Subject: Visual structural foundation evaluation for 9305 W 381h Ave., Wheat
Ridge, CO.
Dear Mr. Henry,
Per your request, I visited the house at the above referenced address on
July 21, 2020. The purposes of my visits were to observe the raised
concrete garage slab and provide centerline support recommendations as
required.
Please note: My investigation was based on observation of accessible,
visible items. Issues/concerns concealed by finishes, if any exist, were
not able to be observed, and were therefore not evaluated as part of this
investigation. Issues and repairs, if required, regarding other parts of the
home were not part of the scope of this investigation.

For purposes of this report, the front of the home faced south. The raised
garage floor consisted of a 6 -inch thick reinforced concrete slab. #5 rebar
was observed at some penetrations. The slab was approximately 37 -feet
8 -inches long and 18 -feet 9 -inches wide. The slab was intermediately
supported on a combination of a composite steel beam and wood bearing
wall. The support line was oriented in the north/south direction and was
located approximately 8 -feet 2 -inches from the west foundation wall. A

portion of the south end of the slab was located in front of the garage
door. The existing steel beam appeared to have been fabricated using a
steel tram rail section (inverted) at the top, and a custom structural tee
(inverted). The tee web was perforated on 12'/2 -inch centers. The
perforations were trapezoidal shaped with the long base against the rail
and 9 -inches long, and the short base 3'/4 -inch long. The connection
between the rail and the tee was slightly more than 3 -inches long.
The homeowner requested the wood stud bearing wall be removed and
replaced with an additional steel beam at the north end of the garage and
both the existing and new beams be supported on new pipe columns on
independent reinforced concrete pad foundations..
Recommendations
1) 1 recommend adding a new 50 ksi W8x18 steel beam at the north
end, and five (5) 4" schedule 40 steel pipe columns to support the
existing and new beams per the attached engineering drawings.
2) Other foundation issues at other locations may exist, however the
scope of this beam replacement plan is specifically limited to the
garage slab support system.
3) It is very important to maintain proper drainage away from the
garage and house. I do not recommend irrigating within 5 -feet of
the foundation. Maintain a minimum 10 % slope away from the
structure, and direct surface water to an adequately sloped swale.
Grass, plants, trees and shrubs with high watering requirements
should not be located within five feet of the foundation. All gutter
downspouts should have functioning downspout extensions, which
extend at least five feet away from the foundation and discharge in
a manner which allows effluent to flow safely away from the home,
or discharge directly to an underground pipe system with an
appropriate daylight location. Perimeter drains, if they exist, should
function properly. Perimeter drains, and sump pumps, if they exist,
should function properly. Proper separation of grade to top of
foundation should be maintained.
I observed the raised concrete garage slab described above. Other city of
unknown concerns may also exist. The Lepetsos Group, Inc. makes
no representations regarding the suitability of other areas, including, Wheat Ridge
Building Division

but not limited to mechanical, electrical, plumbing, previous foundation
repairs, and other systems. The homeowner should understand
additional unforeseen and unforeseeable issues related to soil heaving
and or settlement, and exposure to unwanted water may cause
additional foundation movement in the future. My evaluation and
recommendations are based on my education, experience and
observations made on the date of my visit. The Lepetsos Group, Inc.
makes no representations regarding the ability of the property to meet
current or past building code requirements. Please note it was not
possible to observe every structural detail. Contact this office if
additional information becomes available which could affect our
observations and recommendations.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.
