Reference architectural drawings by Studio LW. DISCUSSION Our observation took place on January 16, 2020. The purpose of the observation was to obtain the span lengths and loadings of the proposed new beams. The residence is single story ranch with a full basement. According to the county records the residence was built in 1959. For the sake of convenience, the front of the residence will be assumed to face south. In a telephone message, Mr. Randy Slusser the chief building official indicated a letter would be satisfactory for the proposed work. The first beam will be located on the main level in lieu of the wall between the kitchen and the living room. The span is about twenty-six feet from the west exterior wall to the north -south wall at the east side of the entry. This new beam is to be a dropped W10x30 steel member (ASTM A36) with four 2x4 (Douglas -Fir Standard grade or better) wood stud columns under each end. This beam has been designed for a thirty pound per square foot roof live load. The next two beams are to be located under the main level floor in the basement. The bearing wall being removed runs from the west end of the family room to the east end of the hallway. This new beam will have a total span of about twenty-six feet with an intermediate column under the east end of the new main level beam. This new beam shall be aW6x20 (ASTM A36) and may be broken over the intermediate column. All three new basement columns shall be three inch diameter adjustable steel (ASTM A53, Type E or equivalent) pipe columns. These beams have been designed for a forty pound per square foot floor live load. The intermediate concrete (3000 psi. compressive stress strength at 28 days, field cured) column support pad in the basement shall be fifty-four inches square and ten inches deep. This pad shall be reinforced with 545 bars (Grade 60) each way three inches from the bottom of the pad. The two end concrete column support basement pads shall be thirty inches square and eight inches -�_ AJ ` deep. They shall be reinforced with 345 bars each way three inches from the bottom of the pad. Concrete and rebar shall be the same as called out for the intermediate pad. The concrete pads have been sized based upon the soil report prepared by A. G. Wassenaar, Inc. with Project Number 201126, dated February 14, 2020. All new basement pipe columns shall be anchored to new support pads with at least two one-half inch diameter anchors. The main level floor system shall be solidly blocked under the main level columns down to the foundation wall at the west end and down to the new steel beam at the east end. Both new steel beams shall have a 2x nailer attached to the top flange of the same. Each existing floor joist and existing ceiling joist shall be anchored to the new nailer. The existing roof and floor systems shall be shored up during the removal of the existing bearing walls. DISCUSSION There is a double floor joist spanning the hallway at the bottom of the basement stairway which has one of the floor joists notched out for the entire width of the joist to allow for the installation of a new toilet waste line. The double floor joists span about seventeen feet and the notch takes place about one foot from the end. DISCUSSION The location of the concrete pad for the east end of the new main level roof support beam has had to be adjusted. We have instructed the contractor to install a new thirty inch square by eight inch deep pad under the south wall of the basement hallway near the furnace. Clearing an asbestos abatement project "applies to areas of public access where the amount, of ACM that will be abated exceeds the trigger levels.' Colorado Regulation Number 8 "Trigger Levels : (1.) Residential - 32 SF., 50 LF., equivalent of a 55 -gallon drum (2) 69mrmeirial or Demolition - 160 square feet (160 SF), 260 linear feet (260 LF), or the equivalent of a 55 -gallon drum). Final Clearance Air Sampling: Once an area has passed a final visual inspection and no dust or debris have been found, the certified AMS shall collect final clearance air samples from the work area, prior to tea•rdown of the containment struchrre. Final Clearance Criteria: An abatement action shall be considered complete when the average concentration of asbestos of five air samples collected within the abatement work area, and analyzed by the TEM method is does not exceed the filter background level of seventy (70) SABI' or the laboratory may analyze air monitoring samples collected for clearance purposes by PCM. Analysis of each these samples must show airborne asbestos fiber concentrations of less than or equal to 0.010 f/cc. If concentrations exceed this limit, the abatement contractor must re -clean the work area, and a second set of final clearance air samples must be collected and analyzed. This process is repeated until the analytical results of the final clearance air samples meet the regulatory standard. Only upon successful completion of the final clearance air monitoring process may an asbestos abatement project be considered complete, and teardown of the containment structure can commence. Final Visual Inspection: At the conclusion of an)- abatement action and with only critical barriers still in place, a certified AMS, who is independent of the GAC, shall visually inspect each work area where such action was conducted, and behind critical barriers, to determine whether all dust and debris has been removed. If any such dust or debris is found, the area shall be re -cleaned until no dust or debris is found. If a critical barrier is removed for cleaning purposes, the area behind the critical barrier shall be cleaned and the critical barrier immediately replaced. Friable (F): Friable materials are materials that can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Full Containment, Polyethylene sheeting shall be used to seal all windows, doors, ventilation systems, and wall penetrations, and to cover ceilings, walls and floors in the work area. Prior to constructing the containment, the GAC shall install critical barriers, establish negative pressure, construct the decontamination area, pre -clean surfaces and cover fixed objects. Miscellaneous Materials (MMT These materials are other suspect materials, including flooring, ceiling tiles, insulation, and finishing materials. Qption Out of Colorado Regulation Number 8 aka, Opt::out: A homeowner is allowed to "opt -out" of Colorado Regulation Number 8 waiving protections that are, entitled to a public or commercial building for a fee and up to 365 -days. OSHA "Trigger Levels": 10 SF, 25 SF, or 35 CF; however, OSHA states "no amount of asbestos is safe". Secondary Containment: a system of airtight barriers to isolate the work area to prevent the migration of air from the work area. LO INTRODUCTION Advanced Environmental Consulting, LIT (AEC) was retained to perform industrial hygiene services related to post - asbestos abatement work area/containment visual and air monitoring services and, PCM laboratory analysis at, the above - referenced location. The asbestos air clearance was conducted by the above referenced CDPHE and AHER.A certified AAIS. AEC is a CDPHE certified Asbestos Consulting Fire► (Certification Number 20874). 2.0 SITE DESCPJMON AND PROJECT DETAILS The building is a single-family residential dwelling. AEC was responsible for consulting with the abatement representative (Kenny), to ensure the abatement areas (entire basement & upstairs kitchen, living, dining, entry; bathroom & hallway) and materials (Drywall, Plaster, VCT & Sheet Vinyl) removed were understood so that the clearance(s) could be properly performed. 3.0 FINAL VISUAL PROCEDURES It is the AEC AMS -s responsibility to be certified in the State of Colorado and to perform clearances independent of the GAC. AEC's checklist for final visual inspection`s include: making sure only critical barriers are in place, verifying decontamination and waste load -outs are still intact, all waste bags are removed from the containment, all exhaust filters are in operation, the area, is completely dry and sufficiently well lit, having access to all areas, all equipment is clean including the outside of the NAM, HoMold shower water working properly, a• clear view port minimum of 12" x 12", and all dust and debris has been removed from the work area including behind critical barriers, It was determined that the abatement contractor had properly cleaned the work area completely of all dust and debris prior to air monitoring collection. 40 FINAL CLEARANCE AIR MONITORING AND SAMPLING Final clearance air samples were collected using constant flow high-volume air sampling pumps, twenty-five (25) MM air sampling cassettes, flexible tubing and adjustable sampling stands. The sampling cassettes were attached to one end of the flexible tubing and placed on the stands approximately forty-eight (48) inches above the floor. The opposite ends of the flexible tubing were attached to the air sampling pumps. The PCM sample pumps were activated and operated at a flow rate of approximately fifteen (15) LPM for a period of approximately forty-five (45) minutes. Reference architectural drawings by Studio LW. DISCUSSION Our observation took place on January 16, 2020. The purpose of the observation was to obtain the span lengths and loadings of the proposed new beams. The residence is single story ranch with a full basement. According to the county records the residence was built in 1959. For the sake of convenience, the front of the residence will be assumed to face south. In a telephone message, Mr. Randy Slusser the chief building official indicated a letter would be satisfactory for the proposed work. The first beam will be located on the main level in lieu of the wall between the kitchen and the living room. The span is about twenty-six feet from the west exterior wall to the north -south wall at the east side of the entry. This new beam is to be a dropped W10x30 steel member (ASTM A36) with four 2x4 (Douglas -Fir Standard grade or better) wood stud columns under each end. This beam has been designed for a thirty pound per square foot roof live load. The next two beams are to be located under the main level floor in the basement. The bearing wall being removed runs from the west end of the family room to the east end of the hallway. This new beam will have a total span of about twenty-six feet with an intermediate column under the east end of the new main level beam. This new beam shall be aW6x20 (ASTM A36) and may be broken over the intermediate column. All three new basement columns shall be three inch diameter adjustable steel (ASTM A53, Type E or equivalent) pipe columns. These beams have been designed for a forty pound per square foot floor live load. The intermediate concrete (3000 psi. compressive stress strength at 28 days, field cured) column support pad in the basement shall be fifty-four inches square and ten inches deep. This pad shall be reinforced with 545 bars (Grade 60) each way three inches from the bottom of the pad. The two end concrete column support basement pads shall be thirty inches square and eight inhes deep. They shall be reinforced with 345 bars each way three inches from the bottom of the pad. Concrete and rebar shall be the same as called out for the intermediate pad. The concrete pads have been sized based upon the soil report prepared by A. G. Wassenaar, Inc. with Project Number 201126, dated February 14, 2020. All new basement pipe columns shall be anchored to new support pads with at least two one-half inch diameter anchors. The main level floor system shall be solidly blocked under the main level columns down to the foundation wall at the west end and down to the new steel beam at the east end. Both new steel beams shall have a 2x nailer attached to the top flange of the same. Each existing floor joist and existing ceiling joist shall be anchored to the new nailer. The existing roof and floor systems shall be shored up during the removal of the existing bearing walls. General. This is a contractual agreement ("Agreement") by and between O'Connor Roofing Company (hereinafter called "Contractor"), and the Owner(s), as reflected on the reverse side, whereby, upon Contractor's acceptance, Contractor agrees to supply and furnish the materials and labor at the location(s) set forth on the reverse side, to repair, restore, refurbish or build the improvements to the real property, and whereby Owner(s) dgree to pay to contractor all invoices within ten (10) days after job completion. 2. Materials and Workmanship. Contractor agrees to supply and furnish and use materials of a value and quality reasonably equivalent to those previously existing (if any), if reasonably available, provided, however, Owner may require materials of a different value or quality than those previously existing (if any) if specified in writing and agreed to by Contractor. Unless otherwise specified by Owner in writing and agreed to by Contractor, Contractor, in furnishing materials and labor, agrees to match reasonable existing textures, colors, plans and materials but does not guarantee exact duplication. 3. Access to Work. The Owner shall at all times have access to the work wherever it is in preparation and progress. 4. Zoning and Modification. Owner agrees to furnish all information necessary to secure plans and permits, if required or necessary, unless otherwise specified, and Owner warrants the work contracted for to be in compliance with zoning, classification and building codes. In covenant -controlled communities or neighborhoods that have an HOA, it is the responsibility of the property owner to make sure that such governing body approves selection of product. Owner agrees that Contractor may employ an architect, on behalf of Owner, to submit plans to the property authorities for approval, inasmuch as this Agreement may be based upon an estimate of what will be required to restore property to its equivalent status before loss, if any modification in the original plans and/or of the contents of the property before loss is desired by Owner, or if any additional work required by lawful authorities to bring the plans, contents and/or work into compliance with code requirements shall entail additional costs. Owner agrees to pay such additional costs and architects fees to be covered by the Estimate. 5. Pre -Existing Conditions. Owner agrees that Contractor assumes no responsibility for pre-existing conditions of inferior workmanship or materials; damage due to termites, rot or deterioration, or violations of zoning, classification or building codes or leaks above the roof line! 6. Hazardous Materials. Unless otherwise provided, Contractor assumes no responsibility for testing for the presence of, removal, or the disposal of asbestos containing or other hazardous materials. Contractor agrees to promptly notify the Owner if such materials are encountered and recognized as the work progresses, and may, at Contractors option, stop work until such time that suspect materials have been properly tested or arrangements by the Owner have been for the lawful removal and disposal. Contractor may also at Contractors option issue to the Owner for all work completed to the date of work stoppage, with such billing to include all costs including direct and indirect overhead, taxes and profits on the work completed. 7. Ownership of Materials. All work and materials delivered on the premises by Contractor to form part of the work, whether actually incorporated therein or not are to be the property of the Contractor until the same have been paid for in accordance with the terms hereof. All surplus and salvage materials remain the property of Contractor. B. Acceptance. This Agreement, having been signed by Owner as accepted, constitutes an irrevocable offer to be accepted or rejected by Contractor within thirty (30) days from date of its delivery from Owner. 9. Performance. Performance of this Agreement is made subject to weather, labor strikes, fires, wars, acts of God and the ability of Contractor to obtain materials. 10. Changes During Process. Insurance and Waiver of Subrogation. The Owner shall maintain property insurance upon the entire structure including all work to be performed under this contract to the full insurable value thereof. Payments. Unless otherwise provided, Owner agrees to pay Contractor within ten days of completion. Severability. Should any part of this Agreement be void, unenforceable, or contrary to public policy, only such portion of this Agreement shall be deemed eliminated herefrom, and the remainder of this Agreement shall remain valid and subsisting. 14. Collections. Should collection of payment under this Agreement be placed by Contractor in the hands of an attorney after payment is due, Owner agrees to pay Contractor's attorney fees, court costs, and expenses of collection. If the balance is not paid within thirty (30) days, Owner agrees to pay a late charge on any outstanding balance from the date of completion at the rate of eighteen percent (18%) per annum, compounded monthly until paid. Any collection or legal fees will be added to the balance and subject to the same interest charges. Owner will be deemed to have accepted Contractor's performance under the contract in total unless Owner otherwise notifies Contractor in writing within thirty (30) days after completion of project.. 15. Ice Damming. Contractor has no responsibility or liability for ice damming issues or any consequential damages from ice damming that occur after the roofing project Is completed. It is the Owners responsibility to take preventive measures to prevent causes of ice damming. 16. Flexing. When existing roofing material is removed from a roof, it is not uncommon for the roof to "flex". On occasions this flexing may cause small cracks in sheet rock. Contractor cannot be responsible. 17. Nail Pops. Construction of buildings in many cases involves the use of sheetrock interiors. At the time of application, a small amount of spackling may be used to cover the head of the nail for application. Contractor cannot be responsible if spackling is dislodged during roof installation. 18. Disclaimer. Contractor will not responsible for items such as pictures, china, crystal or anything in the interior of the building that may fall from vibration during the work process. It is the owner's responsibility to secure such items. Contractor will not be responsible for exterior items that are not properly secured such as light fixtures or any items attached to the building that may fall from the vibration during the work process. Contractor will not be responsible for items that could be damaged from debris falling from the roof, such as pottery, outdoor furniture, grills. Owner accepts the responsibility of moving such items away from the building during the work process. Contractor will not be responsible for any damage to driveways caused by supply company trucks. Contractor will not be responsible for separation of the sealant of lens of skylights from the frame of the skylight. Contractor will not be responsible for repositioning of satellite dishes. Owner agrees to keep the driveway clear of any vehicles or possessions that would prohibit the supply company from loading the roof. Owner also agrees to keep the driveway clear of any vehicles or possessions during the installation of the roof. 19. Agreement.
