Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change. of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

1. Owner's Authorization & Contractor's Obligations. I hereby retain and authorize Design Roofing LLC and its joint
venture partner SF Construction Management ("Contractor") to perform certain reconstruction and repair work ("Work") on
my Property made necessary by the above-described Loss event. Contractor will furnish all labor, services, equipment, and
materials to make the necessary restoration repairs. All Work will be completed in a workmanlike manner according to
industry standard practices.
2. Work Scope.

3. Price and Payment Terms.
• $10,889.00 + cost of permit
The Price does not include any additional work that may be necessary to make the repairs to correct the damage caused by
the loss event giving rise to this claim, nor does it include any additional work to address unforeseen conditions. Such work
shall be invoiced at Contractor's cost plus 20%. Fifty per cent (50%) of the total Price shall be due the day work begins. The
balance shall be due within three (3) calendar days of completion. Punch list items identified during the final walk through
will be placed on a warranty list and will not be subject to subtractions from final payments. Interest shall accrue on
payments not received within such time at the lesser of (i) the maximum lawful interest rate or (ii) one and one-half percent
(11/2%) per month. Should Contactor retain the assistance of a third party, including without limitation an attorney, to assist
with collection of unpaid amounts due and owing, Owner agrees to pay Contractor's costs associated therewith including
without limitation reasonable attorneys' fees, court costs, and interest at the maximum legal rate.
5. Right to Cancel & Acknowledgement. I understand that 1 may cancel this Work Authorization at any time before
midnight on the fifth business day after I have signed this work authorization. 1 have read and agree to this Work
Authorization and the Standard Terms and Conditions on the backside of this Work Authorization.

STANDARD TERMS AND CONDITIONS
"All capitalized terms herein shall have the meaning assigned to them below and on the opposite page of this Work Authorization.
general Notice to the Owner. You are entitled to an exact copy of the paper you sign. This Contract and all materials and Work are subject to the
following Standard Terms and Conditions. Any additional, different or inconsistent terms proposed by Owner, whether in writing or otherwise, are hereby
objected to and rejected. Neither Contractor's acceptance of this Work Authorization nor performance of Work shall constitute its assent to any different
or additional terms specified by Owner.
Cancellation The parties agree that either party may cancel this Work Authorization for convenience within five (5) days of signing. The parties further
agree that if Owner were to cancel this Work Authorization outside of the 5 -day cancellation period set forth herein, it would be difficult to determine
actual damages incurred by Contractor. Based on the amount of time involved, effort expended, and material and labor costs incurred during the pre -
construction period, the parties agree that 5% of the total amount of any settlement paid by Owner's insurance company or companies in connection with
the Claim or $500.00 whichever is greater, is a reasonable estimate of the damages that Contractor would accrue if Owner were to cancel this Work
Authorization outside of the 5 -day cancellation period. Contractor and Owner agree that the amount of liquidated damages is fair and reasonable and
would not act as a penalty.
Limited Warranty. Conditioned upon payment in full of all amounts due Contractor, Contractor warrants that the Work will be free from defects for a
period of three (3) years from the first day the Work is occupied or is ready to be occupied (whichever occurs first) by Owner. For a complete roof
replacement, Contractor warrants that the roof Work will be free from defects for a period of five (5) years from the date of completion. Contractor makes
no warranty to Owner regarding materials installed (other than a warranty of title). The only warranties applicable to the materials installed are those, if
any, extended by the respective manufacturer. Upon request, Contractor shall furnish to Owner any and all applicable warranty documents. Contractor
hereby assigns to Owner, without recourse, any applicable warranties extended to Contractor. Such assignment shall constitute Contractor's sole
obligation and Owner's sole exclusive remedy from Contractor with regard to defective materials installed. This limited warranty is in lieu of all other
warranties, statutory or otherwise, express or implied, all representations made by Contractor, and all other obligations or liabilities respective of the Work
provided at the Property. Contractor's Limited Warranty specifically excludes defects caused by structural or seismic movement. Unauthorized repairs or
attempted repairs shall void this warranty entirely.
Exclusions to Limited Warrantyi Contractor shall not be liable during or following the Warranty Period for any: (a) damage due to ordinary wear and
tear or abusive use; (b) defects that are the result of characteristics common to the materials used; and/or (c) loss, injury or damages caused in any way
by the weather elements. Contractor's limited warranty specifically excludes cracking, etc. of any concrete, drywall, plaster, caulking, sealant, tile, or any
other product subject to movement of any kind. This warranty also excludes any loss caused by or consisting of any mold or microbial growth whether or
not caused by Contractor or its subcontractors or suppliers. Owner further agrees to hold Contractor harmless from all claims for personal, professional,
or property damage related to mold, microbial growth, fungi, mildew.
Disclaimer of Implied Warranties CONTACTOR DISCLAIMS ALL OTHER WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WITHOUT
LIMITATION ANY IMPLIED WARRANTY OF WORKMANLIKE CONSTRUCTION, IMPLIED WARRANTY OF HABITABILITY, IMPLIED WARRANTY
OF FITNESS FOR A PARTICULAR PURPOSE OR USE, AND/OR IMPLIED WARRANTY OF MERCHANTABILITY.
Limitation of Liability. Under no circumstances shall Contractor be liable to Owner for loss of time, loss of use, inconvenience, or any other incidental or
consequential damages that may arise from this Work Authorization. Contractor shall not be liable for any damages or losses caused directly or indirectly
by vibrations during the course of the Work.
Pre-existing Conditions Contractor assumes no responsibility for inferior workmanship and/or materials pre-existing at the Property, damage due to
termites, rot or deterioration, and/or violation of zoning rules/laws.
Hazards & Toxins- Contractor shall not, without further written agreement, be responsible for the identification, testing, sampling, removal, abatement or
remediation of any environmental pollutant, contaminant, hazard, toxin, condition, irritant, material or problem, including but not limited to molds, fungi,
bacteria, mildew, myotoxins, spores or other irritants of any kind or the correction of building, fire, or housing code violations, should they exist.
Asslanment of Benefits If this Work Authorization is for the repair of damages that are compensable under the terms of a contract for insurance
between Owner and one or more insurance companies, Owner agrees that any settlement made between Owner and such insurance company or
companies shall include Contractor as payee and that Contractor may present a signed copy of this Work Authorization or a photocopy of the same to
any such insurance company to include Contractor as payee on any settlement payment made to Owner. Should any amounts paid by any insurance
company to Contractor be insufficient to pay the agreed upon price herein, Owner agrees to pay the difference between any such insurance payment and
the agreed upon price. The holder of any mortgage or lien, including purchase money liens, against Owner's Property is authorized and directed to
release any insurance draft, insurance payment, or the contents of any escrow account received or held by it directly to Contractor after first obtaining
Owner's endorsement thereon, and to include Contractor as a named payee on any draft issued by any such holder.
Work Schedule. The work will begin within twenty (20) business days after execution of the Work Authorization. Due to the nature of the reconstruction
process, no completion date is specified. No verbal agreements for completion shall bind Contractor to a specific completion date. Performance of this
Work Authorization is subject to delays caused by government agencies, labor strikes, fires, acts of war or terrorism, acts of God, adverse weather
conditions not reasonably anticipated, unusual delays in transportation, Contractor's ability to obtain materials, and/or any other cause beyond
Contractor's control.
Trip Charges. Contractor shall coordinate the scheduling of the work start date with Owner, who may be subject to a trip charge of $150.00, to be
determined at the sole discretion of Contractor, should Owner cancel the work start date within (6) six hours of the scheduled time work is to begin.
Supervision Responsibllity. Contractor shall supervise and direct the work at Owner's Property, using reasonable skill and attention. Contractor shall
be solely responsible for the construction means, methods, technique, sequences, and procedures for all work performed at Owner's Property pursuant
to this Work Authorization. Owner shall not interfere with Contractor's work forces, subcontractors, equipment, and/or material stored on site.
Insurance. Contractor represents that it carries public liability insurance in connection with the work performed pursuant to this Work Authorization.
Severability. Should any part of this Work Authorization be adjudged to be void, unenforceable, or contrary to public policy, only such void or
unenforceable portion shall be stricken and eliminated hereof while the other portions remain valid and enforceable.
Performance. If Owner fails to perform any of Owner's obligations herein or if Contractor, in good faith, believes that the prospect of payment or
performance to be impaired, Contractor may upon seven (7) days written notice to Owner terminate this Work Authorization while retaining all mechanic's
lien rights as well as right to payment for the full amount of work performed plus reasonable overhead and profit, interest, attorneys' fees, and other
charges due and unpaid.
Agreement. Owner and Contractor acknowledge they have read and understand each of the foregoing provisions and the terms and descriptions
contained in the Work Authorization, Estimate, and other contract documents prior to execution and agree to Comply with each term and condition without
reservation. This Work Authorization constitutes the entire agreement between Owner and Contractor. No agreements, representations, or warranties
other than those specifically set forth herein shall be binding on any of the parties unless set forth in writing and signed by both parties.

Pre-existing Conditions. Contractor assumes no responsibility for inferior workmanship and/or materials pre-existing at the Property, damage due to
termites, rot or deterioration, and/or violation of zoning rules/laws.
Hazards & Toxins: Contractor shall not, without further written agreement, be responsible for the identification, testing, sampling, removal, abatement or
remediation of any environmental pollutant, contaminant, hazard, toxin, condition, irritant, material or problem, including but not limited to molds, fungi,
bacteria, mildew, myotoxins, spores or other irritants of any kind or the correction of building, fire, or housing code violations, should they exist.
Assignment of Benefits. If this Work Authorization is for the repair of damages that are compensable under the terms of a contract for insurance
between Owner and one or more insurance companies, Owner agrees that any settlement made between Owner and such insurance company or
companies shall include Contractor as payee and that Contractor may present a signed copy of this Work Authorization or a photocopy of the same to
any such insurance company to include Contractor as payee on any settlement payment made to Owner. Should any amounts paid by any insurance
company to Contractor be insufficient to pay the agreed upon price herein, Owner agrees to pay the difference between any such insurance payment and
the agreed upon price. The holder of any mortgage or lien, including purchase money liens, against Owner's Property is authorized and directed to
release any insurance draft, insurance payment, or the contents of any escrow account received or held by it directly to Contractor after first obtaining
Owner's endorsement thereon, and to include Contractor as a named payee on any draft issued by any such holder.
Work Schedule. The work will begin within twenty (20) business days after execution of the Work Authorization. Due to the nature of the reconstruction
process, no completion date is specified. No verbal agreements for completion shall bind Contractor to a specific completion date. Performance of this
Work Authorization is subject to delays caused by government agencies, tabor strikes, fires, acts of war or terrorism, acts of God, adverse weather
conditions not reasonably anticipated, unusual delays in transportation, Contractor's ability to obtain materials, and/or any other cause beyond
Contractor's control.
Trip Charges. Contractor shall coordinate the scheduling of the work start date with Owner, who may be subject to a trip charge of $150.00, to be
determined at the sole discretion of Contractor, should Owner cancel the work start date within (6) six hours of the scheduled time work is to begin.
Supervision Responsibility. Contractor shall supervise and direct the work at Owner's Property, using reasonable skill and attention. Contractor shall
be solely responsible for the construction means, methods, technique, sequences, and procedures for all work performed at Owner's Property pursuant
to this Work Authorization. Owner shall not interfere with Contractor's work forces, subcontractors, equipment, and/or material stored on site.
Insurance. Contractor represents that it carries public liability insurance in connection with the work performed pursuant to this Work Authorization,
Severability. Should any part of this Work Authorization be adjudged to be void, unenforceable, or contrary to public policy, only such void or
unenforceable portion shall be stricken and eliminated hereof while the other portions remain valid and enforceable.
Performance. if Owner fails to perform any of Owner's obligations herein or if Contractor, in good faith, believes that the prospect of payment or
performance to be impaired, Contractor may upon seven (7) days written notice to Owner terminate this Work Authorization while retaining all mechanic's
lien rights as well as right to payment for the full amount of work performed plus reasonable overhead and profit, interest, attorneys' fees, and other
charges due and unpaid.
Agreement. Owner and Contractor acknowledge they have read and understand each of the foregoing provisions and the terms and descriptions
contained in the Work Authorization, Estimate, and other contract documents prior to execution and agree to comply with each term and condition without
reservation. This Work Authorization constitutes the entire agreement between Owner and Contractor. No agreements, representations, or war anties
other than those specifically set forth herein shall be binding on any of the parties unless set forth in writing and signed by both parties.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.
