Please describe below the work practices and procedures to be employed in conducting the abatement of asbestos. BE SPECIFIC. Indicate type(s) of ACBM to be abated (e.g. VAT,
ceiling tile, TSI, etc.). Use another page if necessary.
MHES will construct a SECONDARY containment throughout the interior of the basement level according to CDPHE Rules and Regulations. MHES will abate 560 sq ft of floor the and mastic using spud bars and
picks. Non -friable floor tile will remain non -friable throughout the abatement process. An airless sprayer not to exceed 1500 PSI (using amended water) will be utilized to ensure materials are kept adequately wet
during the removal process. Final clean will include a HEPA vac and wet wipe of all surfaces within the removal area. An FAC will be conducted prior to the tear down of the containment.
MHES will place 6 mil poly and asbestos caution tape on either side of the windows to be removed. A total of 5 windows with approximately 35 sq ft of non -friable caulking will be removed using pry -bars and
picks. Material will remain non -friable throughout the removal process. An airless sprayer not to exceed 1500 PSI (using amended water) will be utilized to ensure materials are kept adequately wet during the
removal process. Final clean will include a HEPA vac and wet wipe of all surfaces within the removal area. A visual inspection by a third party will be conducted prior to the tear down of the Asbestos Caution tape.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€rms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.
