Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding AM -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturers€'"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.
THIS APPLICATION DOES NOT CONSTITUTE A PERMIT. DO NOT BEGIN
REROOFING UNTIL PERMIT HAS BEEN ISSUED.

CENSpace Structural, LLC conducted a follow-up investigation of the subject location on
February 1, 2007 to further investigate the cause of cracks in the exterior fapade and interior
ceiling of this single family residence. The following report includes the project background,
objective, observations recorded from the site visit, analyses of the observations as well as
recommendations to repair the damage. Supplemental photos and sketches have been attached
and are referenced in this report.
BACKGROUND•
This is considered a follow-up inspection to the inspection performed on November 9, 2006. At
the first inspection, the cracks in the ceiling were determined to be the result of long-term
deflection of the ceiling members. However, the cause of the exteriar cracks in the brick
masonry and the wood trim as well as the issue with the front door sticking was unknown,
because the overhead interior roof areas were not accessible during the inspection, either on the
interior or exterior. At that time it was recommended that some of the exterior soffit material
near the front door be removed in order to conduct a more thorough investigation of the roof
framing in that area.
OBJECTIVE:
The cause of the cracks in the exterior brick masonry fagade as well as the interior drywall
cracks along the south exterior wall is to be determined as well as recommendations to repair any
damage.
OBSERVATIONS•
The following observations briefly describe the condition of the home at the time of the
inspection. The issues described below are based upon visual inspections and field
measurements. For the purposes of this report the front of the home faces south.
1. Cracks were visible on the ceiling in the living room and kitchen areas (Refer to the previous
report dated November 15, 2006). There were also some small veRical cracks along the

interior drywall finish of the front living room (south exterior) wa1L This wall was measured
to be leaning slightly outward (about 1/2° out of plumb) using a digital Ievel.
2. Masonry cracks were visible emanating from the upper west corner of the front entty door.
This cracking ran between the corner of the door and the top of the concrete block masonry
wall cap. No evidence of damage was observed along the top (wood) plate of the wall (Refer
to Figure 1).
3. The wood fascia above the front door was removed to expose the top of the wall and the roof
framing in that area.
3.1. The top layer of masonry on the wall is constructed of concrete block and one corner of
the concrete appeared to have been broken loose from the wall. This loose corner has
left the wood soffit beam that carries the corner of the porch overhang essentially
unsupported on one end (Refer to Figure 2).
3.2. The roof framing along the south exterior wall of the home appeared to be undamaged
(Refer to Figure 3). There was no blocking observed between the roof rafrers or the
ceiling joists along the visible portions of this wall.
4. Several twisted ceiling joists were discovered over the kitchen area where the joists rest on
the central bearing wall of the home. These joists are toe-nailed into the top of the supporting
wall and have no solid blocking between the supported ends (Refer to Figure 4).
5. Small cracks were observed in the ceiling finish of the southwest bedroom. The cracks were
not addressed in the first inspection because they appeared to be minor and unrelated to the
cracks that were observed in the living room. These cracks are not detectable from the attic
space.
6. Another brief inspection of the south end of the foundation was conducted to look for
evidence of foundation movement or damage. The exterior foundation walls appeared to be
intact and undisturbed.

ceiling joists at the interior beam may be the result of two factors: (1) inadequate connection
between the ceiling joists and interior beam as noted in the first inspection report and (2) outward
pressure on the south exterior wall pulling the ceiling joists away from the interior beam. It is
likely that excessive outward pressure was placed on the south exterior wall by heavy snow
accumulation on the south roof slope. Based upon the roof framing configuration, a heavy snow
load could push the sloping rafrers outward, placing significant lateral loads on the top of the
south exterior wall. The measured deflection of the south wall described in Section 1 above,
couid have produced the small cracks in the drywall and exterior brick faqade that was observed.
It is difficult to determine if one of these factors contributed significantly more than the other to
the damage observed.
A heavy snow load pushing t6e rafters down and causing the south wall to deflect outward may
also have caused the ceiling joists to be pulled toward the exterior of the residence and away
from the central support beam. It is reasonable to assume that an excessive snow load could

have exerted enough pressure on the structure to cause failure of the ceiling joists' connections to
the central support beam.
Cracking in the wood fascia above the front entry door is likely the result of a corner of the
concrete block breaking away from the top of the wall, causing a loss of support on one end of
the beam carrying the porch overhang. This lack of support would allow movement of the beam,
and cause cracking in the adjacent wood fascia particularly if that section of the roof experienced
heavy snow loads.
The cracks in the southwest bedroom were not visible from the attic area. These cracks are
believed to be cosmetic only, and seem to generally follow a seam between two perforated
drywall panels in the ceiling. No other structural damage was visible in this area.
The twisted ceiling joists over the kitchen area are likely the result of two factors: (1) the
movement of the adjacent joists and central support beam could be pulling unevenly on these
joists, and (2) moisture entering the attic space from the kitchen below. The uneven movement
of adjacent joists, as well as the central support beam, could be causing twisting of some ceiling
joists over the kitchen area. Typically, some twisting is prevented by providing solid blocking
between the joists at their supports, which was absent in this roof assembly. The twisting of these
joists, over time, could also be a result of the fluctuation of humidity in the attic space. As the
humidity in the attic space is raised and lowered, the water content of the timber members in the
space also fluctuates. Repeated fluctuation of water content within the timbers can cause twisting
of the members over time.
RECOMMENDATIONS:
Based upon the analyses of observations stated above, it is recommended that the ceiling over the
living room and kitchen be reinforced then repaired. The ceiling joists over the living room and
kitchen area, as well as tbe center beam, should be reinforced to meet the current minimum
design loading standards; it may be necessary to first jack up the center beam and/or the ceiling
joists to remove the existing deflection/sag in the ceiling. These structural members may be
reinforced by scabbing additional material alongside them and providing proper connections
between all the structural elements. Blocking should also be added between the ceiling joists as
specified in the attached drawings. Once these structural issues have been resolved the ceiling
plaster can be refinished.
The damaged concrete block should be repaired to provide adequate support for the beam
canying the overhang of the front porch. This repair should be carried out according to standard
consVuction practice.
