I . This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a changeof the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable cgq,or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

Earth Energy Environment (E3) is pleased to submit results for the bulk sampling at the residential
home ("subject property") located at 10035 West 29th Avenue, Wheat Ridge, CO. The sampling
was conducted on February 3, 2020.
The sampling was conducted for the purpose of determining if asbestos would be disturbed as part
of the master bathroom renovation.
For the survey, samples of surfacing material were collected using the EPA random sampling
protocol outlined in the EPA publication, "Asbestos in Buildings: Simplified Sampling Scheme
for Friable Surfacing Materials" ("Pink Book', EPA 560/5-85-030x, October, 1985).
LIMITATIONS
This investigation was conducted and the report prepared solely for property owners. No
warranties for other uses are applied and intended. Evaluations of conditions at this site were made
from a limited number of observation points and conditions cannot be warranted at non -
observation points or non -accessible areas.
The sampling included the suspect asbestos containing materials based on visual observations and
discussion with the property representative.
Interpretations and conclusions made in this report that pertain to the asbestos are based solely on
results of independent laboratory analyses of the samples taken at the property at the date and time
indicated.
E3 has no control over such testing and analyses and, therefore, disclaims any responsibility for
errors or omissions arising there from.

INSPECTION RESULTS
E3 identified one (1) homogeneous area, as summarized below. E3 collected at least the minimum
number of samples for the homogeneous area necessary to meet all regulatory requirements.
Homogeneous Areas (HA):
Homogeneous areas were selected from the building materials observed during the site inspection.
The homogeneous areas sampled during the inspection are listed below.
HA -1: Bathroom Walls and Ceilings
Three (3) samples were collected for asbestos testing at the subject site. The laboratory separated
and analyzed individual layers within each sample, if present (see Attachment A). Representative
photographs of the building materials are available on request.
According to the laboratory analysis of the materials sampled, asbestos was detected in the
ceiling texture.
Any potentially asbestos containing materials identified in the future or during demolition that
were not sampled in this, or any other previous survey, should be tested for asbestos content or
assumed to contain asbestos before being disturbed.

Environmental matrices by the National Voluntary Laboratory Accreditation Program (NVLAP), Lab Code
101896-0 for Transmission Electron Microscopy (TEM) and Polarized Light Microscopy (PLM) analysis and the
American Industrial Hygiene Association (AIHA), Lab ID 101533 - Accreditation Certificate #480 for Phase
Contrast Microscopy (PCM) analysis. This laboratory is currently proficient in both Proficiency Testing and PAT
programs respectively.
Reservoirs Environmental, Inc. has analyzed the following samples for asbestos content as per your request.
The analysis has been completed in general accordance with the appropriate methodology as stated in the
attached analysis table. The results have been submitted to your office.
RES 455465-1 is the job number assigned to this study. This report is considered highly confidential and the
sole property of the customer. Reservoirs Environmental, Inc. will not discuss any part of this study with
personnel other than those of the client. The results described in this report only apply to the samples analyzed.
This report must not be used to claim endorsement of products or analytical results by NVLAP or any agency of
the U.S. Government. This report shall not be reproduced except in full, without written approval from
Reservoirs Environmental, Inc. Samples will be disposed of after sixty days unless longer storage is requested.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding AX -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€7"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.
