AGR Roofing and Construction, Inc. ("Contractor") and the
Customer identified above and signing below ("you" or "Customer"). Customer wishes to engage
Contractor as a general contractor to perform work on property owned by the Customer, or by a
person that Customer represents, as Contractor recommends. Customer will pay Contractor for this
work as calculated in this contract. Contractor agrees to communicate with Customer's carrier to
determine the scope and price and then perform labor and furnish materials in accordance with the
scope of work approved by the carrier. Customer authorizes Contractor to communicate with the
carrier and irrevocably instructs the carrier to cooperate fully with Contractor on this claim.
Contractor's obligation to perform labor and furnish materials is contingent upon the carrier
approving the recommended services or materials and authorizing sufficient funds. if a deposit is
paid, Contractor will hold it in trust until roofing materials have been delivered to the work
site or a majority of the scope of roofing work on the property is done.

1. PAYMENT. You are personally obhganxl to paythe final price for all work and materials
provided by Contractor. The final price will bedeterrmnedbycalculating the Replacement Cost
Value (RCV} of the work performed. You will pay the RCV amount calculated by your
insurance company. In addition to this, you also agree to pay the amort of any insurance
supplements and any costs, including profit and overhead, related to necessary- wok not

L DUE DATE Payment equal to the Actual Cast Value (ACNI ofyour insurance claim is due

accepted If any dispute arises, you must send all payments, including those offered as
settlement or payofL to Contractor's emanneys and not to Contractor directly.
3. LATE PAYMENT. If a payment is not received by Contractor within 5 calendar days of
the due date, you arc in default. In that event, you will pay a one-time 10% late fee an the late
payment. The total amount owing shall bear interest from the date due at the lesser of 18%per
annum compounded annually or the maximum rate permitted by law. Payment may not be
withheld (and late payment is not excused) by allegations ofimprope work, set -o% waiting for
inspections, or for any other reason. You are solely responsible to pay for all of Coutractor's
costs, damages, and attomey fees for any dispute related to this contract. Contractor has a
security interest in the property at which the work is performed for any amount due under the
terms of this contract. This interest may be enforced by filing a mechanic's lien. You agree to
waive your homestead exemption, if any, with respect to any lien, foreclosure, collection, or
judgment. The security interest and lien arises under C.RS. § 38-41-212 and your homestead
exemption is not applicable. These remedies are cumulative and nothing in this section limits
Contractor's rights under Colorado or other law.
4. INSLRANCE. 4(s). Communication. You authorize Contractor to comtact and negotiate
with your carrier to file a claim on your behalf and determine the scute of work, Replacement
Can Value, Supplements, and all other issues related to the facts ofthe work and the price. You
imsttuet your tamer to cooperate with Contractor, and agree that you and your insure shall
cooperate fully and in good faith with Contractor. You authorize and instruct your carrier to
include Contractor as a payee on all payments related to this contract. You instruct any
mortgage comnpany *be may come into possession of inatrmtce f ands to unrrediately convey
any insurance funds received to Contractor. These directives are irrevocable and you
understand Contractor is relying on them.
4(b). Scope of Work Because the servius rendered byCenrsetormaybe inte+ded to address
an emergency situmirm it may not be possible to determine the final scope of work and prior
before workbegims. Contractor will undertake reasonable efforts to agree on a scope of work and
price with yaw comer, but cannot guarantee that your carrierwill agree of approve the work.
In the event that Contractor and your comer agree on a scope of wok, you agree that Contractor
will perform all work in that scope for the prices identified in that scope unless you and
Contractor agree, in writing which specific items an the scope of work will not be undertaken,
4(c). Adequacy. Contractor's obligation to perform the scope of work tadeir this contract is
contingent upon your carrier authorizing sufficient limits to complete the project. It; in
C'ontractor's sole discretion. Contractor determinas that your currier cannot be made to pay
funds commercially reasonable and sufficient to complete the project, Contractor may cancel
(his contract without penalty to Contractor.
4(d) Astigm ent You today assign to Conructa the right to collect funds that may be owing
to you or paid by yam carrier to you related to Contractor's scope of wort. Contractor will
receive all funds paid by your cattier, tmlgss you and Contractor reach a specific written
agreement, signed by both parties, specifying which potions ofthe insurance scope ofwurk will
net be performed and the total amount of related insurance funds that will conveyed to you.
4(t) Funds Held in Trust. Ifyouor any third party, including any mortgage company, receive
army money from a carrier, to the extent you owe or will owe funds to Contractor, that matey is
owned by Contractor and shall be held in trust for Convector. The funds will not be subject to
any claim or lien by any cher person, and you indemnify Contractor against any damages or
costs related to the sarmc. You agree that you or anyone else possessing or disbursing these
finds will be subject to liability, including liability under C.R.S. §§ 18-4-401, er seq_
4(f) No Withholding by Mortgage Company. Ifany insurance finds; come intothe possession
of or must be endorsed by an entity holding a mortgage or other rmerest in the real property

mortgage company's compliance with this provision, including signing levers instructing it to
release tincts or provide its endorsement. You acknowledge that delays in payment caused by
your mortgage company may expose you to late fees, interests, and default under this contract.
5. WARRAN7'IESANDLIABILITY. 3(n.) Injuries. You agree not to enter orpermit others,
including pets, to enter under the work area while wok is ongoing or to approach Contractor's
equipmemtormaterials. C)NTRACTORISNOTRESPONSIBLEFORINIIIRB:STOTHOSE.

5(b) Hazard Testing. You represent to Contractor thus, to the best of yoo, knowledge,
understanding, and belief, your home docs not contain asbestos ormyother hazardous material.
Nevertheless, ifContractor determines that a test for asbestos or other environmental hazard is
necessary, you agree to pay the costs for such a test and approve delay related to such resting.
5(c) Incidents] and Consequential Damages. You agree that Contractor is not liable fur my
damages ofany kind, %father direct. consequential, incidental, or otherwise, resulting from the
actions of Contractor or its subcontractors, and you agree to indemnify and hold harmless
Contractor and any of its employees, agents, or suMwttaaelots from any claim. Thus includes
damages related to Homeowner's Association rules or penalties, disruption to business, team
anrplaints, danagc Ito uti lily lines located underneath decking, mold or water damage, or any
otheralleged damage orharm resulting from Contractor's work. Coraraeur's liability is tinted
to its % aranty.
5(d). Pre -Existing or Hidden Conditions. C'anractor is not responsible for inspecting the
gutters, or nasnm& Ku rl work can change the land on the structure, leaning to cmcXm&
distortion, changes in the foundation, ur tither manifestations of hidden defects or
unavoidable events. You acknowledge that discovery of any such deficiencies at hazards
may allow the scope ofthe work, cattle damage, increase the price ofthe work, or cause delay
or additional testing, at Contractor's sole discretion.
5(e). Hanging Items and Attachments. Roofing wok creates vibrations that may damage
hanging items, so you agree to remove ileus banging from inside and outside walls, soffits,
and ceilings. [fit is necessary or prudent to remove roof mounted equipment to perform
wort you will remove and reinstall the some at your cost Contractor may remove the
equipment if you have nor done so in order to complete work arid, if it does, will have no
obligation to reinstall m align the equipment.
5(I) Warranty and limitation. Contractor warrants that its work will be performed in a
good and workmanlike manner and that your ruof will pass any inspection required by law.
This warranty lasts 2 years Goan the date wall is complete. You agree to notify Contractor
within 72 hours ofdisconecringanyproblem with its work andto inanediately takemistmable
action to prevent any further damage and allow Contractor to inspect and cure my alleged
defects. Ifyou do not, Contractor shall haven liabilityand its wrim my shall be void. Upon
proper notice, Contractor will investigate the problem. Multiple investigations may be
necessary. If Contractor deternines that furtherwork is required, it will cooperate with you
in ensuring that wok is completed. You agree to cooperate with such inspections or
additional work by providing to Contractor access to your property at reasonable times.
Some ofthe material used tnyour project maybe coveted by manufacvucr's warranties, and
if so, those warranties me u affectod by this contract. Contractor will transfer all material
warranties to you upon payment in full. Contractor is not responsible for work done by
others. if you have another canratttr finish. redo, or perform destructive testing on
Contractor's work without Contractor's prior written approval, all warranties are void.
EXCEPT AS SPECIFICALLY %WrrEN HERE, CONTRACTOR MAKES NO
WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTS OF FITNESS OR
MERCHANTABILITY. ALL WARRANTIES ARE VOID IF YOU BREACH THIS
CONTRACT OR IF YOU FAIL TO TIMELY PAY ANY AMOL1\7 OWING.
5(g). Accidents or Acta of God. You agree that Contractor is not responsible for damage
farm fire, rain, windstorm, hail, tornadoes, terrorist attack, distortions, failure or cracking of
the roof deck, walls, or foundation, hurricanes, or ohm hazards to work in progress,
completed work, or the property or your personal property. You agree to maintain in force
a policy of insurance covering casualty to your property from such conditions.
6. APPRAISAL I PUBLIC ADJUSTING. Contractor's communications with yaw carrier
are limited to obtaining facts surrounding a loss or famishing technical assistance of an
incidental nature. Contractor is not acting as your public adjuster or other agent and has no
fiduciary duty to you. You shall indemnify and hold Contractor harmless from all costs or
damages related to its communications unications with your carrier a tender. If you hire an appraiser
or public adjuster related to this ptsrperty, that you will give a right of first refusal w an
appraise or public adjuster reco®rendel by Contractor.
7. CANCELLATION I RESCISSION. You may only cancel and rescind this contract by
delivering written notice to Contractor atl300 Carr Street, Lakewcud, Colorado 80214,
received onor before 72 haus after. (1) you enter imothis contract; or(2) your clearer denies
all m part of you claim You may give notice by personal delivery or mail You may also
cancel or rescind this contract under C.R.S. § 6-22-104. If you cancel before Contractor
begins work, you agree to pay the Contractor the sum of 206/ of the final price as
compensation for Canraetor's preparations as liquidated damages. If you cancel after
Contractorhas performed work, you will pay the price for amwork completed already and
such work as is necessary, at Contractor's discretion, to prevent damage or lass to property
atter cancellation.
S. EXCESSMATERIAL. AsanunavddaMeconsequeaceofConu=tWseffomtoenstre
your project is properly completed, there may be excess materials purchased. If so. those
excess materials remain the property of Contractor.
9. AUTHOR1717Y. Contractor is not obligated under this contract until it is signed and
approved by an officer of the company, an authorized agent, or work has commenced. A
Sales Representative is not an officer or agent ofthe Contractor.
10. ASSIGNMENT AND BENEFICIARIES: This contract, inhaling the limited
warranty, is non-transferrable, and you may not assign or delegate airy of your rights or
obligations. There are an Hurd -patty beneficiaries.
11. GOVERNING LAW. This contract shall be governed by the laws of Colorado. All
lawsuits or other disputes related to or arising tinder this contract shall be brought only in
Arapahoe County, Colorado, or in the County in which the property is situated at

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.
