Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection

manufacturer technical representative stating that "the application of the roof at (project

Manhard Consulting (Manhard) is acting as the point of contact for the slope failure at 6985
and 6987 W. 48th Avenue. We are working with the property owner (Lisa Achziger),
Michael W. West and Associates, Inc (MWWAI), and a specialty contractor to stabilize the
slope and prevent further instability.
The slope failure occurred primarily over a saturated sand a gravel layer overlying
claystone bedrock. The failure affected a timber wall but did not appear to harm the two
existing concrete walls upslope.
We are proposing that monitoring and stabilization be put in place as soon as possible.
Manhard will establish a system of survey points along the existing concrete walls to verify
that no further movement is occurring. In addition to establishing drainage address the
seepage, we are proposing that the slope be stabilized with soil nails or ground anchors.
Enclosed is a letter from MWWAI explaining in more detail what I summarized above.
With these steps approved and put into action in a timely manner, per the letter
condemnation of the structure or eviction of the occupants will not be necessary.

SUBJECT: Letter Report, Slope Failure, 6985 W. 48th Ave., Wheat Ridge, CO
Dear Ms. Achziger:
Michael W. West & Associates, Inc. (MWWAI) is providing this letter
report as an evaluation of a slope failure at your residence as referenced above.
We understand the failure occurred on or about Saturday, June 18, 2016. The
undersigned Principals of our firm both visited the site on Wednesday, June 22,
2016, at the request of Mr. David Moore of Manhard Consulting. The purpose of
our visit and evaluation is to understand the characteristics and extent of the
failure to the extent possible from observation, evaluate risk to the existing
residence and occupants, and recommend a remedial strategy.
The structure in question is a duplex, with street addresses being 6985 and
6987 W. 48`h Avenue. The slope failure occurred on the north facing slope in
back of 6985 and is substantially limited to that side of the duplex, though we
understand you own the entire duplex.
The slope failure occurred primarily within alluvial terrace deposits
(predominantly sand and gravel) associated with nearby Clear Creek. The terrace
deposits overlie claystone bedrock. The failure appears to be primarily within the
terrace deposits, though some surficial displacement and or sliding within or
along the upper weathered zone of the bedrock has probably occurred. Most
likely, the reduced shear strength of the upper weathered claystone, in a saturated
condition, contributed to the failure. Significant amounts of seepage were
occurring during our visit along the gravel bedrock contact. In our experience,
failures with these characteristics are a common occurrence where relatively
permeable sands and gravels overlie relatively impermeable clays, shales, or
claystone — on a steep slope.
The failure undermined an existing timber retaining wall, but did not
damage two existing concrete retaining walls further upslope. The existing scarp
of the failure is north of the lower of the two concrete walls. The scarp is steep —
on the order of 1:1, and about 15 to 20 feet (+/-) in height.
We understand that these two concrete walls were constructed in the mid
1990's, and are founded on 25 foot deep drilled piers. However, plans for these
walls are not available. The walls are performing well, and do not appear to have

been damaged or displaced by the recent slope failure. Photos from our June 22
visit are on file in our office.
The structure of 6985 and 6987 W. 48th does not appear to have been
damaged by this ground movement. We understand the structure was likely built
in the 1960's or 70's, and that you are not the original owner. The foundation
type is not currently known. The configuration of fencing, walls, and other
landscape features is such that access to the over -steepened scarp area is
controlled.
Our recommendations include both monitoring and stabilization of the
recent failure. While the structure does not appear to have been damaged, and the
existing concrete retaining walls are performing well, we recommend a
monitoring program to collect data. We recommend establishment of a
benchmark on stable ground south of the failure, and probably in the front yard or
driveway of 6985. Using this benchmark (or if necessary, a system of multiple
benchmarks), 6 to 8 locations on the north wall of the residence can be checked
for movement. We recommend that baseline readings be obtained as soon as
practical, and that subsequent surveys be made on an approximate weekly basis,
for at least the next 3 weeks. Ideally, the accuracy of the surveys should be
capable of discerning movements on the order of % inch or less, and data would
be presented in elevation, northing, and easting, and provided to our office for
review as soon as possible after each reading. Monitoring should continue
through the stabilization program discussed below, and at Ieast one set of readings
should be made after stabilization measures are in place.
The existing scarp is over-steepend and should be stabilized. In addition,
drainage measures to facilitate collection and discharge of seepage from the
bedrock contact described above should be included. While the existing concrete
walls are performing well, and appear to have limited the extent of the slope
failure, their ability to do so in the long term is unknown. An earth buttress could
be considered to stabilize the scarp area, but a large volume of earth would be
required. Probably, a more expedient and less disruptive stabilization program
would consist of either soil nails or ground anchors drilled into the existing scarp
area, coupled with application of a reinforced shotcrete or similar facing. Perhaps
the existing (but undermined) timber wall could be stabilized in place and re -
backfilled. Drainage measures to collect and discharge seepage from the bedrock
contact area should be included. This system would most expediently be
designed by a specialty contractor, experience and specialized in emergency
response to slope failures or landslides such as this.
Provided our recommendations above for monitoring and
stabilization are implemented in a timely manner, we do not believe that
condemnation of the structure or eviction of occupants is necessary. Access
to the scarp of the failure should be controlled. Monitoring data should be
provided to us for review, immediately after each reading.
A qualified specialty contractor toured the site with us on June 22. We
expect to receive a proposal shortly (probably by close of business today) from
this contractor. They report that they would be able to mobilize in a matter of

days if their proposal is acceptable to you, and if necessary permits can be
expedited. We will review their proposal and provide recommendations.

Attached is the Building Permit Application for the slope stability construction work on Lisa's
back yard. GSI would like to start work next week so if you could review this application and
issue the permit we would greatly appreciate it.
We would like to request that the building permit feet be waived as it does not pertain to a structure
and it will not be inspected by the City of Wheat Ridge. This work is very expensive and is a
hardship on the owner so any consideration you can give in this situation would be greatly
appreciated.
Attached are the stamped drawings from GSI and a site plan showing the area of work. It is our
understanding that GSI is not required to have a contractor's license in the City of Wheat Ridge
for this type of work.
Please process the building permit and contact me regarding the fee and any other information that
you may have.

Construction Sequence/Work Schedule:
• GSI will provide and install the specified soil nails and surface treatment per the construction documents.
Size and Spacing of Nails:
• GSI will mark the locations of the proposed soil nails with survey marking paint.
• The Soil Nails will be injected with grout. The grout will be a Type I,Il,or III Portland Cement. The water/cement ratio will be
0.40 to 0.60. No additional aggregate or admixtures will be added to the grout.
Facing and Drainage System:
• Drain strips will be provided and installed between the soil nails every 10 -feet along the face of the excavation. The drain
strips shall be placed with the geotextile side against the ground. Drain strips will be continuous and any splices shall be
made with a one -foot minimum overlap such that the flow of water is not impeded. Drain strips shall extend beyond the face
of the shotcrete at the downhill face.
Reinforcing Steel Placement:
• Welded wire mesh will be placed along the face of the excavation with a separation of approximately 2 inches between the
wire mesh and the soil.
• No. 4 Rebar will be tied to the wire mesh. Vertical bars will extend for approximately 24 inches and the horizontal bars will be
continuous (with overlap splices) in the shotcrete.
Bearing Plate Placement:
• 8" x 8" x 3/8" Steel Bearing Plates will be placed over the nails and attached with a hex nut to secure the wire mesh and
rebar during shotcrete placement. If the soil nails extend beyond the hex nuts they will be trimmed using a gas powered
demolition saw.
Shotcrete Application:
• Shotcrete will be placed from the lower part of the area upwards to prevent accumulation of rebound. The nozzle will be
oriented a proper distance from and approximately perpendicular to the working face so that rebound will be minimal and
compaction will be maximized. Care will be taken while encasing reinforcing steel and mesh to keep the front face of the
reinforcement clean during placement operations, so that shotcrete builds up from behind, to encase the reinforcement and
prevent voids or pockets from forming.

Shotcrete Mix Design:
Shotcrete shall comply with the requirements of ACI 506.2, "Specifications for Materials, Proportioning and Application of
Shotcrete", except as otherwise specified. Shotcreting consists of applying one or more layers of concrete conveyed through a
hose pneumatically projected at a high velocity against a prepared surface.
The wet -mix process consists of thoroughly mixing all the ingredients, introducing the mixture into the delivery equipment and
delivering it, by positive displacement, to the nozzle. Air jet the wet -mix shotcrete from the nozzle at high velocity onto the
