I. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change, of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit f r or an apprany iel 'on of any provision of any

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

LICENSE AGREEMENT
LICENSE AGREEMENT entered into as of by Russell E. and Janice P. Anderson with an address at
22993 Valley High Rd., Morrison, Co 80465 (hereafte~icensor") and United Advertising Corporation of Colorado, Inc.
with an office at 498 West Iliff Ave., Denver, CO 80223 (hereafter "Licensee").
IN CONSIDERATION of the payment of a certain sum of money (the "License Fee"), by Licensee to Licensor,
payable monthly in advance in equal installments on a yearly basis, as set forth in 2., below, and other good and valuable
consideration, the receipt of which are hereby acknowledged, the parties agree as follows:
1. Licensor hereby grants to Licensee for the exclusive right to construct, operate and maintain one (1) or
two (2) 14'h x 48'w advertising faces supported by one single-pole structure measuring up to 50' in overall height
(hereafter "Sign Structure"), for a for the property known as 6400-6470 West 48th Ave., Wheat
Ridge, CO 80033 also known as Parcei iu # ~y-13's-07-001 in the City of Wheat Ridge, Colorado (hereinafter
"Premises"). The Sign Structure shall be constructed on a mutually agreed upon portion of the Premises as generally
described and attached hereto as Exhibit A.
2. Licensee shall pay Licensor the License Fee, in equal monthly installments on a yearly basis according
to the License Fee Payment Schedule attached hereto as Exhibit B. no later than the 10t" day of each month,
commencing April 2, 2004 (hereafter "License Date"). Licensor and Licensee acknowledge that the Premises is currently
subject to an existing lease with Viacom Outdoor, which Licensor agrees to terminate according to the lease provisions,
effective April 1, 2004. Licensee shall be permitted to erect its sign structure immediately following the vacation of the
Premises by Viacom after termination of the Lease.
3. In the event that the License Fee is not paid on or before the 10th day of the month Licensee shali pay a
late fee of 5% of the rent for said month. Failure to pay License Fee and any applicable late fee on or before the 20th
day of the month shall cause this Agreement to be terminated at the sole option of the Licensor. Licensor cannot
terminate License for non-payment of the License Fee unless Licensor provides written notice by certified mail, return
receipt requested, and first class mail to the address listed below, attention accounts payable, and Licensee has not
cured the non-payment within 15 days of the mailing of notice.
4. This License is granted for the construction, operation, repairing, rebuilding within the size limitation as
described in paragraph 1., and maintenance of the Sign Structure, with free access and convenient access to the Sign
Structure. Licensee is herewith granted the sole and exclusive right to display advertising copy on the Sign Structure
unless othervise specified in this agreement. Licensee shall have the right to erect, place and maintain advertisements,
sign structures and all necessary equipment therefore on the Premises and post, paint, illuminate and maintain
advertisements on the Sign Structure. All displays, equipment and materials placed upon the Premises by the Licensee
shall always remain the property of, and may be removed by the Licensee at any time prior to the expiration of this
License, irrespective of whether and how such property is affixed to the Premises. After this License expires, Licensee
agrees to remove the Sign Structure within fifteen (days) and restore Premises to its original condition, reasonable wear
excepted.
5. This License Agreement may be terminated by Licensee if Licensee is or shall be prevented by any
present or future law or ordinance, or by the authorities having jurisdiction, from constructing or maintaining on the
Premises the Sign Structure. Licensee shall obtain any required permit or approval (collectively the "Permit") from the
appropriate governmental or quasi-governmental authorities having jurisdiction for the construction and maintenance of
the Sign Structure. Licensor agrees to execute any forms required in order for Licensee to obtain and maintain a permit
only for the term of this License.
6. Licensee will save Licensor harmless from all damage to persons or property by reason of accidents
resulting from the negligent acts of its agents or employees utilized in the construction, operation, repair, maintenance, or
removal of its Sign Structure on the Premises. Licensee, throughout the term of this License, shall maintain a policy of
public liability insurance not less than $1,000,000 single limit coverage.
IN WITNESS WHEREOF, the parties hereto, after reviewing this three-page agreement, have executed the
same on the date shown above.
