An approved permit is effective for a period of one (1) year from date of issue and may be renewed. All fees shall be in accordance with those fees established by the Uniform Building Code or other applicable City adopted Resolutions or Ordinances. Note: All public improvements, when constructed, shall be maintained (by the respective indivdual(s) and/or company responsible for the construction of that public and/or private improvement in the respective development) on a daily basis, or as needed, such that they are free of mud and other construction debris tracking from the site. Failure to comply with this requirement will result in the enforcement of Article III, Specified Nuisances, Section IS-16 (4) of the City of Wheat Ridge Code of Laws. MINOR GRADING, EXCAVATION, AND FILL PERMITS 1. 1 -50 Cubic Yards: No permit is required for dumping or excavation of earth materials which do not exceed 50 cubic yards, provided, however, that any fill deposited is on natural terrain of less than three (3) horizontal to one (1) vertical slope, or such fill is less than three (3) feet in depth and is not intended to support permanent structures, and in addition, such fill or excavation does not obstruct or otherwise adversely affect any drainageway. Should any of the above standards be exceeded, or a drainage way be affected, a permit shall be required under the guidelines for No. 2. 2. 51 -500 Cubic Yards: Dumping or excavation of earth materials not exceeding 500 cubic yards may be allowed with a permit approved by the City Engineer. All applications are to be submitted on the appropriate completed application form and shall be accompanied by the appropriate fee and sketch plan indicating the following information before the permit will be issued. A. Location and dimensions of all property boundaries and structures on the site. B. Location and extent of areas to be filled and/or excavated. C. Location of existing and proposed drainageways, irrigation ditches, etc., and indication of how and where historic run -off will be maintained on and through the site. D. Cross- section area to be filled and/or excavated indicating original slope, new slope and depth of fill. E: Statement that indicates the proposed use or purpose for said fill or excavation. F. Relative elevation of adjacent properties. G. Erosion control plan showing placement of control devices such as hay bales, etc. 3. 501 - 10,000. Cubic Yards: Operations in which earth material fill or excavation exceeds 500 cubic yards, but does not exceed 10,000 cubic yards, may be allowed by a permit issued by the City Engineer after review of an application and supporting information. The following information shall be submitted with the required application form: A. A site plan, prepared and signed by a Colorado registered professional engineer, at a scale of no less than I inch to twenty feet (1.20) which illustrates the following: B. Location and dimensions of all property boundaries and structures on the site. C. Location and extent of areas to be filled and/or excavated. D. Location of existing waterways and drainage courses indicating any changes.. (for a site containing an established irrigation ditch, a letter of approval from the appropriate ditch company shall be required). E. Location of existing and proposed points of ingress. F. Location and extent of existing vegetation, proposed changes in such vegetation and methods of rehabilitation on site vegetation after earthwork operations are complete. G. Erosion control plan. H. Grading plan with existing (dashed lines) and proposed (solid lines) ground contours with contour intervals of 2 feet and spot elevations. I, At least two cross sections (east to west and north to south) through the site showing depth of fill and/or excavation. J. A drainage report may be required if site runoff characteristics are changed. K. Relative elevations of adjacent properties and structures. L. Statement of purpose of intended fill and/or excavation. 4. Performance Standards: The following provisions shall apply to all Minor Grading, Excavation and Fill Permits: A. Rehabilitation: Within thirty (30) days after cessation of filling and/or excavation, rehabilitation for the site shall have been completed in accordance with the approved plans. Rehabilitation shall consist of leveling, grading, landscaping or any combination thereof to minimize potential erosion and be acceptable to the City Engineer. B. Debris: Debris and/or contaminants shall not be used except with the approval of the City Engineer. "For the purpose of this Section, the term debris shall have the same meaning as the terms "garbage, trash or junk" as defined in the Wheat Ridge Code of Laws, Section 15 -4." In no case shall debris or contaminates identified or classified as hazardous waste by local, state or federal agencies be used as fill in any fill areas within the City of Wheat Ridge. C. Excavation and/or fill areas shall be graded to facilitate weed control until final grades are set and site rehabilitation and use occur (must be leveled with side slopes not to exceed 3 to 1). Petroleum contamination in the soils in the vicinity of the former waste oil tank appears to have degraded to below detectable levels and is no longer a concern. . The groundwater flow direction is to the northeast. • The extent of groundwater contamination is defined to the east by MW-5. • However, the extent of groundwater contamination is not yet defined to the north as observed by benzene concentrations over the maximum contaminant levei (MCL) in MW-6. 1_ , The repor recommenus to estabiish idiVV-o as a downgradient poini-or-compiiance (POC) well and to continue groundwater monitoring until concentrations in MW-6 are at or below the MCL. OIS agrees with this recommendation and monitoring plan with the following conditions: If after one additional year of quarterly groundwater monitoring the benzene concentration in MW-6 still exceeds the MCL, additional actions will be taken to define the extent of contamination and to bring MW-6 into compliance with the MCL. - 2. A well inventory was presented in the February 16, 1995 PetroSite report entitled " Removal of Underground Storage Tanks and Soils Testing Report". PetroSite further describes the four closest wells in the April 14, 1995 "II Secondary Site AssessmenY' report. OIS requests that the well inventory be updated as necessary and more information be obtained concerning the four closest wells with regard to their use, construction, and present status. As these wells represent the closest receptors to the site and the contaminated groundwater, we request that efforts be made to locate and sampie these welis. These four wells should then be included in quarterly groundwater monitoring, as appropriate. This conditional approval will become effective upon receipt, by OIS, of your written acceptance of all the above provisions. Note: only the owner/operator's (or a designated representative of the company, corporation, etc.) signature is valid. Consultants, etc. cannot accept this conditional approval for you. The groundwater monitoring plan will then include all of the above provisions, as well as those in your response that do not contradict the provisions herein. Armstrong with PetroSite Environmental Services. • OIS file containing the history of investigations at the site. Based on this review, we have determined a Corrective Action Plan (CAP) is required for the site. The reasons a CAP is needed include the following: 1. Groundwater contamination present in MW-1 exceeds Colorado Drinking Water Standards for benzene, toluene, ethylbenzene and xylene (BTEX). Groundwater contamination present in MW-2 exceeds standards for benzene and ethylbenzene. 2. Over the 3 year period of record of groundwater monitoring, concentrations have declined. However, the October 1998 data actualiy show increases from the April 1998 data. 3. There is a private water supply well directly downgradient of the site. This well was sampled in 1995 and showed non-detect concentrations of BTEX. It appears this well was not properiy sampled in accordance with State guidelines and the sample obtained may not be representative of the shallow groundwater aquifer. At the time of the site characterization, free product was detected in soil boring SB-1. This boring is located on the northeastern side of the gasoline tank pit. There has been no further investigation to the east to determine the extent of contamination in that direction. 5. Well screens in the existing monitoring wells at the site are piaced from 15 to 25 feet of depth. Based on the depth to water measurements from the wells, it would appear that at numerous times the water level has been above the top of the screen. Therefore, the wells may not be able to detect the presence of a floating product layer if it is present. 6. Soil exceeding State of Colorado guidance concentrations may still be present at the base of tfie waste oil pit. Also, soils placed above clean fill in the gasoline tank pit excavation may also be contaminated above guidance concentrations. 7. A groundwater flow velocity has not been presented. 8: It is unclear how utility corridors present at depths of 5 to 6 feet would intercept groundwater at depths of 13 to 17 feet. Effective February 1, 1999, the OIS adopted new Storage Tank Regulations that incorporate risk-based corrective actions (RBCA). Owners/operators can utilize the RBCA approach or follow the previous Storage Tank Regulations, effective date January 1, 1997, for assessment and remediation of releases reported before February 1, 1999. Therefore, you have a choice as to which regulations to follow at your site. OIS encourages use of the new RBCA regulations. The new Storage Tank Facility OwnedOperator Guidance Document, the Storage Tank Regulations, the Colorado Statutes, and the new report formats are located on the Internet at http://oil.cdle.state.co.us/. The Regulations (commodity # 615-82-44-0899) and the Storage Tank Facility OwnedOperator Guidance Document (commodity #615- 82-44-0626) may also be obtained at the Colorado Centrai Stores: phone (303) 321- 4164. For an electronic version (disk) of the new report formats call (303) 620-4029. The Colorado Revised Statutes can be found at your local library. The requested CAP can be prepared using either the 1997 regulations, or the new RBCA regulations. The advantage to using the RBCA regulations is that the "risk" presented by the contamination at the specific site can be evaluated. It is possible the cleanup leveis determined based on risk may be less conservative than those required by the 1997 regulations. However, if you choose to follow the RBCA regulations, completion of the Site Characterization RepoR (SCR) is recommended. Much of the data needed to complete the SCR is already contained in the Second Level Site Assessment (April 14, 1995) and Tank Removal Report (February 16, 1995). However, the full extent of contamination in soil and groundwater will need to be determined to complete the SCR and to determine a site classification. 7300 West 44th Ave., Wheat Ridge, CO October 22, 1999 Page 3 - In response to the comments offered by Mr. Armstrong in his letter of October 21, 1998 in response to OIS comments of September 28, 1998 we offer the following: Natural attenuation is a remediation option offered by the regulations, if it can be demonstrated with actuai site specific data that naturai attenuation will be effective. 2. Procedures for well sampling are described in the Guidance Document. Sampling of the downgradient water supply weil on neighboring property is essentiai. If at all possible, this well must be also be purged of three well volumes prior to sampling. Existing pumping equipment in the weli can be used to purge the well, if available. The sample can be collected directiy from a faucet nearest to the weilhead, preferably prior to any water softeners or other treatment systems. 3. The full horizontal and vertical extent of both soil and groundwater contamination is required to be determined to evaluate risk and remedial options, including natural attenuation. OIS requests notification within 15 days of your decision as to how to proceed with the investigation of this site. The CAP (1997 regulations) or the SCR and CAP (RBCA regulations) is requested to be returned within 60 days of the date of this letter. IvSW 2 is on the north property line and is the most down-gradient point of the property. Silco requests that MW 2 be established as a down- gradient point of compliance versus installing additional monitoring wells in West 44th Avenue, and limitation posed by utilities found in W. 44th Avenue. For your use, we enclose a copy.of Exhibit D, Summary of Groundwater Tzsting Results. Utility comdors were fiilly delineated in the Secondazy Site Assessment Report, Exkubit A. and summarized on page 5. 4. The geology ofthe site was charactenzed on page 3 ofthe Secondary Site Assessment Report. Thz lucal geology is al,o described in detail on six rinll'ng logs enclosed with this report. 5. The proxmity, quality, depth, cutrent and future uses of nearby surface and ground water was discussed on page 6 of the report. The Tank Closure Assessment Report dated Februazy 16, 1995, provided a list of all peimitted wells in the quarter section and .S mile radius, sown on E-Itibit E. On February 9, 2000, the OIS plan was accepted in writing. Under the plan approved by OIS: a.) Quarterly monitoring of all monitoring welis is to be performed. b.) Four private supply wells are to be included in the sampling events. 3.0 CONDUCT OF WORK On February 28, 2000 a report was provided to OIS regarding research activrities to locate the ciosest private supply wells in the area. Wells were identified by referencing State Engineers records for pertnitted supply wells in the area, and by neighborhood reconnaissance. The closest private supply wells were identified: NORTH Fatzinger weli (FW) at 7301 W. 44th Avenue, approbmately 100 feet north of site. NORTH Laconte well (LW) ffi 4470 Upham Street, approbmately 1500 north of the site. EAST Rusch well (RV1) at 7280 W. 44th Avenue, approbmatelyl 90 feet east of the site. NORTHEAST Thelander well (1W) at 4490 Telier Street, approbmately 2000 feet northeast of the site. Due to the distances involved to the Laconte well (LV) and the Thelander well (TV) we requested that these wells be eliminated from quarterly testing. On March 2, 2000, OIS contacted PeVoSite, Inc., and advised that the four closest private supply wells were to be sampled, regardless of the distance from the site. We advised the Gient of the communication. Access letters were mailed to property owners, not previously contacted.. Foilow up contact was made by telephone and neighborhood visit. On March 2, 2000 pertnission was secured to sample the Thelander supply well. We also determined that the Laconte well had been plugged. We have not identified any other private supply welis, downgradient. On March 3, 2000, all wells were sampled. Water samples were collected in accordance with standard monitoring or supply well testing procedures. All water samples were placed in clean glass containers prepared for the work by Environmental Chemistry Services, Inc. Samples were sealed and tabeled on site, and stored on ice until taken to our offices where they were placed in a refrigerator for the weekend. On March 6, 2000, 9 sets of water sampies were delivered to Environmental Chemistry Services, Inc. A Chain of Custody Record was prepared and signed. 4.0 FINDINGS On March 9; 2000, testing results were received from ECS. Based on these resuits we submit the foliowing findings: ➢ Hydrocarbon levels in groundwater continue to show natural attenuation: a.) MW-1, point of release shows a 66% improvement in Benzene levels. b.) NNV-2, immediately downgradient of the former tanks, shows a 99% improvement. c.) MW-6, downgradient, point of compliance well has trace levels 95% less than NNV-2, and is located within 72 feet directiy downgradient of MW-2. d.) A private supply well (Falzinger well) directly downgradient of MW-6, and within a distance of 75 feet has tested Non-Detect on two occasions. e.) Another private supply well (Rusch well) is located within 100 feet of the former tanks and is directiy cross-gradient. The Rusch well has tested Non-Detect on two occasions. ➢ MW-6, downgradient, point of compliance wells shows trace levels exceeding the State groundwater standard (0.1 parts per miilion-Benzene, as compared to 0.05 parts per million- Benzene, State groundwater standard.) 5.0 CONCLUSIONS ➢ All of the evidence indicates that groundwater contamination is not wide spread. This is consistent with hard packed clay soils present at the site. ➢ Hydrocarbons in groundwater do not present a threat to public health of the environment. ➢ We have advised the Fatzinger property owner of the ebstence of low levels of hydrocarbons in NNV-6. The property owner has assured us that the supply well is used only for yard irrigation. On November 10, 1999, a meeting was attended at OIS, Dale HutchingsSilco Oii Company, James Artnstrong-PetroSite, inc., Dick Piper, and Jane Bral, OIS. On November 15, 1999, a proposal was submitted to Silco Oii Company for additional investigations to be conducted at the site per the meeting with OIS. 3.0 DESCRIPTION OF WORK Per agreement with OIS, additional investigations were to be conducted before submittal of a CAP as requested by OIS. The additionai investigation work was to consist of: ➢ Instail two additional down-gradient, off site monitoring weils, within 50 feet of MW-2. ➢ Colled one soil sample from fortner waste oii tank at a depth of 9 feet. Analyze sample for TRPH; if levels exceed 500 parts per miilion, analyze sample for PAH compounds. ➢ Colled and analyze one soil sample from MW-5, and 6, from highest PID reading or at the water tabie if field-tesGng not definitive. ➢ Re-survey well head elevations for ail monftoring wells. ➢ Sample all monitoring weils. ➢ Submit a written report of the results of all testing. 4.0 CONDUCT OF WORK After meeting with OIS, work was compieted to secure off site access and to locate underground utilities from the City of Wheat Ridge, and from two neighboring properties. As part of the access agreement from the neighboring properry east, a private suppiy well was to be sampled. Due to underground utilities, MW-5 and MW-6 were located within 60 feet of MW-2. On December 27, 1999, two groundwater monitoring wells were installed. MW-5 was located approximately 50 feet east of MW-2. MW-6 was located approximately 55 feet north of MW-2. Both MW-5 and MW-6 are down-gradient of known contamination areas. Monitoring weils were instalied in accordance with standard monitoring weli instailation practices. During the course of the work, soii samples were screened at five (5) foot intervals. One soii sample was collected, in each boring, beriveen 1416 feet via a drive sample. Per instructions from OIS one soil sample was collected from the former waste oii tank area at a depth of 9 feet. The sample was collected by drive sample and identified for testing for BTEX, and TRPH. Instructions provided to the lab were to also test this sample for PAH compounds if TRPH levels exceeded 500 ppm. On December 28, 1999, MW-5 and MW-6 were developed. The wells were purged and sampled in accordance with standard weil sampiing procedures. An oii-water interface probe was used to measure free product in all wells. No free product was found in any well. A new survey of well head elevations was completed this date by Herb Linn, licensed surveyor. Three soil samples and six water samples (3-voa's each) were delivered to Environmental Chemistry Services and a Chain of Custody record was prepared and signed at 4:40 pm. 5.0 FINDINGS On January 5, 2000, testing results were received from ECS. Based on these results we submit the following findings: > TRPH levels reported in S-1 were below the threshold level and PAH analysis was not run. ➢ No free product was found in any monitoring well. ➢ The soii sample coilected from MW-6 indicated staining and low Ieveis of NPH/BTEX at a depth of 16 feet. This visual evidence indicates that hydrocarbons migrated to the noRh, in soil at an approximate depth of 16 feet, just above the water table. Soiis analysis indicates hydrocarbons below PQL's for soiis in both samples. i No Detectable levels of hydrocaebons are reported in MW-3, MW-d, or MW-5. ➢ Elevated levels of hydrocarbons are reported in on-site NPN-1 and MW-2. Benzene levels in MW-1 show a 25% improvement from October, 1998. Benzene levels in MW-2 show a 8% reduction from December, 1997. ➢ MW-5 has no detectable hydrocarbons. MW-5 is a down-gradient point of compiiance weil to the east. ➢ MW-6 reports 44 parts per biliion Benzene which exceeds the State GWS of 5 ppb, however ali other constituents are below SGWS. The pattern of degradation (or lack of migration) is evidenced by comparing levels of Benzene in MW-2 and MW-6. Benzene levels in MW-6 are 50 times lower than in MW-2. MW-6 is located 72 feet directly North of MW-2. We believe this is conclusive evidence supporting lack or any significant migration of hydrocarbons off site.. ➢ MW-3 and MW-4 are up-gradient point of compliance weils. ➢ A revised groundwater contour map confirtns prior investigations and a northeast direction of underground water flow. 6.0 CONCLUSIONS ➢ Releases at this site occurred over 15 years ago. Hard packed clay soils have trapped hydrocarbons in the soil and shaliow groundwater on site. > Minimal off site migration has been found. A pdvate supply well located an additional 75 feet North of MW-6 has previousiy been tested ND. ➢ The shallow acquifer water is not used for public consumption. ➢ Existing levels of hydrocarbons are not potentially hazardous to human health or the environment. Hydrocarbon levels are gradually being diminished by naturai forces. 7.0 RECOMMENDATIONS 2 We recommend that MW-6 be estabiished as a down-gradient point of compliance well. Direction of water flow has been established as Northeast at a gradient of 0.0097 ft/ft. 3.0 SCOPE OF CURRENT WORK In accordance with an approved groundwater monitoring well testing plan, four Groundwater Monitoring wells are to be tested quarterly and reports filed with OIS. This work is done to comply with the direction of Oii Inspection Section. 4.0 DESCRIPTION OF WORK On October 6, 1998, four Moniforing Wells were sampied. PetroSite arrived the site at 11 am. The property is now open for business as an Auto Repair Garage. z Ali MW manhole covers were un-bolted and the weli caps removed. Depth to groundwater was measured using an electronic water level indicator and the results recorded. Each well wes bailed by removing one well volume and allowed to re-charge. After the wells had recovered, a sample was collected using disposable plastic bailers. As each water sample was obtained it was placed in clean. VOA's provided by Environmental Chemistry Services, sealed, identified, and stored on ice pending delivery to the testing laboratory. Dupiicate water samples were obtained from each well, the caps were secured and the manhole covers bolted. The manhole bolts were lubricated to prevent rustings On October 6, 1998, four water samples were delivered to Environmental Chemistry Services, inc. A Chain of Custody Record was prepared and signed. Testing was specified for Total Volatile Petroleum Hydrocarbons by EPA Method 5030, and for BTEX by EPA Method 8020. Sample containers were prepared by ECS, Inc., with 5% HCL, 4 degrees Centigrade. 5.0 RESULTS OF LABORATORY TESTING On October 14, 1998, the testing results were received from ECS, Inc. Copies of the lab reports are enclosed. 6.0 FINDINGS The levels of hydrocarbons found in the water samples obtained for this quarterly report, together with results of previous sampling, are summarized on Exhibit A. Testing results indicate that groundwater quality is gradually being restored by natural forces. Additionally the results of testing for a period of 36 months indicates that: . Up-gradient Monitoring Wells No. 4 has not been impacted. Up-gradient MW-3 shows trace levels below the State GWS. Groundwater contamination, at levels above the State GWS, appears confined to the former tank and island areas. . Hydrocarbon leveis are gradually being diminished by natural forces. MW-1 shows a 23% improvement since April, and MW-2 shows a 62% improvement since April. Benzene is the only constituent that exceeds the State groundwater standard. . Based on prior testing of a private supply well at 7301 W. 40 Avenue, no off-site migration has occurred. . There have been no complaints of groundwater contamination from neighboring properties and there are several private supply wells found down-gradient of the site. • The Wheat Ridge Fire Department recently investigated the presence of free product in the sewer lines running along West 44t" Avenue. Their investigation indicated that subject site was not the source of the free product. . No motor fuels or oils have been stored or sold at the site since 1981. The present use does not include storage or sale of motor fuels or oils. The direction and gradient of underground water flow remains Northeasterly. A revised Groundwater contour map, based on current elevations of water is enclosed as Exhibit B. 7.0 RECOMMENDATIONS Quarterly sampling should be continued until such time as the schedule is revised by State Oil Inspection. MW-3 has tested No Detectable leveis since April, 1995. MW-4 has reported trace levels below the State ground water standards for BTEX. Direction of water flaw has been established as Northeast at a gradient of 0.0097 ft/ft. 3.0 SCOPE OF CURRENT WORK In accordance with an approved groundwater monitoring well testing plan, four Groundwater Monitoring wells are to be tested quarterly and reports filed with OIS. This woric is done to comply with the direction of Oii inspection Section. 4.0 OESCRIPTION OF WORK On June 30, 1998, four Monitoring Welis were sampled. PetroSite arrived the site at 10:30 am. The ptoperly is now open for business as an Auto Repair Garaga. ' z All MW manhole covers were un-bolted and the well caps removed. Depth to groundwater was measured using an electronic water level indicator and the results recorded. Each well was bailed by removing one well volume and allowed to re-charge. After the wells had recovered, a sample was collected using disposable plastic bailers. As each water sample was obtained it was piaced in clean VOA's provided by Environmental Chemistry Services, sealed, identified, and stored on ice pending delivery to the testing laboratory. Duplicate water samples were obtained from each well, the caps were secured and the manhole covers bolted. The manhole bolts were lubricated to prevent rusting. On June 30, 1998, four water samples were delivered to Environmental Chemistry Services, Inc. A Chain of Custody Record was prepared and signed. Testing was specified for Total Volatile Petroleum Hydrocarbons by EPA Method 5030, and for BTEX by EPA Method 8020. Sample containers were prepared by ECS, Inc., with 5% HCL, 4 degrees Centigrade. 5.0 RESULTS OF LABORATORY TESTING On July 6, 1998, the testing results were received from ECS, Inc. Copies of the lab reports are enclosed. 6.0 FINDINGS The levels of hydrocarbons found in the water samples obtained for this quarterly report, together with results of previous sampling, are summarized on Exhibit A. Testing results indicate that groundwater quality is gradually being restored by natural forces. Additionally the results of testing for a period of 36 months indicates that: . Up-gradient Monitoring Welis No. 4 has not been impacted. Up-gradient MW-3 shows trace levels below the State GWS. Groundwater contamination, at levels above the State • GWS, appears confined to the former tank and island areas. . Hydrocarbon levels are gradually being diminished by natural forces. MW-1 shows a 23% improvement since April, and MW-2 shows a 62% improvement since Aprii. Benzene is the only constituent that exceeds the State groundwater standard. . Based on prior testing of a private supply well at 7301 W. 44`h Avenue, no off-site migration has occurred. . There have been no complaints of groundwater contamination from neighboring properties and there are several private supply wells found doyyn-gradient of the site. . The Wheat Ridge Fire Department receqtly investiqated the presence of free product in the sewer lines running along West 44t' AYenue. Thp-ir investigation indicated that subject site was not the source of the free product. . No motor fuels or oils have 4een stored or sold at the site since 1981. The present use does not include storage or saie of motor fuels or qils. A down-gradient private supply well at 7301 W. 41th Acenue was also tested on ]uly 19; 1995. Peimission to test this well was obtainad from the property oR•ner, Mr. Bob Famnger. 4.0 DESCRIPTION OF VVORK PetroSitz aaived the site at 9 am. The pricata supply well was tested 5rsf. A teflon bailer was loa•erzd into the well and allowed to fill. Depth to water was measured from ground surface. After the watzr sample was collected it was placed in clean sample container, sealed and mazkzd for idzntification, and placzd on ice for storaga. After sampling the privatz supply well, the bionitoring wells at the Silco site were tested. All well manhole covers wzre un-bolted and the well caps rzmoved. Depth to watzr was meuurzd and recordzd. Three bailer volumes wzrz removed from each well. ARzr recovery all watzr depths were re-measured. At this time watzr samples were collected from MW 1- htW 4. Aftet each a^ater sample was collected it was placed in clean sample containers, sealed and mazked for identificafion, and placed on ice for storage. In March, 1995, Silco Oil Company authorized PetroSite to conduct addi!ional investigations at 2.0 SCOPE OF WORK The purpose of this work is to comply with the directive of CDPHE, define the extent of soil contamination around the gasoline and waste oii tanks, and associated piping, determine depth to groundwater, direction and velecity of urderground water flow, and to test subsurface groundwater for the presence of petroleum hydrocarbons. Additional objectives of the work are to estabiish a Remedial Action Category and to submit findings and recommendations for future corrective action. The Niork plan consists of core sampling of soils around the gasoline and waste oil tanks and associated piping, installation of four Groundwater Monitoring Wells, testing of shailow aquifer water for the presence of petroleum hydrocarbons, determination of depth to groundwater at the site, caiculation of the direction of underground water iloNi and estimat°d veiocity, investigations as to utility corridors whieh may ad as receptors for migration, assessment of potential impacts to buildings, surface water, or xriater supply wells, and reporting of results to COPHE. The conduct of this assessment is consistent with Federal and State requirements for anvironmental testing and monitoring associated with patroleum hydrocarbons released from underground storage tanks containing gasoline and waste oil products. The utilized methodology was the most applicable for the actual conditions at the site. 3.0 RELEVANT SITE HISTORY In February, 1995, the Site was vacant. The site had recently been operated as a repair garage, however the storage and sale of gasolines and Niast= oii were discontinued in 1987. Relevant past history Nias reviewed in the February, 1995, raport. Silco Oil Company is the owner of the property and of the UST system. 4. Gasoline s±oraye tanks were formerly located on the east side of the building along Upham Street. Soils testing conducted at the time of tank removal indicated that contaminated soils were Non- datectable on the east, north, and south walls of the gasoline tank pit, low levels of 140 ppm TVPH were found on the west wall, and that the majoriy of the contamination in this area was confined to the bottom of the tank pit, in the north end at depths below 15 feet. The extent of contamination found in the waste oil tank ezcavation was not defined below 9 feet, due to the proximity of the building foundation. Stained soils were observed beneath the building foundation. The Site Plan and Utility Corridors are shown on Exhibit A. A Summary of the level of soil contamination found at the time of Tank Removal is showm on Exhlhit B. 5.0 PRELIN1INARY WORK Prior to additional investigation, preliminary work was completed: a). All underground utilities were re-located prior to drilling. b). A Utility Corridor study was Completed. All underground utilitiy lines, including size of lines and depth of burial were obtained from the various utility companies and suppliers. Details are showm on Exhibit A. . c). Coordination of the work with Silco Oil Company was done. The driiling work wias scheduled for April 4, 1995. d). CDPHE was advised of the work schedule. e}. Arrangements were made with sub-contractors DRILLING Corrocon, Inc., provided a Canaterra CT 150 portable drill rig to advance 4 inch hollow stem augers. Soil samples were collected from bit samples. All drill bits and drill pipe were de- contaminated between collection of soil samples. After completing soils core sampling, four Groundwater Monitoring Wells (GWMW's) were installed. Each well was constructed of two inch diameter, 0.010 slot, schedule 40 PVC screen and casing. Eacfi well was set at least five feet below the water table. A sandpack of 10-20 grade silica sand was placed in the annular space of each well, one (oot above the screened interval. A six inch bentonite seal was placed above the sandpack and the remaining annulus filled with,bentonite slurry. Each soil sample was physically classified on a boring Jog Two soil samples from each bor=hole (S81 thru S86) Nihich exhibited the highest PID reading, or the sample obtained from thz Icwest deoth available, were submitted to the laboratory for TVH and BTEX analysis. W= noted that field testing rzsults were not 100% reliable due to the age of suspected releases. Soil samples obtained from SB7 and SB8 (off-set bore holes driiled in around the evaste oil tank) were analyzed for Total Recoverable Petroleum Hydrocarbons and BTEX. FolloNiing ground,oiatedwell stabilization, fluid levels were measured in all wells and a check for the pr=sence of free product was conducted. Fcllowing ftuid level measurements, all monitoring wells will be sampled fer groundwater c,uality. The wells were evacuated until three well volumes were removed or until the wells were dry. GroundNiater sampies were collected from NIW 1- MW 4 and analyzed for TVPH and BTEX. Soil samples were collectzd in advance of the hollow stem augers by collecting bit samples from the drill bit. The soil sampies were logged in the field using the Unified Soil Classification Sys;em, readings from the PID in ppm, lithology, grain-size, color, moisture, and odor. AII drill cuttings were stored in DOT approved 55 galion steel drums and disposal made by Corrocon, Inc. SURVEY: Mr. Herb Linn, Licensed Surveyor was engaged to determine vyell head elevation for each monitoring vriell. Oetermining the elevation from a temporery benchmark (established at 100.0 feet at east gasoline island) for each well head elevation was used in connection with fluid levels collected to croafe a potentiometric surface map and establish a direction of underground water flow. This data together with awe!I slug test performed by LeakSearch of Colorado, calculates hydraulic conductivity, veolicty, and direction of underground water movement. All details are shown on Exhibit E. 6.0 DESCRIPTION OF WORK On April 4, 1995, work ccmmenced Corrocon, Inc., advanced eight soils borings identified as SB-1 thru SB-8. Two soil samples were collected from SB-1 thru SB-6, and one soil sample was collected from SB-7 and SB-8. Bore hole logs were prepared for each Soii Boring. After collecting soil samples from each bore hole, they Nrere tested in the field. The sample shcwing the highest PID reading was then placed in a clean soil sample jar, sealed and identified on the job, logged to the Site Plan for Soils Testing, and placed in a refrigerated cooler in preparation for deliyery to the testing laboratory. SB-2, SB-3, SB-5, and SB-6 were completed as GroundVVatzr Nlonitoring VVzlis and idzntified as MW-1, MW-2, MW3, and MVV-4. SB 1, SB 4, SB 7, and SB 8 were plugged after soil samples ware collected using bentonite hole piug. As each well was complet2d th2 Ievei of water was measured, and the well bailed and aliowed to re-charge. Details of well depth, bailiny, and re-charga time aie snown on the well logs enclosed with this repoR. All wells indicated full re-charge withfn 1 hour. After each wiell had recharged, a water sample was collected via use of a teflon bailer, sealed and idzntrtied on the job site, placed in clean water containers provided by Environmental Chemistry Servfces, and stored on ice pending delivery to the testing laboratory. The teflon bailer was de-contaminated behvzen collection oF water samples by use of an alanox rinse. Datails of the locations of each Monitoring Well are shown on Exhibit D, Summary of Water Testing Results. Work commenced at 730 am and was completed by 3:30 pm. Details of all drilling activities, ara noted on the enclosed Drilling Logs, Exhibit G. At 4:20 pm, 14 soil samples and 4 water samples were delivered to Environmental Cheinistry Senrices, Inc. A Chain of Custody Record was prepared and signed by both parties. On Apnl 7, 1995, a slug test was performed by LeakSearch of Colorado. Details are shown on Exhibit G. LeakSearch also maasured the depth to groundwater in MW 2 and MW 4 this date. A Rising head slug test using a bailer and electronic water level indicator was nerformed on MVV 4. VVatar level measurements were taken and the time recorded. With these results, a computer program was used to calculate K from the collected field data. A value for hydraulic conductivity of K= 0.000074 fflmin (10 ftJMin) was calculated. The lithologic and hydrologic characteristics (permeability and conductivity) of the saturated interval are described on the Drill Logs and calculated on the Slug Test results, shown on Exhibit F. The rate of water flow (discharge and re-charge rate) is calculated at 19.66 feeUday. On April 11, 1995, all Monitoring Wells were re-measured at 9 am and 5 pm, using an elzctronic water level indicator. The results were recorded. The measurement obtained at 5 pm was used to calculate the elevation of groundwater, and the direction of underground water flow. All details are shown on Exhibit E. 7.0 RESULTS OF LABORATORY TESTING On April 12, 1995, we reviewed the results of testing from Environmental Chemistry Services. All soils testing results are summarized on Exhibit C. All groundwater testing results are summarized on Exhibit D. Based on the results of soil and groundwater samples submitted for laboratory anlaysis we submit the foliowing findings: 8.0 FINDINGS Tha areal extent of soil contamination had been defined within guidelines establisned by CDPHE. Soil contamination is confined to the site. MW 3 and MW 4 constitute up-gradient point of compliance monitoring wells. Water contamination is Non-Detect in both MW 3 and MW 4. Hydrocarbon soil migration was vzrtical in the sandy soils to a depth of 10-12 teeL Clay soils found below 10-12 feet have higher resistance and the majority of petroleurn contaminated soils appear to be trapped in the clay soils below the water level. Potential for migration off site is r2duced, in our opinion. due [o utilfty corridors found in bo[h streets; specificaily a 8 inch sewer line, buried 5-6 feet beloNi ground surface, in the center of VVest 44th Avenue. . Hlso, wa believe, there is limited potertial for migration to privata supply vretls found in the area. This is based upon: a). Utility corridors found in both adjacent (and down-gradient) streats would act as receptors. During the course of investigating utility information no utility company or represantative reported any hydrocarbons tound along utility corridors. b). The closest private supply well east (permit #14691, Sam Laconte, 4470 Upham Street, total depth of 60 feet, water reported at 25 feet) is not in use during the winter months and is used only for irrigation purposes. This private supply well is approximately 165 feet east of the 5ilco site. c). Th2 closest privat2 supply wells noRh is found at 7301 West 44th Avenue. This is a residential property owned by Mr. 6ob Fatzinger. 6ased on well permits obtained from tha Sta[e Engineer's office, this well is not permitted. We visited with Mr. Fatzinyer's grandson on April 11, 1995 and observed the well to be in use for irrigation purposes. Water depth appeared to be approximately 16 feet this date. This well is approximately 125 feet north of the Silco site. d). Other private supply wells in the area, not within 500 feet of the Silco site, based on vvell permit data ara: (i). Permit #10187, Emil Thelander, 4490 Teller Street, total depth of 35 feet, water reported at 8 feet, permit dated 11-3-61. ' (ii). Permit #22319, W. E. Carver, 4400 Saulsbury, Total dep'th 50 fe2t, Niatar reporied at 17 feet, permit dated 11-24-64. To our knowledge, none of the private wells, in the area, are used for drinking water purposes. We confirmed that the residential properties dosest to tha Silco site have Cfty water Details of private supply and monitoring wells in the immediate area are shown on Echibit H. To our knowledge ali of the residentiai properties In the area are connected to City of Wheat Ridge municipal water supply. The majority of the private supply wells found in the area are old permits used primarily for irrigafion purposas. CDPHE has outlined suggested ground water cleanup leveis in the State of Colorado in their December 11, 1992 investigatory guidelines and UST Owner/Operator Guidance Documents dated April 15, 1994. The cleanup levels are dependent on the Remedial Action Category (RAC) assiyned by CDPHE. Three Remedial Action Categories are currently specified by CDPHE. These are: RAC 6: Petroleum contamination of groundwater currently being used as a public or private drinking water supply or having the potential to be used as a public or privatge drinking water supply or having the potential to be used as such requires remediation in cornpliance with the current ground water standards of the Colorado Water Quality Control Divison and the Federal safe Drinkiny Water Acts. RAC II: Petroleum contamination of ground water currently being used as a privatge drinking water supply or has the potential for being used as a public drinking water supply not included in RAC I requires remediation in compliance with Water Quality Control and Safe Drinking Water Acts at the location of existing water supply wells. RAC III: Ground water not being used and with little or no potential tor being used as a public or private drinking water supply shall be remediated to appropriate levels as determined by CDPHE on a case by case basis. The levels of petroleum hydrocarbons found in the subsurface soils and groundwatar are consistent with an old release. Service station operations were disconiinued in 1987. Due to the tightly packed clay soils below the depth of 10-12 feet, and the fact that utflity corridors In both streets seperate private supply wells from releases at the site, there is limited potential for migration off site, or iinF;acts to public health or the environment. The low level of BTEX repoRed in Soil Sample SB-7, at a depth of 15 feet (.0131 ppm) is below CDH guideline levels for additional investigatfon, whereas the TRPH level reported from this same sample is 290 ppm. Considering the fact that TRPH levels were 6900 ppm at 9 feet, beneath tlla waste oil tank, we believe these results are sufficient to estabiish the.vertical and hortizontal extent of soil contamination in the vicinity of the waste oil tank, and further supports our conclusion that migration was confined to the tank area of the area. 9.0 RECOMMENDATIONS We recommend that two copies of this report be filed with the Colorado Department of Publfc Health and Environment, Attn: Cynthia Silva-Burbach. Subject to final review by CDPHE we recommend testing of the private supply well at 7301 West 44th Avenue. Upon d2termination of size and contents, a Site Plan is to b2 prepared, notice provided to the State Inspector of Oils, and a Fire Permit for removal obtained. At removal, PetroSite will inspect the tank area for the presence of contamination, conduct fielcl screening of soils from the tank area, coliect representative soil samples from the tank area, submit these samples to a Stata approved lab for testing for petroleum hydrocerbons associated with gasoline storaga and waste oil tanks, and provide the client with a written report of the results. Documentation as to destruction of the tanks, and disposition of any tank liquids or sludye, is to be provided. The scope of the written environmental report is specified in 6 CCR 1007-5, Subpart G., 280.72, "Ass2ssment of the Site at Closure". Reinoval and disposal of the tanks constitutes closure as specified in 6 CCR 1007-5. Our client is Silco Oil Company, Attn: Mr. Dale Hutchings, 181 East 56th Avenue, P.O. 6ox 16528, Denver, CO 80216, Phone (303)292-0500. Silco Oil Company owns the property. Gasoline storage and sale was discontinued in 1987. Removal of the tanks and assessment of the site was authorized to decommission the service station operation. Prior to the removal of the tanka certain preliminary work was completed: 1} On 1-18-95 we notified State Oil Inspection of the pending removal. Encloszd with our nocice was a Site Plan. This plan is enclosed as Exhibit A. 2.) On 1-27-95 we visited the site, met with utility companies for underground utility locat2s, and determined the contents of the tanks. 3.} On 2-6-95, we obtained a permit from the Wheat Ridge Fire DepaRment. Inspection, testing, and removal was scheduled for 2-8-95. 4.) All underground utilities were located by Utility Locate Service Company of Colorado, prior to excavation. Job tickat #258 652 was assigned: A water line runs along the south end of the gasoline tanks. We arranged to have the water line shut off pending removal of the tanks. All utilities are shown on the site plan. 5.) We researched Colorado Department of Health records as to "reported incidents of Leaking Underground Storage Tanks" for this area of Denver. We also located all UST properties in a 1/2 mile radius of the site. Details of UST properties, and LUST properties are shown on the enclosed UST Map, Exhibit B. 6.) We prepared a Land Use Map which identifies the property. Land use in the entire area is mixed use. Details are shown on the enclosed Land Use Nlap, Exhibit C. 7.) We locatzd the site on a 7112 minute topographical map which is enclosed as Ezhibit D- The ciosest surface water is Clear Creek found .6 miles north of the property. Average annual precipitation in the Denver metro area is 15.5 inches per year. 8.) We searched the State Engineer's records for permitted wells in Section 23 and 24, T3S, R 69W, and obtained well permit information for the area. From these records wie praparad a Map of permitted walls, which is enclosad with this report as Exhibit E. PetroSite Environmental > Services ARliale (I( PArwlite. IM. 9.) We prepared a Site Plan for Soils Testing which summarizes all field and lab test results, designated Exhibit F, and it is enclosed with this report. GEOI.OGY: Denver, Colorado lies near the western edge of one of the largest structural basins in the Rocky Mountain region, the Denver Basin. This basin was formed during the late Crataceous and early Tertiary time and is a south trending asymmetrical basin under a gently dirping east flank. The upper Arapaho Formation consists of discontinuous beds of light grey to yellow-brown sandstone and claystone of terrestrial origin, highly variable in texture and composition, consisting of light grey to brown tuffaceous silty claystona, tuffaceous arkosa and andesitic conglomnerates. Near surface soil conditions ar2 described in the Soil Survey of Araphaoe County, Colorado, 1981. Surface and near surface soils at the site are dassified as mixed Fondis-Ascalon s2ries which are of the Holocene and quaternary Piney Creek alluviums. Fondis soils consist of brown to gray brown, silty clay loam soils which are calcareous below the leached zone: Ascalon series soils are brown to tan, sandy loams which become more calcareous with depth of burial. Soils found at the site, consisted of inedium density clay, medium to light brown in color. Soils harden with depth; in the waste oil tank area, soils become hard packed at 9 feet; in the gasoline tank pit, soils become hard packed at 10 feet, and the clayey soils are mixed with claystone below this depth. Total excavation depth was 9 feet in the waste oil tank area, and 17 teet in northwest corner of the gas tank pit. Water was not found. Bedrock was not encountered. HYDROLOGY: The bedrock aquifer generally contains good chemical quality water which is not used for domestic supply Local water supply is provided by the City of Whea[ Ridge. The aquifer at the site is a mixture of clayey silts with sand and gravel lenses located below 15-20 feet. Shallow groundwater is commonly found at depths below 20 feet. Direction of groundwater flow is north by north-east towards Clear Creek. Permitted well data obtained from the State Engineer's office indicate an abundance of private supply wells with depths to groundwater reported at 9-10 feet in close proximity to the site. The City of Wheat Ridge advised that these wells are used for irrigation purposes only. No private supply wells are to be used for drinking water purposes. Most of the permits examined are old (1963 vintage). We do not know if all of these wells are still in use. USTS: The tanks consited of: 2-6,000 Gallon Steel Gasoline Tanks. 6oth manufactured in 1963, UL#729758 and UL#729842. 1-500 Gallon Steel Waste Oil Tank, No date of manufacture and no UL#. The two 6,000 gallon tanks formerly contained gasoline. The 500 gallon tank contained water and small quantities of oil and sludge. Product piping was galvanized steel for the waste oil tank and galvanized steel and fiberglass piping for the two gasoline tanks. Both gasoline tanks were equipped with 4 inch filler manholes with inner-fill tubes, Phase I Vapor Recovery fittings, and 1/3 hp Red Jacket Submersible pumps with line leak detectors. February 7, 1995: The tanks were prepared for removal by excavation of the overburden, consisting of landscape rock over the waste oil tank, and concrete over the rivo gasoline tanks. All piping, and elactrical was removed. The waste oil tank was full of water (450 gallons of water and 50 gallons of sludge/mud). The west 6,000 gallon tank contained 230 gallons} of gasoline, and the east 6,000 gallon tank contained 70 gallons of yasoline. Arranyements were made to have the tanks pumped out by Tri State Oil Recyclers the following day and th2 job was barricaded for the night at 3 pm p2nding pumpout. Barricade fencing providzd by All State Fence Company was erected around both excavations. February 8, 1995: Ail fittings, pipe, and 2lectrical was removzd and the tanks tiltad towards the fill pipes. The tanks were pumped out by Tri State Gil Recyclers by 10 am. The tops, sides, and ends of all tanks were exposed by excavation: The tanks were cleaned and UL numbers recorded for the Wheat Ridge Fire Department. At 11 am the tanks were inerted via introduction of dry ice pellets at the ratio of 15 pound pzr 1,000 gailons of tank capacity. At 12 pm the tanks were tested by Wheat Ridge Fire and removal approved. Subsequently, these two tanks were removed, cleaned and inspected, and loaded on a transport trailer for destruction by Simpson Excavating. Aft2r these tanks were removed we tested several soil samples obtained from beneath these tanks for the presence ot petroleum hydrocarbons using an MSA Hydrocarbon detection kit. Elevated levels of petroleum hydrocarbons were found in the north end of the gas tank pit and beneath the waste oil tank. The west 6,000 gallon tank was re-scheduled for removal the following day and the job site secured at 4 pm. February 9, 1995: Prior to going to the job site, we discussed our preliminary findings with Dale Hutchings, Silco Oil Company. We arrived the job site at 9 am. The west 6,000 gallon tank had been ineRed a second time via introduction of 200 pounds of dry ice at 8 am. The Wheat Ridge Fire Department approved removal and the tank was removed, cleaned, inspected, and loaded for transport. Removal was completed by 10 am. Aftar removal of the final tank, soils were removed from the gas tank pit to a depth of 17 feet in the north end of the tank pit. After excavation, we collected 8 soil samples from the bottom of the tank pit (hvo beneath each tank), and from all tank walls. Field testing indicated the contaminated soils were primarily confined to the northwest corner of the tank pit. This area is directly below two sets of piping. We also collected one soil sample from the gas tank pit stockpile which field tested at 300 ppm. After collecting soil samples from the gas tank area, the wiaste oil tank pit was excavated to a depth of 9 feet. We collected 1 representative soil sample beneath the waste oil tank, and a sample from the waste oil tank stockpile. Approximately 50 gallons of sludge was hand shoveled out of the waste oil tank (mud). This sludge was added to the stockpile. Since the soils which had been excavated from the waste oil tank area were visibly contaminated, the stockpile (consisting of approximately 50 cubic yards) was set aside and secured with visquine. The waste oil tank pit was filled with clean fill dirt and the rear of the station over-graded and compacted. The gasoline tank pit was then fillzd with clean fill dirt to a uniform depth of 5 feet. At this point the stockpiled soils which had baen removed from the excavation were spread on top of the gas tank excavation and compacted. All soil samples we[e sealed and identified on the job site, logged to the Site Plan and placed in a refrigerated cooler for storage pending delivery to the testing laboratory. Soil samples were identified SOC. At 1 pm we delivered all soil samples to Environmental Chemistry Services, Inc., and prepared a Chain of Custody Record. Testing was specitied for Total Volatile Petroleum Hydrocarbons for the gasoline tank pit samples, and stockpile, and for PetroSite Environmental Services emin.weevwue.ma. Total Recoverable Petroleum Hydrocarbons for the waste oil tank pit sample and stockpile. All samples were tested for BTEX. At 2 pm we called State Oil Inspection and reported our preliminary findings to Woody Stevens. We repoRed that releases have occurred at this site as determined by visual observations and field testing conducted during tank removal. FINDINGS: Elevated levels of petroleum hydrocarbon contamination were found beneath the north end of the wast gas tank, beneath both ends of the east gas tank, and beneath the waste oil tank. A low level of contamination was also found along the vvest wall of the gas tank pit. Based on our observations at the site, field testing, and as confirmed by the results of laboratory testing, wie believe releases are associated with: WASTE OIL TANK: The fill pipe rusted out and broke off at the top of the tank. Water from the building drains seeped into the tank forcing waste oils up and out into the surrounding soils. Contamination was greater at the top of the tank pit and diminised with excavation depth. Due to the close proximity of the building foundation we did not feel excavation below 9 feet was advisable. Contamination level was reduced from 15,000 ppm to 6,100 ppm by excavation of 3.5 feet below the bottom of the tank. GASOLINETANKS: The tanks were intact. No evidence was found that the tanks were leaking. Two sets of product piping were found; the original steei piping (which had been abandoned in place at the northwest corner of the gas tank pit), and fiberylass piping. Thz old• piping system was apparently leaking at the time it was replaced with fiberglass piping. There is also evidence of some spillage or overfillage of the east gasoline tank. Contamination appears heaviest in the bottom of the gas and waste oil pits. The tank walls on the east, north and the south are clean. Field samples from these walls did not test above detection limits in any of our field samples. These results are confirmed by lab samples for these same areas. Due to the close proximity of the building on the west edge of this tank pit we did not consider excavation below 17 feet to be advisable. Piping was removed to the edge of the station building (15 feet west of edge of tank pit); thereafter the fiberglass piping was cut and capped. RESULTS OF LABORATORY TESTING: Final testing results were received from Environmantal Chemistry Sarvices, Inc., on February 17, 1995. Copies of the ESC reporis are enclosed herein. All test results are summarized on Exhibit F. Lab results confirm field testing for elevated levels of petroleum hydrocarbons beneath the gas tanks and the waste oil tank. The levels of petroleum hydrocarbons reported by ECS are above Colorado Department of Health Guideline levels for Remedial Action Category (RAC I), which is the most stringent guideline level for cleanup. Due to the proximity of permitted welis in this area, RAC I status is indicated. Soil sample results also exceed CDPHE guidelines for additional investigation. Colorado Department of Health guideline level for cleanup in Remedial Action Category I is 100 parts per million, Total Petroleum Hydrocarbon, and 20 parts per million, Total BTEX. Colorado Department of Health investigation limits for all Remedial Action Categories is 20 parts PetroSite ik Environmental Services ARIiYIenfP0rvJ1te.1.. per million, Total Petroleum Hydrocarbon, and 5 parts per million, Total BTEX. Total Volatile Petroleum Hydrocarbons (TVPH), and Total Extractable Petroleum Hydrocarbons (TEPH) are compatible terms with Total Petroleum Hydrocarbons (fRPH) under CDH guidelines. Colorado Department of Health res2rves the right to review environmental reports on an individual basis. Based on the results obtained, and subject to final review by the Colorado Department of Health, a II Second Level Site Assessment is required within 45 days. Pending results of II Second Level Site Assessment, a Corrective Action Pian is required. Based on the results of samples collected from the east, south and north tank walls, and the low level of 140 ppm in the west wall, we believe that the horizontal extant of soil contamination in the gasoline tank pit has been defined within guidelines set by CDPHE. RECOMMENDATIONS: We recommend that two copies of this report be filed with the Colorado Department of Health. Subject to their final review, a II Second Level Site Assessment and a Corrective Action Plan is required. Due to the proximity of the building foundation in the wasta oil tank area, excavation to lower depths could compromise the building foundation. Due to the proximity of the building foundation on the west edge of the gasoline tank pit, and the fact that some caving occurred in the gas tank pit, excavation to lower depths could result in damage to the foundation of the building in this area. Petroleum contaminated soils which are stockpiled on site should be disposed of at an authorized disposal facility pending waste characterization and acceptance by the landfill. The gasoline tank pii was over-excavated to an average depth of 16 feet which was the maximum depth with the equipment available. Contaminated soils were removed to 16 feet, replaced with clean fill from the bottom up, and over-lain with the contaminated soils from a depth of 5 feet to the surface. This action was taken to lower the potential for migration and to isolate the contaminated soils if removal of contaminated soils is elected versus remediation. LIfVIITATIONS: This report and our Findings and Recommendations are limited strictly to the tank area that is specified on the enclosed Site Plan, Exhibit F. We did not conduct any other testing on any other portion of the property other than is specified on Exhibit F. The number of soil samples submitted for laboratory testing are sufficient, in our opinion, to meet Colorado Department of Health Owner/Operator Guidance Documents dated April 15, 1994. We believe that this report and the results of laboratory testing are adequate to assess the fank area a4 closure as specified in 6 CCR 1007-5, Subpart G, paragraph 28072. a es H.
