In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
Mid -roof inspections may be called in at the same time, 100 percent of the sheathing must
be complete and 25-75 percent of the midroof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. A ladder extending 3
feet above the roof eave and secured in place is required to be provided for all roof
inspections. Roof ventilation is required to comply with applicable codes and/or
manufacturer installation instructions, whichever is more stringent.

Sebe
Blue Mesa Environmental perfoinzed air sampling on the 2"d floor at the 7100 West 44'
Avenue Facility. Samples were collected at the end of the asbestos removal project of
drywall.
Aix Monitoring
Phase Contrast Microscopy (PCM) samples were collected. on 25 millimeter (Mm.)
mixed -cellulose ester membrane filters (0.3 micron pore size). The filters were
preassernbled by the manufacturer in conductive, three -stage cassettes with extension
cowls. Air samples were collected at flow rate of 15.0 liters per minute (TPM) for area
samples. Flow rates were recorded at the beginning and at the end of the sampling period
using a rotameter calibrated against a Bios Defender 510 (DryCal) primary flow
calibrator. All air samples were collected open-faced and positioned at breathing zone
height (approximately five feet above the floor) with the exposed portion of the cassette
facing downward.
PCM Analysis Methodology
PCM samples were analyzed according to the NIOSH 7400 Method ("A" counting rules)
for area samples. The method can be found in the NIOSH Manual of Analytical
Methods. The 1.0 fibers per 100 fields lower detection limit is retained from P&CAM
239. The overall precision is 11.5% to t3% in the 80 to 100 fiber range using the A
Counting Rules. Unused portions of samples are archived three months unless client
requests special handling.

Air Sampling Results
Results of final clearance air samples during the period of November 17, 2017 were
below the EPA monitoring guideline of 0.01 Fcc (fibers per cubic centimeter.) for PCM
clearance. All clearance air samples were collected using aggressive techniques as
described in 40 CFR Part 763 Appendix A (EPA) (1987) as per Colorado Air Quality
Control Commission Regulation Number 8.
CONCLUSIONS
Based on our observation of work performed throughout this project, and on air
monitoring results, asbestos abatement was completed in accordance with applicable
federal, state, and local regulations. At the end of the project, air monitoring data
indicated that concentrations of airborne fibers were less than the concentration
recommendedby the Environmental Protection Agency for re -occupancy following
asbestos abatement.

consideration our proposal for the above referenced tenant finishes. Our proposal is based
upon the following criteria:
1. Our meeting October 3, 2001 with you. During this meeting, we discussed the
general scope of work, and conducted a physical inspection of the work areas.
2. Drawings dated October 10, 2001, floor plan, demo, and reflected ceiling plan.
3. Our outline proposal and scope of work noted herein.
This proposal includes the following Scope of Work:
Suite #101
• Demo all walls, flooring, ceilings, paneling, doors, frames and windows.
• Build new walls: 3 5/8" steel studs at 24" on center with 5/8" gypsum board through
ceiling g,_g„ ceiling height.
4 Sound insulate all new walls.
• Smooth finish all walls. Skim over existing texture to make smooth all existing
perimeter walls.
• 2 x 4 grid ceiling with standard fissured tile. Conference room to receive "second look"
fissured tile.
• Doors to be 3' x 7' Timely browntone prefinished metal frames with solid core oak
doors, stained and finished with polished brass lever passage sets and wall bumpers.
Typical of twelve (12).
• Interior windows to be Timely browntone prefinished metal frames with tempered clear
glass. Typical of five (5).

and cover plates.
• Twenty-two (22) new duplex outlets - owner located.

• Eight (8) telephone/data conduit and box rough - owner located.
• Two (2) exit signs.
• Six (6) can lights in reception area.
• Locations and quantities of electrical items will be done as the work progresses
with owners approval. Owner must approve additional costs (if any) prior to
work being performed.
. Install new ductwork, diffusers, and grilles on existing HVAC system as required.
• Install three (3) new thermostats..
• Provide oak-raised panel lower and upper cabinets (per plan) with plastic laminate
counter top (color by Owner).
• Provide necessary plumbing for sink, dishwasher and refrigerator icemaker. Provide
sink (double or single bowl stainless steel), faucet and garbage disposal.
• Provide refrigerator (18.2 cubic feet) with icemaker and dishwasher. Both to be
installed.
• Flooring to be carpet throughout (except break room - vct with rubber base). Carpet to
be "Dimension-Honors" (or equal) with pad and 4" carpet base. Pad and carpet to be
glued togetherand glued to th6 floor ("double stick"). Colors by Owner.
• 12" carpet border in select areas per plan.
