The suspect materials were sampled on October 6, 2020. A total of three (3) bulk samples of suspect ACM were collected of the materials in question to provide adequate representation of the homogenous materials in question. All samples were analyzed by Polarized Light Microscopy (PLM) at Reservoirs Environmental Inc., an American Industrial Hygiene Association (AIHA) accredited laboratory. Samples demonstrate that the identified material samples are as listed within the table by Reservoirs Environmental Inc., following within this document. In my opinion, wall materials are homogenous, due to the age, exposure, and appearance. According to EPA (NESHAP) as published in Federal Register, 40 CFR, Part 61, Subpart M, and AQCC Regulation g, "if the asbestos content is less than ten percent (10%) as determined by a method other than point counting by polarized light microscopy (PLM), verify the asbestos content by point counting using PLM. The purpose of the inspec- tion was to observe the foundation at the converted garage and front addition of the home, in order to de- termine my opinion of the structural condition and need for structural repair. The inspection did not in- volve any other system or component of the residence. No soil or other material sampling, testing, or analysis was performed by Coyle Structural Inspection Engineers. General Information: • The home is a 1 story single family home structure, assumed to be facing Northwest. The home was reportedly constructed in 1952, but with an additions at the front of the converted garage. • The foundation appears to be of poured concrete construction. • No soils report or other engineering documents were available for review. • Provided for review was a proposal by Mike Schmidt for Pinnacle Structural. Observations, Opinions and Recommendations: The garage was converted to interior space at some point in the past. The floor is a concrete slab on grade, which appears independent of the foundation. Settlement has occurred at the NE wall of the former garage and at the front/NW side of the addition. It is recommended to underpin these areas with 8 Helical Piers.
