Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be prmaode in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

CUSTOMER AND CARRASCO'S ROOFING, HEREIN KNOWN AS "THE COMPANY" AGREE THAT ALL OBLIGATIONS UNDER THE AGREEMENT ARE
EFFECTIVE UPON APPROVAL OF THE AGREEMENT BY THE INSURANCE COMPANY. BY SIGNING THIS AGREEMENT THE CUSTOMER AUTHORIZES
THE COMPANY TO PURSUE THE CUSTOMERS BEST INTEREST FOR PROPERTY REPLACEMENT OR REPAIR AT AN AGREED UPON PRICE TO THE
CUSTOMERS INSURANCE COMPANY AND THE COMPANY WITH NO ADDITIONAL COST TO THE CUSTOMER OTHER THAN THE INSURANCE
DEDUCTIBLE, WITH THE EXCEPTION OF ROOF DECKING AS SET FORTH BELOW. CUSTOMER PLANS TO PAY FOR THE ROOF SERVICES THROUGH

WORK WILL BE PERFORMED AT INSURANCE COMPANY RATES, FIGURES & MONEY. ALL PRICES ARE SUBJECT TO CHANGE.
THE FINAL ROOF PRICE IS THE RCV AMOUNT ON THE INSURANCE PAPERWORK PLUS THE APPLICABLE CONTRACTORS OVERHEAD AND
PROFIT.
CUSTOMER AGREES TO PAY APPLICABLE INSPECTION FEE IF THE AGREEMENT IS CANCELLED AFTER THE INSURANCE COMPANY
APPROVES PAYMENT ON THE ROOF REPLACEMENT. CUSTOMER CAN CANCEL THE AGREEMENT ANYTIME PRIOR TO MIDNIGHT ON THE
THIRD BUSINESS DAY AFTER EXECUTION OF AGREEMENT AND OPTAIN FULL REFUND OF ANY DEPOSITS. CUSTOMER MAY RESCIND THIS
AGREEMENT FOR, THE PAYMENT FOR WHICH SHALL BE MADE FROM THE PROCEEDS OF THE PROPERTY INSURANCE CLAIM, WITHIN 72
HOURS AFTER RECEIVING NOTICE FROM YOUR INSURER THAT THE CLAIM IS DENIED IN WHOLE OR PART.
CARRASCO'S ROOFING SHALL HOLD IN TRUST ANY PAYMENT FROM THE CUSTOMER UNTIL CARRASCO'S ROOFING HAS DELIVERED THE
ROOFING MATERIAL TO THE JOBSITE OR HAS PERFORMED A MAJORITY OF THE ROOFING WORK ON THE PROPERTY.
YOU, THE CUSTOMER, MAY CANCEL THIS PURCHASE AT ANY TIME PRIOR TO MIDNIGHT OF THE THIRD BUSINESS DAY AFTER THE DATE
OF THIS AGREEMENT.
CARRASCO'S ROOFING, HEREIN KNOWN AS THE COMPANY DISCLAIMS ALL WARRANTIES, EXPRESSED OR IMPLIED WARRANTY OF

The Company shall have no responsibility for damages from fire, windstorm or other hazards, as is normally contemplated to be covered by homeowners insurance,
unless specific written agreement has been made prior to commencement of work.
ACCORDING TO APPLICABLE FEDERAL AND STATE CONSTRUCTION LIEN LAW THOSE WHO WORK ON YOUR PROPERTY OR PROVIDE
MATERIALS AND ARE NOT PAID IN FULL HAVE THE RIGHT TO ENFORCE THEIR CLAIM FOR PAYMENT AGAINST YOUR PROPERTY. THIS
CLAIM IS KNOWN AS A CONSTRUCTION LIEN. IF YOUR CONTRACTOR OR A SUBCONTRACTOR FAILS TO PAY SUBCONTRACTORS, OR
MATERIAL SUPPLIERS OR NEGLECTS TO MAKE OTHER LEGALLY REQUIRED PAYMENTS, THE PEOPLE WHO ARE OWED MONEY MAY LOOK TO
YOUR PROPERTY FOR PAYMENT, EVEN IF YOU HAVE PAID YOUR CONTRACTOR IN FULL, IF YOU FAIL TO PAY YOUR CONTRACTOR, YOUR
CONTRACTOR MAY ALSO HAVE A LIEN ON YOUR PROPERTY. THIS MEANS IF A LIEN IS FILED YOUR PROPERTY COULD BE SOLD AGAINST
YOUR WILL TO PAY FOR LABOR, MATERIALS, OR OTHER SERVICES THAT YOUR CONTRACTOR OR A SUBCONTRACTOR MAY HAVE
FAILED TO PAY. APPLICABLE FEDERAL AND STATE CONSTRUCTION LIEN LAW IS COMPLEX AND IT IS RECOMMENDED THAT WHENEVER
A SPECIFIC PROBLEM ARISES, YOU CONSULT AN ATTORNEY.
The Company shall not be liable for failure of performances due to labor controversies, strikes, fires, weather, and inability to obtain materials from usual sources,
or any other circumstances beyond the control of the company whether of similar or dissimilar nature.
The company is not responsible for any damage below the roof, due to ice dams, hail or preexisting construction defects durin g the period of the warranty. This
includes but is not limited to faulty AC lines, supply lines, and water lines, installed along the rafters or attached to the underside of the roof decking. These lines
are to be installed a minimum of 4 inches below roof -lines to prevent roof nails from penetrating the lines, therefore Company cannot be responsible if damage
occurs to the AC lines, or any other water lines, during the roof construction, or any other damages caused by a penetration to one of these lines.
If roofing and sheet metal work is involved, the customer understand s and agrees to that the company's our standard roof guarantee shall be acceptable and that all
terms and provisions therein shall prevail, unless otherwise specifically agreed to in writing prior to the commencement of the work.
If this agreement is breached or canceled by customer after insurance company approval and prior to materials arriving, customer will be liable for and agrees to pay
and inspection fee of $2000 paid to contractor as liquidated damages. If materials have been dropped at the site customer will be liable for and agrees to pay
$4000 for material charges.
This agreement constitutes the entire agreement between the parties. It may be changed only by written instrument by both parties.
If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the remaining provisions of this agreement shall
not be affected thereby.
Any representation, statements or other communications, not written in this agreement are agreed to be immaterial, and not relied on by either party, and do not
survive the execution of this agreement.
During the duration of the work the customer's homeowners insurance will be responsible for any interior damage as long as the Company has taken appropriate
action to protect the roof during the repair of the roof.
The company will provide the customer with a one (2) year limited warranty, which warranty shall be limited to repair or replacement of defective material or
defective workmanship, company shall not be liable for any consequential damages and any damages shall be limited to the return of the agreement price. The
agreement and warranty shall not be assigned and is non- transferable. For the warranty to be valid the agreement must be paid in full.
Lien Laws. Any person or company supplying labor or materials for this improvement to your property may file a lien against your property if that person or
company is not paid for the contributions. In the event of default by the property owner, the property owner agrees to pay all expenses incurred in collection
including reducing to judgment this obligation including a 33% attorney's fee. The proper venue for all legal proceedings arising from this contract shall be in Colorado.
The company has the right to order excess. These materials will not be charged above the agreed upon price. All excess materials belong to the company. The company
is not responsible for evaluating your home venting and circulation system status. We will install roof vents in accordance with local building codes.
Mold is common occurrence in US homes and any question about the dangers from mold can be determined by contacting the centers for Disease Control and US
Department of Health. Customer agrees that the company will not be responsible for any damage caused by mold or any other fungus or biological material,
regardless of whether it may be associated with defects in the Company's construction or services, including but not limited to property damage, personal injury, loss
of income, emotional distress, death, loss of use, loss of value, adverse health effects or any special consequential, punitive or other damages.
These conditions shall be considered a part of any agreement entered into or authorized to proceed, the same as if they were included therein.
"Default " — If customer defaults in any of the terms and conditions contained herein, the Company may terminate this agreement and payments made hereunder may be
retained by Company as liquidated damages. If customer owes payments to the company, customer agrees to pay attorney's fees, cost, disbursements, and interest (at
maximum rate allowed by law) for the cost incurred by the company to enforce its rights under this agreement to collect such payments from customer.
"Permits and licenses" -Customer hereby appoints Company as agent to procure all necessary or appropriate permits and licenses to complete the project.
"Change Orders and Allowances" — Any changes in the agreement, including all changes that affect the price, must be approved in writing by both parties,
indicating the change and any increase or decrease in the price. Unless otherwise agreed to in writing, all adjustments to the agreement price shall be payable upon
completion of the work. However, the company reverses the right to require additional monies for any increases in the agreement price prior to completion of the
work without written approval if deemed necessary to complete the work contracted.
