COMMERCIAL FLAT ROOFS: In order to pass a final inspection on commercial elastomeric or
similar type roofing, a WARRANTY LETTER from the manufacturer technical representative
stating that "the application of the roof at (project address) has been applied in
accordance with the installation instruction for (roof material brand name) roof covering"
is required to be on site at the time of final inspection.
LADDER MUST BE SET FOR ALL ROOF INSPECTIONS: A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections.
ALL PITCHED ROOFS REQUIRE A MIDROOF INSPECTION REGARDLESS OF PITCH.
MIDROOF & SHEATHING: Effective December 1, 2014, asphalt shingle installations require an
approved midroof inspection, conducted when 25-75 percent of the roof covering is
installed, prior to final approval. Installation of roof sheathing (new or overlay) is
required on the entire roof when spaced or board sheathing with ANY gap exceeding one half
inch exists. Sheathing and Mid -roof inspections may be called in at the same time, 100
percent of the sheathing must be complete and 25-75 percent of the midroof may be complete.
Asphalt shingles are required to be fastened to the roof deck with a minimum of 6 nails per
shingle. A ladder extending 3 feet above the roof eave and secured in place is required to
be provided for all roof inspections.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with Building Division Policy
BD -18-001 for Balanced Ventilation Systems, or manufacturer installation instructions,
whichever is more stringent.
FOR ROOF PITCH 6/12 AND OVER: Midroof & Final Roof inspections for ROOFS with 6/12 PITCH &
OVER: 3rd party inspection will be required for both the midroof and final inspections. The
3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped off to the
Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the Homeowner.

I . This permit was issued based on the information provided in thepermit application and accompanying Flaps and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Isuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of an manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

(1) This permit was issued in accrordance with the provisions set forth in your application and is subject to the laws of the State of
Colo2do and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable ordinances of
the City.
(2) This permit shall expire if (A) the work authorized is not commenced within sizty (90) days from issue date or (B) the building
authorized is suspended or abandoned for a period of 180 days.
(3) If this permit expires, a new permit may be acquired for a fee of one-half the amount nortnally required, provided no changes
have been or will be made in the original plans and specifcations and any suspension or abandonment has not exceeded one
(1) year. If changes have been or if suspension or abandonment exceeds one (1) year, full fees shall be paid for a new permit.
(4) No work of any manner shall be done that will change the natural flow of water rausing a drainage problem.
(5) Contractor shall nodfy the Building Inspector twenty-four (24) hours in advance for all inspedions and shall receive written
approval on inspection card before proceeding with successive phases of the job.

ANDERSON PARK BLEACHER REPLACEMENT

Scoae of Work: The City of Wheat Ridge is requesting the construction of an ADA
compliant concrete ramp with stamped or patterned side walls. The ramp must have
ADA compliant handrails and drainage for brick house gutter system. Other work will
include concrete sidewalk and the R and R of existing concrete stairs. Construction of
ramp and sidewalk will be at the Wheat Ridge Historical Park located at 4610 Robb St
All sidewalk concrete is to be 4 foot wide, 4 inch thick; fiber mesh and wire mesh
reinforced, broom finish with saw cut control joints. All concrete is to be colored and
4000 PSI. Raised ADA ramp shall be rebar reinforced to the City of Wheat Ridge's
Building Divisions specifications. Work shall include demolition and disposal of existing
concrete, excavating, grading, forming, compaction, pouring, back filling forms with soil,
finishing, curing, and saw cutting control joints. Bidder must provide all equipment,
materials, and labor. The contractor shall contact the Cities building division at (Chad
Root) 303-235-2853 for all questions pertaining to ADA compliance, no-cost buiiding
permits and building code requirements. Bidder must provide all equipment, materials
and labor. Work times are Monday-Friday 7:00 am - 4:30 pm. Anticipate start date
September-October 2005. Completion date 9/7/05
Minimum Reauirements: Awarded Contractor must obtain a business license in the
City of Wheat Ridge, obtain a no-cost permit to do the work and provide a certificate of
insurance. Work must be completed by 917/05. At least 50% of the project must be
performed by Contractor
Deadline for Questions: 12:00 Noon, 1/9/05 2:00 pm.
Bid Due Date: Bid due 9/205 at 2:00pm Fax to 303-467-5901 Attention: Mark Ruote or
drop off the Parks offices at 9110 W 44t" Ave Wheat Ridge 80033.
Comments: Late receipt of bids will not be considered.
Citv Resaonsibilities: City shall locate all City owned utilities. A final walk through with
a City representative and the contractor shall be held to assure all conditions of the bid
documents have been completed to the Cities requirements.
Contractor Responsibilities: The contractor shall keep the work site clean and safe
during all times of the project. Contractor shall be responsible for all utility locates. The
contractor shall be responsible for the repair of any damage to City property. Final
cleanup will include removal and disposal of all debris, soil and back filling edge of
concrete with soil. All work with vehicles shail be done from parking lot on south side of
Park or Robb St. no vehicles will be allowed on turf areas. Contractor is responsible to
watch concrete and prevent damage from vandalism or inclement weather. Damage to
concrete from vandalism and inclement weather shall be repaired at contractors
expense.
