REGARDING ROOF VENTILATION: Roof ventilation shall comply with Building Division Policy
BD -18-001 for Balanced Ventilation Systems, or manufacturer installation instructions,
whichever is more stringent.
MIDROOF & SHEATHING: Effective December 1, 2014, asphalt shingle installations require an
approved mid -roof inspection, conducted when 25-75% of the roof covering is installed,
prior to final approval. Installation of roof sheathing (new or overlay) is required on the
entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists.
Sheathing and mid -roofs may be called in at the same time, one hundred percent of the
sheathing must be complete and 25-75% of the mid -roof may be complete. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required.
LADDER MUST BE SET FOR ALL ROOF INSPECTIONS: A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections.
COMMERCIAL ROOFS: In order to pass a final inspection of elastomeric or similar type roof
coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

SUBJECT: Front setback
The purpose of this memo is to summarize recent correspondence with a potential developer of
5240 W. 41 Avenue. The property is zoned Residential -Three (R -3), and under current
standards the front setback in R -3 is 25 feet as measured from the front property line.
The potential developer has proposed a 20 -foot setback which is similar to the setbacks on
adjacent properties. Based on measurements using aerial images and GIS, it appears that the
propertyto the east (3995 Sheridan) has a 20- to 25 -foot front setback, and the property to west
(5280 W. 41 Avenue) has a 15- to 20 -foot setback Based on these measurements and the
current provisions of Sec. 26 -611, a 20 -foot setback is permitted at 5240 W. 41 Avenue
without need for a variance.

This approval notice is granted subject to Colarado Air,Quality Control Commission Regulation
No. 8, Part B, adopted December 21, 2007, and effective January 30, 2008 and the Colorado Air
Pollution Prevention and Control Act C.R.S. (25-7-101 and 25-7-501 et seq). This notice signifies
that the structure was inspected for asbestos, luminous exit signs (containing radioactive material),
and Ozone-Depleting Refrigerants and the demolition contractor has properly notified the
Colorado Department of Public Health and Environment pursuant to Regulation No. 8, Part B.
As a contractor, you may be subject to other demol_;tzon licenses and-permi+_s, depending or_ the
requirements of the county and municipality in which the work is being performed. The Colorado
Department of Public Health and Environment, Air Pollution Control Division, strongly suggests
that you check with county and municipal authorities in order to determine any other local
building/permitting requirements that must be met.
Please note that certain asbestos-containing materials (AC1V) may remain in the structure
during demolition. Therefore, any demolition debris left behind after the completion of post-
demolition site cleanup may constitute a"reason to know of asbestos-contaminated soil" at
the site, subject to the requirements of Section 5.5 of the Solid Waste Regulations (6 CCR
