The mezzanine floor consists of the following components: I perfon structural calculations on the components of the mezzanine floor system. Calculations were based on 2009 IBC table 1607. 1, light storage live load of 125 psf. 6/17/13, 13-1032 2 I initially found that several of the floor system components and connections did not have adequate structural capacity to support design loads. Several support joists, beams and connections had to be reinforced in order to meet structural criteria to support design loads. I provided structural reinforcing recommendations to strengthen the joists, beams and connections in question. On June 11 and June 14, 2013, 1 inspected the joists, beams and connections following the reinforcing installation. I found that the members had been adequately reirdorced to support design loads. Ac the time of the observation, the Eoundaqon was 5nished, the concrete floor slab was complete, and most of the reinforcement and embed plates witiiin the toncrete tilt-up panels were in place. IJpon observing the fabrication of the cancrete tilt-up panels, ic appeared that the size and spacing of the reinforcement conformed to strucnual drawings. T'he embed plates were located properly. puring my visit i was in conversation with Glenn Massorotti. We discussed the consolidation of the conerete during the plaezment process. We slso address the proper concrete cover needed. A few horizontal reinforcement bars at window jamb iocations needed to 6e cutoff to ensure 1" concrete cover. Glenn ussured me the bars would be shortened. Overall, the reinforcement and the embed plates appesred co conform to the strucrural drawings. My next observation will include [he structural neel packsge, i.e. columns, steel joists, meuanine, and roof deck. As requested, density tests were taken on a periodic basis on the fill being placed at the site of the subject project. Submitted are Daily Report Ntunbers 1 and 2 covering observations of the fill through the period ending September 6, 2001. The density test results indicate that the fill in the areas tested is being placed according to the proj ect specifications which have been given us. These tests should be considered as periodic observations. The placement of the fill was not continually observed by our firm. The Contractor has been informed of the field density test results. The referenced Proctor compaction test value of 115.4 pcf maximum dry density at 14.0% optimum moisture content is from a representative sample taken at the site for laboratory testing. At the time of my visit, all the alt-up panels were in place. T'he steel members, excluding the stairs, were erected. The roof and mezzanine decks were futened to tlie steel joists and ihe masonry wnll was about 30% complete. Upon observing the connection of the tih-up concrete panels w the footing embed pla[es, the weld size and leng[h appeared to be in confonnance wich the contract documents. T'he non-shrink beneath the panels wa5 properly in place. The steel members appeared ro be in compliance with die structural drawings except for the wide flange beam connection az the east entry. Due m the difficulty of placing the heam, an alternative coanection was made. 7'his cannection consisted of construcdng a saddle in lieu of the direct bearing plate. The connection appeared to sufficient. After observing the Eastening of the roof deck as well as che mezzanine deck to the s[eel joists, I found die weld size, spacing and worlmenship conforming to the srrucwral drawings. The deck angle wss in place and was the proper size. The welds connecting the deck and to the wall appeared to be correct size and lengtli. Lastly, I observed the consavction of the south masonry wall. The reinforcemeat size and spacing is in compliaoce with the strucnual drawing.s. The grout cells appeared m be full. This reporc completes our observation of the project. A separate permit will need to be obtained from the Building Inspection Division of the City of Wheat Ridge for the installation of the fire alarm system 2. The Electrical Inspector for the City of Wheat Ridge will conduct the ruffwiring and 120 volt power connections. The Fire Marshai's Office will aLso need to conduct a ruff location inspection and witness the acceptance test. 3. Please provide lock on circuit breaker providing power to the fire alarm panel. 4. Sprinkler control valves will need to indicate a trouble condition to the supervisory agency. Is this a distinctly different signal as required by the fire code to indicate that the sprinkler valves aze being closed? 5. The location of the outside horn and light may change as we have not yet seen sprinkler plans. 6. Audibility tests will need to be conducted to determine that a minimum of 15 decibels above the ambient noise level is obtained in the presence of the Fire Mazshal's Office. If resuhs aze not achieved, additional units may be required. Two telephone lines will need to be provided to the Sre alarm panel or DACT. One will need to be a dedicated line for the fire alarm panel. The second may be a business line. 8. The installer shall provide written documentation that the system has been installed and tested in accordance with approved plans and specifications. A copy ofthe Certification form is attached. 9.
