Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

ADDITIONAL CONTRACT TERMS AND CONDITIONS
THE CONTRACT BETWEEN ALL ANGLES COVERED, LLC ("AAC") AND THE PROPERTY OWNER
("CUSTOMER'S IS SUBJECT TO THE FOLLOWING ADDITIONAL. TERMS AND CONDITIONS:
1. AAC may order excess materials. Excess materials will not be charged above the Contract price. All excess materials belong to
AAC. If materials need to be restocked or reordered due to a cancellation by the Customer, Customer will be responsible for an
additional RESTOCKING FEE of $500.00.
2. All materials are guaranteed as speed. All work to be completed in a workmanlike manner according to industry standards.
AAC warrants its workmanship for 5 years for any roof replacement and gutter installation and 1 year on all repairs. Materials
are warranted by the manufacturer pursuant to manufacturer's written warranty. AAC will supply a Completion Certificate and
manufacturer warranty information upon receipt of payment in full.
3. Alterations or deviations from approved specifications resulting in additional labor or materials costs will be executed only upon
written Change Order. Customer is responsible for the cost of all Change Orders above the estimate and Customers obligation
will not be limited to the amount of insurance deductible. All agreements and service dates contingent upon strikes, accidents,
or delays in obtaining materials, inclement weather or other causes beyond _IAC's reasonable control.
4. The Customer shall carry general public liability insurance and property damage insurance (including contents and other
personal property), for all-risks, including fire, smoke, tornado, hail, wind and rain, and all other insurance appropriate for the
Customer. ROOF REPAIRS OR REPLACEMENTS REQUIRE THE INTERIOR OF THE STRUCTURE BE
EXPOSED TO THE ELEMENTS DURING_ THE PROGRESSOF THE WORK. AAC WILL USE
COMMERCIALLY REASONABLE EFFORTS TO IDENTIFY THE POTENTIAL FOR INCLEMENT
WEATHER EVENTS AND TO PROTECT THE ROOF AND STRUCTURE DURING THE PROGRESS OF
THE WORK BUT SHALL HAVE NO LIABILITY FOR ANY DAMAGES CAUSED BY WEATHER OR OTHER
CAUSES. AAC shall have no responsibility for damages from rain, tornado, windstorm, hail, ice, ice dams, or other perils, nor
for any damage from nail pops, driveway cracking or below the roof or to the interior of the premises or any contents or
personal property located therein and the Customer shall be responsible for properly safeguarding (and removing, if necessary)
its property from the jobsite.
5. Customer agrees that AAC will not be liable for any structural movement, settling, cracks in dry wall, or driveway due to the
performance of the work or the unloading and loading of materials or equipment
6. The Customer shall designate an area for AAC to place trailers, dumpsters, or stored materials.
7. The dollar amount of the Contract is the amount approved by the Customers insurance company ("RCV") for the scope of
loss, plus overhead and profit (if applicable), and any agreed upgrades or extras. ANY replacement of damaged or deteriorated
decking, flashing, roof jackets, fascia boards, vents or other materials or components, unless otherwise stated in this Contract
and approved by the Property Owners Insurance Company are EXCLUDED and if such work is requested will be charged to
and paid by Customer as extra materials and labor, which amounts are in excess of insurance deductible.
8. Supplemental amounts paid by the Insurance Company for additional labor or materials above the original scope of repairs shall
be paid directly to AAC.
9. AAC is not responsible for pre-existing conditions or previous design, construction, or maintenance conditions or problems
with the premises, whether known or unknown at time of acceptance. AAC may alert and inform the Customer on correcting
any problems it may observe, but AAC has no such obligation or responsibility.
10. Final payment to AAC is due upon substantial completion of the work, without delay for any inspections by city- or other
governmental authorities or others. Any amounts not paid within fifteen (15) days shall accrue interest at the rate of eighteen
percent (18%) per annum from the due date, until paid. Customer shall be responsible for all attorneys' fees, costs, and legal
filing fees incurred by AAC to collect any amounts due or otherwise enforce the terms of this Contract In the event of a legal
proceeding arising out of this Contract, the prevailing party shall be awarded their attorneys' fees, and costs.
11. The Contract is binding on the Customer for all purposes upon execution on the reverse side of this Contract, so long as the
Property Owner's Insurance Company approves the work. Customer is not entitled to cancel this Agreement for any other
reason except if mandated by and in strict compliance with C.R.S. § 6-22-101 et seq., if applicable.
12. By signing this Contract, Customer represents that s/he is the owner of the property on which the work is being performed and
authorizes AAC to perform the work as approved and specified by the Property Owner's Insurance Company, and the
Customer agrees to deliver to AAC all Insurance Company payments on account of the work, properly endorsed to the
Company, immediately upon receipt, and to otherwise pay to AAC all other amounts, including the deductible portion, if
applicable, or for extra work, immediately upon receipt of invoice.
13. The provisions limiting out of pocket expenses to the amount of Property Owners insurance deductible applies only to the
work as approved and specified by the Property Owners Insurance Company identified on the reverse side of this Contract, and
not to any extra work or upgrades, and not to any costs, expenses or legal fees.
14. This Contract contains the entire agreement between the parties and may be changed only by a written instrument signed by all
parties. All prior negotiations, statements or representations shall be deemed merged herein and shall not be relied on and do
not survive the execution of this Contract. If any provision of this Contract should be held to be invalid or unenforceable, the
validity- and enforceability of the remaining provisions shall not be affected thereby. A

15. Customer agrees to provide a copy of Insurance Claim Report to AAC promptly upon receipt.
16. To the extent any term or condition of this Contract is void, illegal, or unenforceable under any state or federal law or statute,
such void, illegal or unenforceable provision shall be deemed stricken, but the balance of this Contract shall remain in effect
17. If Property Owners mortgage lender requires that all or any portion of the payments issued by Property Owner's Insurance
Company be paid to or retained by the mortgage lender, Property Owner shall remain liable for payment of such amount
directly to AAC.
18. Property Owner represents that Property Owner is current on all mortgage payments owing on all mortgages on the Property.
MANDATORYCOLORADO DISCLOSURES
In Compliance with C.R.S. § 6-22-101, etseq.
1. AAC shall hold in trust any payment received from Customer until roofing materials are onsite and work has begun.
2. AAC carries both general liability and workers compensation insurance. General liability insurance is produced by Signature
Select LLC., 1705 17th St., Suite 100, Denver, CO 80202 (303) 534-4567. Policy # CGL0052701-22.
3. RESCISSION. Customer may cancel this contract and receive a full refund of any amounts paid at Customer's sole discretion
within 72 hours of execution of this contract In witness whereof purchaser(s) each acknowledge receipt of a completed copy of this
contract, and all disclosures on the day and year written below. If you plan to use the proceeds of a property and casualty insurance
policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive written notice from the property
and casualty insurer that your claim has been denied in whole or part. In the event of rescission.. AAC is entitled to retain payments
or deposits to compensate AAC for roofing work actually performed in a workmanlike manner consistent with standard roofing
industry practices.
4. DEDUCTIBLE. Your deductible is the portion of your claim that is not covered by your insurance company. You, the
insured, are responsible for paying your deductible amount to AAC. Example: Your claim equals $10,000. If your deductible is
$500, the insurance provider will pay $9,500 in total. Leaving you, the insured, to pay the difference, $500 to AAC. In compliance
with Colorado law, AAC cannot pay, waive, rebate, or promise to pay, waive, or rebate all or part of any insurance deductible
applicable to the insurance claim for payment of roofing work on your property. As the insured, your portion of payment will be
your deductible, plus the cost of any upgrades or options you choose that are not included in the Scope of Work.
5. Start date will be no more than 180 days after contract date. Once the contract is executed and color chosen, AAC will order
permits and materials to be delivered to the above listed home. This process is expected to take up to 2 weeks. XX'ork is expected to
be completed in a few days thereafter.
SOLAR PANEL AND ACCESSORYITEMADDENDUM
1. Customer agrees to take necessary steps to remove, protect and reinstall all solar panels, conduit and satellite antennas
("Accessory Items") in order to properly protect and safeguard them during performance of the work.
2. The Accessory Items must be placed at a mutually agreeable storage location that will not interfere with the work of AAC.
3. AAC shall have no responsibility for Accessory Item damage, loss or peril.
4. AAC shall have no liability for loss, damage or theft, or damage to driveways or other areas caused by the placement or storage
of such items.
SPECIAL ORDER AM TERIALSADDEND UM
1, If Customer selects materials which require a special order from AAC's customary supplier, payment of 50% of the contract
total will be required when the materials are delivered to the location of the work.
2. Customer will be responsible for all costs of special order materials, including any amounts which are non=refundable upon
cancelation or termination of the contract.
3. Customer is responsible for all costs beyond the amount of the RCV associated with impact resistant shingles which may be
required by Customer's insurance carrier following a hail claim AAC does not warrant the durability of impact resistant shingles.

This memo is to advise you that the building permit application received on Apri124, 1998 for the above
referenced address can not be processed by the Public Works Department until the following revisions are
made to the drainage/site plan submitted, and an acceptable resolution is achieved (see comment #4),
regarding the correct location of the eastern property line:
oCl. Please designate the existing and proposed topography of the entire site.
Olt 2. We will need to show on the site grading plan how flows from the site aze to be properly conveyed so that
adjacent properties are not adversely impacted by the proposed onsite improvements. This is
especially critical due to close proximity of the proposed concrete driveway to the east property line, and
the proposed location of the 4-caz garage to the east and north property line, (see also comment #4 below)
a concise cross sectional detail will need to be provided for each property line. This is needed to confirm
the placement of acceptable vertical and horizontal grading to the respective adjacent properties.
&,3. A finished floor elevation needs to be provided to confirm this elevation with the existing, adjacent
topography.
6P 4. It has been brought to my attention that there is a potential discrepancy regarding the correct location of
AO, the eastern property line of this lot. Specifically, this property line may be located 4.41-feet further to the
~L fl• west. If this can be substantiated, it will effect the current locations proposed for the driveway and 4-car
~arage. It is my understanding that the affected homeowners aze to discuss this issue in an attempt to
eIb'4 resolve this problem.

This response is to concerns brought to our attention by James Bahrenburg, 7505 W. 47th Place,
regarding turn around time related to issuance of building permits.
Permit #7631:
1. Application was received Monday, August 3, 1998 for a new patio enclosure.
2. Plans were submitted to zoning for approval. (No date available)
3. Pians were reviewed and approved by the Building Department Tuesday, August
11, 1998.
4. A permit was issued on August 14, 1998.
Permit #7253:
1. A permit appiication was received Tuesday, April 21, 1998 for a new detached
garage.
2. Plans were submitted to zoning for approval (no date available)
3. Plans were submitted to Public Works, a memo was issued to John Eckert, Chief
Building Inspector Monday, April 27, 1998 stating the permit could not be issued
due to probiems with the drainage/site plan.
4. Plans were resubmitted and received by Public Works Friday, June 5, 1998 and
okayed the same day.
5. A permit was issued Tuesday, June 9, 1998.
Turn around time for the first permit appeared to be 7 to 8 days, 5 to 7 days is the normal
turn around time for this type of permit.
Turn around time for the second permit appeared to be 5 to 6 days for the first submittal and
2 to 3 days for approval of the re-submittal. Applicant had the perinit for more than 30 days making
the required corrections needed to issue the permit, again our turn around time for this project was
7 to 9 days.
It does not appear the turn around time for these permits was excessive and if there
is anything else we can do to be more efficient I am always open to suggestion.

Dear Madame:
How nice it would be if, in fact, it were government ^of the people, by
the people" rather than a not-so-benevdlent procrastinationship. I refer to
the building permit department o£ Whea+ Ridge.
Two weeks ago I was assured by someone in that office that "it should
take only a day or two." On August 10 I was toldt "your permit is an his
desk and will be signed by the end of today__guaranteed." On Augast 11 I went
to the building permit office and was informed that "he hasn't signed it yet.
Did I call you?" My response: "No, I called you yesterday and was told
rhe previous building permit I applied for took 2 months to be issued. I
now have a contractor standing byp waiting for the issuance of this ^in a day
or two" and as yet unsidned pex-nit. If 'ha' hasn8t signed it yet, whyF Where
is he2 Where is he not4 Why is the by-the-book homeowner required to endure
the tail-wags-the-dog wait ad nauseumZ Especially when 2 weeks ago I was told
that~ it would be a day or two.
And there are those that wonder why govermment has such a bad reputation.
Doing things by the book is an exercize in frustration BIG TI.^1E.
