(Family Environmental) is pleased to submit this report summarizing the inspection activities conducted at the commercial building located at 11415 W. 1-70 Frontage N. Road, Denver, CO 80033 (the Site). Family Environmental was contracted by Kirby Kunz (the Client) on September 8, 2015 to inspect and identify suspected Asbestos Containing Material (ACM), collect and submit samples for laboratory analysis, and provide an inspection report. Fieldwork was performed on September 9, 2015. Data gathered during the visual inspection of the Site, collection of bulk samples of inspected suspect building materials, and the interpretation of analytical results is intended to provide information that meets federal, state, and local regulatory requirements. The sampling and inspection process was conducted in accordance with the following standards: • Asbestos Hazard Emergency Response Act (AHERA) — 40 CFR 763 Subpart E sections 763.80 to 763.99 • Colorado Department of Public Health & Environment (CDPHE) Regulation Number 8 (Reg. 8) • United States Environmental Protection Agency (EPA) National Emissions Standards for Hazardous Air Pollutants (NESHAP) — 40 CFR 61 Part M • Occupational Safety and Health Administration (OSHA) asbestos regulations — 29 CFR 1926 parts 58 and 1101, and 29 CFR 1910 Parts 134 and 1001 1.0 REGULATORY COMPLIANCE This inspection was performed in anticipation of renovation/demolition activates that are expected to impact suspected asbestos containing building materials. Asbestos has been added to over 3,000 materials and products produced and used in the United States. Suspected ACMs include nearly all building materials with the exception of glass, metal or rubber. According to AHERA and CDPHE Reg. 8, suspect ACMs must be properly sampled and submitted for analysis before any disturbance occurs at the Site. For a building material to be categorized as ACM, it must contain asbestos in a quantity greater than one percent, as confirmed by an EPA accredited National Voluntary Laboratory Accreditation Program (NVLAP) laboratory. ACMs are categorized into two subsequent categories, non -friable asbestos and friable asbestos. ACMs identified in this report are subject to the EPA NESHAP Regulations for Asbestos and the CDPHE Reg. 8. The CDPHE is responsible for the administration of the EPA NESHAP program in Colorado. EPA NESHAP prohibits the release of asbestos fibers into the atmosphere during renovation or demolition activities. The asbestos NESHAP requires that potentially regulated ACMs be identified, classified, quantified and removed where necessary prior disturbance. All friable ACM as well as non -friable ACM that can be rendered friable during demolition or renovation activities must be removed by a licensed General Abatement Contractor (GAC). Materials containing asbestos quantities of one percent (1%) or less are also be subject to OSHA regulations if air concentrations sampled during demolition or renovation activities are at or above the permissible exposure limit (PEL) of 0.10 fibers per cubic centimeter (f/cc) as an eight hour time weighted average, or the excursion limit of 1.0 f/cc as averaged over a 30 minute sampling period. OSHA regulations for General Industry and the Construction Industry include provisions for communication of hazards by the building owner, to building employees, or in the case of subcontracted building services, the subcontractor to subcontractor employees: • The employer must inform maintenance staff and subcontractors if building components have the potential to expose staff and subcontractors to asbestos. • The building owner shall maintain records concerning the presence, location and quantity of ACMs in the building for the duration of ownership, and records shall be transferred to any successive owners. • The building owner shall affix labels or signs to positively identified ACMs so that employees and building visitors will be notified of materials that contain asbestos. I-70 Frontage N. Road, Denver, CO 80033. This was a pre -renovation investigation. Drawings of the existing conditions and renovation designs were not provided to Family Environmental prior to the inspection and sampling activities. The building is approximately 12,297 square feet (ft) and is primarily composed of wood, metal, wall board, and drywall construction. The commercial building featured two (2) bathroom, kitchen, offices and a storage room. During the inspection, the inspector was able to identify and inspect: • Fifteen (15) functional spaces were identified. • Thirteen (13) functional spaces were surveyed. • Two (2) functional spaces were inaccessible or excluded from this inspection. o Wall/ceiling cavities in the building o Exterior on the building 3.0 FIELD ACTIVITIES On September 9, 2015, an AHERA/CDPHE accredited asbestos inspector from Family Environmental conducted a limited asbestos inspection of the Site. The purpose of this asbestos inspection was to evaluate for the presence of ACM in the building on this property. The inspection was conducted in accordance with the methods and procedures outlined in the aforementioned documents. 3.1 Visual Assessment Inspection activities began with visual observation of interior and exterior of the shop to identify homogeneous areas for suspect ACM. Homogenous areas (materials) are classified into one of the following three categories: • Surfacing Material (S) refers to a wide range of trowel or spray -applied materials typically used for acoustical, decorative or fire proofing purposes. • Thermal System Insulation (TSI) refers to insulation that is applied to heating or mechanical system components. Examples include pipe, tank and boiler insulation. • Miscellaneous Materials (M) refers to refers to all other materials that do not fall into one of the previously mentioned categories. Examples include floor tiles, adhesives, composite drywall systems and ceiling tiles. Appendix A provides a summary of the areas determined to be homogeneous and the laboratory results for those areas. 3.2 Physical Assessment and Sample Collection A tactile assessment of each homogeneous area of suspect ACM was conducted to assess the general condition and the friability of the material. Friability was assessed by physically touching suspect materials. Random bulk samples of suspect materials were collected in each homogeneous area using wet methods as applicable to reduce the potential for fiber release. During the inspection, Family Environmental staff identified thirteen (13) suspect homogenous materials, and collected Thirty-eight (38) bulk samples. The following major categories of homogenous areas were identified as being suspect asbestos containing materials: • Baseboard Adhesive (BBA): Two (2) unique BBA's were identified during this inspection and two (2) were sampled. Laboratory analysis determined that the two (2) BBA's identified during this inspection were non -detect for asbestos. • Carpet Adhesive (CA): Two (2) unique CA were identified during this inspection, and two (2) were sampled. Laboratory analysis determined that the two (2) CA identified during this inspection were non -detect for asbestos. • Composite all (CDW): One (1) unique CDW was identified during this inspection, and one (1) was sampled. Laboratory analysis determined that the one (1) CDW identified during this inspection was non -detect for asbestos. • Floor Tile Composite FTC): Two (2) unique FTC's were identified during this inspection and two (2) were sampled. Laboratory analysis determined that the two (2) FTC's identified during this inspection were non -detect for asbestos. • Lav -In Ceiling Panel (LIP): Two (2) unique LIP'S were identified during this inspection and two (2) were sampled. Laboratory analysis determined that the two (2) LIP's identified during this inspection were non -detect for asbestos. • Textured Surfaces (TS): Three (3) unique TS's were identified during this inspection and three (3) were sampled. Laboratory analysis determined that the one (1) WP identified during this inspection was non -detect for asbestos Each sample was placed in a sealed container and labeled with a unique sample number using an indelible marker. Appendix C contains the Laboratory Analytical Summary of all of the bulk samples collected. 3.3 Chain of Custody Samples were delivered along with a completed chain -of -custody (COC) to River North Environmental Testing (RNET) Laboratory. Laboratory personnel signed and dated the COC at the time of delivery to acknowledge receipt of the samples. The laboratory included the signed original copy of the COC with the final analytical report for evidentiary purposes. 4.0 LABORATORY ANALYSIS All bulk samples were relinquished to RNET. All samples were analyzed via Polarized Light Microscope (PLM) in accordance with EPA/600/R-93/116 Method for the Determination of Asbestos in Bulk Building Materials. RNET is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP) under Laboratory Code 200448-0. A copy of the RNET Report of Test Results can be found in Appendix C of this report. 5.0 ASBESTOS CONTAINING MATERIALS The United States EPA and OSHA define an asbestos containing material as a material containing greater than 1% asbestos, or a material that is assumed to contain asbestos. Materials that contain asbestos at or below one percent (<%) are considered to be trace asbestos containing. Current EPA and State of Colorado regulations require the removal of both confirmed, and assumed, FRIABLE ASBESTOS prior to renovation or demolition activities that will impact these materials. The following is a list of friable ACM observed by the inspector: • No friable asbestos was observed during this inspection. Depending on the renovation or demolition techniques used, the current EPA and State of Colorado regulations may require the removal of both confirmed, and assumed, NON -FRIABLE ASBESTOS prior to any activities that may impact these materials. The following is a list of non -friable ACM observed at the site: • No non -friable asbestos was observed during this inspection. Prior to any disturbance, the following ASSUMED ASBESTOS (ACM) should be sampled by a certified asbestos building inspector to determine the actual asbestos content, or be treated as asbestos containing: • No asbestos was assumed to contain asbestos during this inspection. Current EPA and State of Colorado regulations do not require the removal of materials found to contain TRACE ASBESTOS (<1%) prior to initiating renovation or demolition activities that will impact this material. If trace (<1%) asbestos materials exist on the site, and they are not removed, the renovation or demolition contractor must be notified of the presence of this material prior to disturbance and comply with all current OSHA regulations for employee exposure to asbestos. Additionally, the renovation or demolition contractor must comply with applicable NESHAP asbestos emission control regulations for structures containing trace amounts of asbestos. These materials may not be subject to sanding, grinding or cutting during renovation or demolition activities. If during demolition/renovation activities, materials are encountered which have not been discussed in this report; stop all activities in the immediate area, and contact a certified asbestos building inspector to sample the suspect material. Family Environmental is available to perform further sampling at your request. 6.0 LIMITATIONS This asbestos inspection was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale.
