Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

contractors only. This type of permit is ONLY being processed online --do not come

This house was examined with the intention to install a new concrete roofing tile system
on the house. We examined the attic to see the roof supports, the house for existing
drywall cracks or other evidence of movement. There are always areas that are not
observable and we attempted to determine the framing method in those locations.

On May 23, 2018, we examined the framing in the attic of the house. The trusses have
relatively short top chord spans with the exception of a large combination truss. The wood on
that truss is rated as Doug -Fir. The spans are acceptable for the predicted loading. The trusses
are stamped 30/55. This indicates that they were designed for a tile roofing system.
When the roof is torn off, the roof deck should be checked to verify that the sheathing is
nailed using 1 fastener every 6" along the seams and 12" on -center in the field of the
sheathing.
Please note, anytime you reroof there is a rapid change of loading as the old roof is removed
and the new roof installed. This change in loading, movement of materials and men, and the
pounding that occurs, can cause small drywall cracks. These are not structural. If any
cracking is observed, please call our office so we can make sure that there is nothing else that
may have occurred.
There was no significant storage in the attic space. It is assumed that no additional storage
will be done in the attic. Loading the bottom chord of trusses can change their behavior. If
heavy items are to be stored in the attic consult with CSS, Inc. to make sure the loading is
acceptable.
This opinion is based on the conditions observed. There are sometimes areas of framing that
are not observable. Calculations for acceptability are based on the conditions observed using
current versions of analysis software. Change of use or loading by conditions outside of those
observed can change the results of the analysis. Alterations to the framing will require a new
analysis.
This house can c9rry an 11 psf concrete tile roofing system.

am. The following are comments that shall be addressed priorto acceptance from the Public Warks Department:
1. The soil that is in the gutterpan along the frontage ofthis property is a result ofthe construction at this address.
This material shall be removed from within the City right-of-way.
2. There are several sections ofwalk that have been damaged that are to be replaced prior to issuance ofthe C.O.
These sections have been located with orange paint on the damaged sections.
3. The asphalt along the frontage ofthis address could not be properly inspected due to the material in the gutter
and asphalt azeas. The Public Works Department reserves the right to inspect these areas forpotential dunage
to the asphalt after the removal of this material.

The sum of four thousand seven hundred twenty-five dollars ($4725.00) will be held in escrow with the
City of Wheat Ridge until all street improvements and repairs are completed at 3845 Pierson Court,
Wheat Ridge, Colorado 80033.
The City of Wheat Ridge shall have the absolute right to withdraw the escrow funds for the purpose of
completing the street improvements and repairs at 3845 Pierson Court, Wheat Ridge, Colorado 80033.
The City of Wheat Ridge further agrees that 3845 Pierson Court shall have no further obligation to make
any payments for such street improvements.
Mr. Bob Salvado understands that the obligations to make such street improvements and repairs are Mr.
Salvado's; but with the funds in escrow, is relieved of such obligation and that Mr. Salvado gives up all
rights, interests or claims to the escrow funds and such escrow funds are surrendered or relinquished
to the City of Wheat Ridge as of May 31, 2001.

Dear Mr. Abeyta:
A letter was faaced to us by 3D Piering on November 15, 1999, showing the depth and final blow counts of
the pipe piles installed at the above referenced address. The final blow counts range from thirty-one to
thirty-eight (31-38) for the set of piles, and the depth reaches thir[y-two feet or greater (32'+) according to
the pier chart compiled by 3D Piering. The relative placement of each pier labeled on the pier chart could
not be determined because no reference plan was included with the chart.
Because the installation of all pipe piles was completed without any inspection from KLP Consulting
Engineers, a test pile was installed. KLP representatives were present on November 19, 1999, for the
installation of the test pipe pile and again on November 22, 1999, for the actual testing of this pipe pile.
Jim Kelsey of 3D Piering and his crew installed the test pile to a depth of approximately thirty-three feet,
five inches (33'-5") at which point refusal was reached and the final blow counts exceeded forty (40)
without further penetration of the pipe pile. For anchorage purposes, four helical piers were installed in
evenly spaced quadrants surrounding the pipe pile. Using a system of steel beams and a hand-pumped,
hydraulic jack, an initial load of three kips had been placed on the pile. All deflection gauges were then
zeroed. The pipe pile was loaded in 6,000 pound increments over five minute intervals, and vertical
deflection was measured at each interval. The data recording during the testing of the pipe pile has been
compiled and is attached at the end of this repor[.
Based on these numbers, we feel that the capacity of each pipe pile installed at the above refere ed address
is adequate to support the design loads of Abeyta Model B3185. The locations of the existing ' e piles
have been confirmed and are adequate. All pipe piles have been fully grouted and ' h a length
