Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

TERMS AND CONDITIONS
1. STANDARD OF CARE. Ethos General Contractors, LLC
("Ethos") will perform the services in connection with this
agreement in accordance; with the level of care and skill ordinarily
services under similar circumstances at the time Ethos provided

2. LIMrrED WARRANTY. Ethos warrants to property owner that
for a period of two years from the date of this agreement, Ethos's
workmanship will materially comply with the standard of care set

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

315t Avenue in Wheat Ridge, Colorado to investigate renorted damage from a fallen
tree. This report contains the findings and conclusions reached as a result of that
investigation.
Batkground: The subject structure is a south-facing, wood-framed, two-story residence
with stucco and brick-veneer exterior cladding. Original construction of the home, as
indicated by the Jefferson County Assessors office, occurred in 1979. Reportedly, in
1999, the home was expanded, which included the conversion of the original garage
into a two-stary living space, and the construction of a two-story addition a# an oblique
angle to the home's original eastern exposure. For reference, a photograph of the
subject home is contained in Figure 1.
Reportedly, on or about May 26, 2006, a large tree that was located south of the subject
structure fell, striking the roof, as depicted in Figure 2.
Purpose: The purpose of this investigation was to determine the extent of structural
damage resulting from the subject fallen tree, and to provide repair recommendations
for tree-impact-related damage.
Procedure: Knott Laboratory performed inspections of the subject structure on May 26
and June 1 of 2006. Available evidence was collected, documented and photographed.

Findings and Distussion: The preliminary inspection of the subject home took place
immediately following the tree impact, which revealed that the tree had struck the roof
covering the converted garage area. At that time, Knott Laboratory provided
temporary bracing recommendations to the on-site contractor, Disaster Restoration,
Inc., in order to minimize the potential for further dantage prior to removal of the
subject tree.
Knott Laboratory inspected the structure following the removal of the tree and found
that the roof framing in this area consisted of north/south-oriented, manufactured
wooden scissor-trusses, which were spaced at 24 inches on-center. Five severely
damaged trusses were observed in the vicinity of the tree-impact, as depicted in Figures
3 and 4. Significant damage was also observed to the blocking along the southeastern
hip (Figure 5), as well as to isolated additional blocking elements in the area of the tree
impact. Damaged OSB sheathing was observed coincident with these elements.
The presence of roofing and interior drywall, coincident with limited attic access
prevented a direct visual inspection of the remaining roof strucrixre components.
However, an inspection of the remainder of the roof surface revealed that the ridge,
valley and hip lines, as well as the field areas were visibly straight and true. The roof
slope was found to be relatively consistent throughout the field areas of the roof, and
minimal damage to interior drywall away from the impact area was visible. Therefore,
Knott Laboratory concluded that structural damage to the roof was likely limited to the
aforementioned 5 trusses and adjacent blocking. However, a subsequent inspection
following the removal of the damaged trusses should be performed to affirm the limits
of structural damage.
Knott Laboratory inspected the exterior walls of the home and found the walls to be
straight and plumb. No significant cracking and/or displacements of the exterior brick
and stucco was observed. Therefore, Knott Laboratary concluded that structural
damage resulting from the fallen tree was limited to the roof framing elements
discussed in the first paragraph of this section. However, as previously indicated, a
subsequent inspection should be performed when adjacent framing elements are
visible, following the removal of the damaged trusses.
Repair Recommendations: In conducting this investigation, Knott Laboratory contacted the
local building official, the City of Wheat Ridge Building Department, (City) and found
that the City has adopted the 2003 International Building Code (IBC) and the

International Residential Code (IRC) with local amendments. Additionally, the City
requires that new roof structures be capable of resisting the following loads:
Uniform snow-loads of 30 pounds-per-square-foot (ps fl,
Wind-loads induced by a 110 mile-per-hour (mph) wind (3-second gust) acting on a
category B exposure.
It is Knott Laboratory's opinion that rehabilitation to the aforementioned damaged
trusses is not possible. Therefore, Knott Laboratory recommends that the trusses be
removed and replaced with new manufactured wooden trusses. The new wooden
trusses shall be designed by an engineer registered in the State of Colorado, and truss
shop drawings shall be submitted bearing the truss-design engineer's seal. Loads used
in design shall be clearly indicated on the shop drawings: 30 psf snow-Ioad,10 psf dead
load on the top chord, 8 psf dead load on the bottom chord, and wind-loads induced by
110 mph wind acting on a category B exposure and calculated in accordance with the
2003 IBC. The new trusses shall be anchored to the exterior walls using Simpson
Strong-Tie H2.5a Hurricane Ties. All damaged blocking shall be replaced with similar
members.
New roof sheathing, consisting of 19/32-inch thick, 40/20 span rated OSB shall be
installed on the new trusses and shall be fastened using 10d nails spaced at 6-inches at
the panel edges and 12-inches elsewhere.
Knott Laboratory shall be contacted to perform an inspection of the structure
coincident with the truss removal. A minimum of 48 hours lead time is required.
Contlusion: Based upon Knott Laboratory's inspection, the available evidence, and these
engineers' education, training, and experience, the following conclusions have been
reached within a reasonable degree of engineering certainty:
• Structural damage resulting from the fallen tree is likely limited to five
broken trusses, fractured adjoining blocking and crushed sheathing in the
area of the tree impact.
• The observed structural damage shall be repzired in accordance with the
recommendations contained in this report.

• Knott Laboratory shall be contacted to perform an inspection coincident with
the removal of the damaged trusses. A minimum of 48 hours lead time is
required.
The opinions and findings expressed in this report are based upon the information
available to these writers as of the date of this report and are the result of limited non-
destructive visual inspections of the exposed building components. Due to the limited
nature of the investigation Knott Laboratory cannot be held responsible far any hidden
defects that may negatively impact the performance of the structure. This report is

recommendations for structurai repairs to components damaged by the subject fallen
tree, and should not be used as an indicator of future performance of unrelated

Dear Mr. Hoffman:
Pursuant to the provisions set forth in the Engineering Report dated June 5, 2006, Knott
Laboratory conducted a supplemental inspecrion of the property located at 6895 West
31st Avenue in Wheat Ridge, Colorado following the removal of the interior drywall
ceIling in the area affected by the fallen tree. This supplemental inspection was
recommended in order to affirm the limits of structural damage resulting from the tree,
which fell onto the roof of the subject structure on May ?6, 2006. The inspection was
performed on June 23, 2006 and revealed that structural damage to the home was in-
fact, limited to the 5 broken trusses and blocking components that were identified in the
June 5 Report. Consequently, Knott Laboratory's repair recommendations contained in
that report remain unchanged.
In conducting the June 23 inspecrion, Knott Laboratory also inspected the truss beazing
condirions on the southern wall and found the wall to be relarively straight and absent
of physical damage. However, at the time of the re-inspection, a failed truss remained
in place at the approximate mid-span of a window opening, thereby preventing a
thorough inspection of the top plate/header beam profile immediately above the
window. Therefore, upon the removal of the trusses, the Contractor shall check the
profile of the wall top plate using a level string line, and shall contact Knott L.aboratory
if any deflections greater than 1/4-inch are observed.
Provided that the string line test does not reveal significant deflections as outlined in
the previous pazagraph, ICnott Laboratory shall conduct a compliance inspection of the

new roof framing. That inspection shall be performed following truss and sheathing
installation, and prior to any application of interior drywall and permanent roof
weatherproofing components. A minimum of 48 hours lead time is needed.
The opinions and findings expressed in this supplemental report are based upon the
information available to these writers as of the date of this report and are the result of
limited visual inspecrions of the exposed building components. Due to the limited
nature of the investigation Knott L.aboratory cannot be held responsible for any hidden
defects that may negatively impact the performance of the siructure. This report is
intended to provide an overview of the exisring conditions, and to provide
recommendations for structural repairs to components damaged by the subject fallen

PRELIMINARY REPORT
At your request, Knott Laboratory, LLC. inspected the property located at 6895 West
315t Avenue in Wheat Ridge, Colorado to investigate reported damage from a fallen
tree. This report contains the findings and conclusions reached as a result of that
investigation.
Backt,~round: The subject structure is a south-facing, wood-framed, two-stary residence
with stucco and brick-veneer exterior cladding. Original construction of the home, as
indicated by the Jefferson County Assessors office, occurred in 1979. Reportedly, in
1999, the home was expanded, which included the conversion of the original garage
into a two-stary living space, and the construction of a two-story addition at an oblique
angle to the home's original eastern exposure. For reference, a photograph of the
subject home is contained in Figure 1.
Reportedly, on or about May 26, 2006, a large tree that wus located south of the subject
structure fell, striking the roof, as depicted in Figure 2.
PurRose: The purpose of this invesrigation was to determine the extent of structural
damage resulting from the subject fallen tree, and to provide repair recommendations
for tree-impact-related damage.
Procedure: Knott Laboratory performed inspections of the subject structure on May 26
and June 1 of 2006. Available evidence was collected, documented and photographed.

Findings and Distussion: The preliminary inspection of the subject home took place
immediately following the tree impact, which revealed that the tree had struck the roof
covering the converted garage area. At that time, Knott Labaratary provided
temporary bracing recommendations to the on-site contractor, Disaster Restoration,
Inc., in order to minimize the potential for further damage prior to removal of the
subject tree.
Knott Laboratory inspected the structure following the removal of the tree and found
that the roof framing in this area consisted of north/south-oriented, manufactured
wooden scissor-trusses, which were spaced at 24 inches on-center. Five severely
damaged trusses were observed in the vicinity of the tree-impact, as depicted in Figures
3 and 4. Significant damage was also observed to the blocking along the southeastern
hip (Figure 5), as well as to isolated additional blocking elements in the area of the tree
impact. Damaged OSB sheathing was observed coincident with these elements.
The presence of roofing and interior drywall, coincident with limited attic access
prevented a direct visual inspection of the remaining roof structure components.
However, an inspection of the remainder of the roof surface revealed that the ridge,
valley and hip lines, as well as the field areas were visibly straight and true. The roof
slope was found to be relatively consistent throughout the field areas of the roof, and
minimal damage to interior drywall away from the impact area was visible. Therefore,
Knott Laboratory concluded that structural damage to the roof was likely limited to the
aforementioned 5 trusses and adjacent blocking. However, a subsequent inspection
following the removal of the damaged trusses should be performed to affirm the limits
of structural damage.
Knott Laboratory inspected the exterior walls of the home and found the walls to be
straight and plumb. No significant cracking and/or displacements of the exterior brick
and stucco was observed. Therefore, Knott Laboratory concluded that structural
damage resulting from the fallen tree was limited to the roof framing elements
discussed in the first paragraph of this section. However, as previously indicated, a
subsequent inspection should be performed when adjacent framing elements are
visible, following the removal of the damaged trusses.
Repair Recommendafions: In conducting this investigation, Knott Laboratory contacted the
local building official, the City of Wheat Ridge Building Department, (City) and found
that the City has adopted the 2003 International Building Code (IBC) and the

International Residential Code (IRC) with local amendments. Additionally, the City
requires that new roof structures be capable of resisting the following loads:
Uniform snow-loads of 30 pounds-per-square-foot (pso,
Wind-loads induced by a 110 mile-per-hour (mph) wind (3-second gust) acting on a
category B exposure.
It is Knott Laboratory's opinion that rehabilitation to the aforementioned damaged
trusses is not possible. Therefore, Knott Laboratory recommends that the trusses be
removed and replaced with new manufaciured wooden trusses. The new wooden
trusses shall be designed by an engineer registered in the State of Colorado, and truss
shop drawings shall be submitted bearing the truss-design engineer's seal. Loads used
in design shall be clearly indicated on the shop drawings: 30 psf snow-load, 10 psf dead
load on the top chord, 8 psf dead load on the bottom chord, and wind-loads induced by
110 mph wind acting on a category B exposure and calculated in accordance with the
2003 IBC. The new trusses shall be anchored to the exterior walls using Simpson
Strong-Tie H2.5a Hurricane Ties. All damaged blocking shall be replaced with similar
members.
New roof sheathing, consisting of 19/32-inch thick, 40/20 span rated OSB shall be
installed on the new trusses and shall be fastened using lOd nails spaced at 6-inches at
the panel edges and 12-inches elsewhere.
Knott Laboratory shall be contacted to perform an inspection of the structure
coincident with the truss removal. A minimum of 48 hours lead time is required.
Conclusion: Based upon Knott Laboratory's inspection, the available evidence, and these
engineers' education, training, and experience, the following conclusions have been
reached within a reasonable degree of engineering certainty:
• Structural damage resulting from the fallen tree is likely limited to five
broken trusses, fractured adjoining blocking and crushed sheathing in the
area of the tree impact.
• The observed structural damage shall be repaired in accordance with the
recommendations contained in this report.

Dear Mr. Stoeftler:
We are in receipt of your analysis of the pernut submittal for the remodel and addition at the
above noted address.
With respect to the ten items listed for correction we are herewith submitting the following:
(1) We have reanalyzed the project using the 1997 UBC and have so stipulated on Sheet
Al.
(2) We have included an 1/8" scale drawing on sheet A2 showing the existing wall
configuration and the walls to be demo'd as a resuit of the remodel. Please note that
no structural walls will be removed and the two bedroom areas aze to remain.
(3) Please note the window schedule on Sheet A2 and the accompanying note referencing
section 1203.1.2.3 of the 1997 UBC and section 310.4. All windows including
existing windows (not to be removed) comply with these sections.
(4) Please note that the stairs do terminate at a concrete patio. If required we will isolate
a 30 pad in the concrete patio. All other exterior doors either open onto concrete
drive or concrete patio.
(5) We have includgd smoke detectors per UBC Section 310.9. See Sheet A8.
(6) See electric service calculations Sheet A8.
(7) The outlet at the kitchen island has been designated GFCI. See Sheet A8.
(8) Please see Sheets A2 & A9 for location and size of existing water service.
(9) Existing timber retaining wall will be removed and lot graded accordingly. See Sheet
Al. In no instance will grades exceed a 4 to 1 slope.
(10) We sincerely apologize for the lack of coordination between the surveyors spot
elevation sheet and Sheet Al. The A1 sheet originally submitted was preliminary and
should not have been included with the submittal drawings.
