Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a filial inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

laboratory located in Midlothian, Virginia. Hayes Microbial is accredited by the American
Industrial Hygiene Association in Environmental Microbiology Laboratory ID: 188863. The

3.0 FINDINGS, CONCLUSIONS AND RECOMMENDATIONS
During the walk-through of the dining room and garage, extensive water staining and wall and
ceiling damage due to water intrusion was noted. Minimal clearance limited our access to the
attic. Our visual inspection was limited to above the ceiling through the garage attic access
panel.

Materials containing asbestos quantities of one percent (I%) or less are also be subject to
OSHA regulations if air concentrations sampled during demolition or renovation
activities are at or above the permissible exposure limit (PE) of 0.10 fibers per cubic
centimeter ( /cc) as an eight hour time weighted average, or the excursion limit of 1.0 f/cc
as averaged over a 30 minute sampling period.
2.0 SITE / BUILDING INFORM.ATION
The Site is residential property located at 10505 West 26 1h Avenue, Wheat Ridge,
Colorado, 80215. This was a limited—destructive, pre-renovation investigation.

Drawings of the existing conditions and renovation designs were not provided to Family
Environmental prior to the inspection and sampling activities. The residence is
approximately 4,800 square feet (ft2 ) and is primarily composed of brick, concrete, wood
and drywall construction. The inspection area was limited to the dining room and garage.
3.0 FIELD ACTIVITIES
On February 4, 2015, an AHERA/CDPHE accredited asbestos inspector from Family
Environmental conducted a limited asbestos inspection of the Site. The purpose of this
asbestos inspection was to evaluate for the presence of AGMs in the dining room and
garage which had a prior water intrusion event. The inspection was conducted in
accordance with the methods and procedures outlined in the aforementioned regulations.
3.1 Visual Assessment
Appendix A provides a summary of the areas determined to be homogeneous and the
laboratory results for those areas,

A tactile assessment of each hornogeneous area of suspect ACM was conducted to assess
the general condition and the friability of the material. Friability was assessed by
physically touching suspect materials. Following categorization for material type and
friability, a sampling strategy was developed based on the following requirements:

Each sample was placed in a seated container and labeled with a unique sample number
using an indelible marker. Appendix C contains the Laboratory Analytical Summary of
all of the bulk samples collected.
3.3 Chain of Custody
Samples were delivered along with a completed chain-Of-CUstody (COC) to River North
Environmental Testing (R,NET). Laboratory personnel signed and dated the COC at the
time of delivery to acknowledge receipt of the samples, Upon receipt, the laboratory
arranged the samples in numerical order to establish any discrepancies between the

samples set which was delivered and the COC forrn. Where (it) discrepancies exist,
laboratory personnel noted and initialed the COC and reported the error to the inspector.
The laboratory included the signed original copy of the COC with the final analytical
report for evidentiary purposes,

Current EPA and State of Colorado regulations do not require the removal of materials
found to contain TRACE ASBESTOS (<]%) prior to initiating renovation activities that
will impact this material. Materials containing trace asbestos are still subject to OSHA
worker protection laws. The following materials contain trace amounts of asbestos:
0 SMDW: white surface material on white drywall located on the garage walls.
6.0 CONCLUSIONS & RECOMMENDATIONS
Based on our observations and the analytical results, friable asbestos is present in the area
of the inspection. All friable materials must be removed by a certified abatement
contractor prior to renovation or demolition.
If during demolition/renovation activities, materials are encountered which have not been
discussed in this report; stop all activities in the immediate area, and contact a certified
asbestos building inspector to sample the suspect material. Family Environmental is
available to perform further sampling at your request.
