Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS AND PRICE: This Agreement does not obligate the Customer or Aspen Exteriors®, Inc. ('Aspen") in any way unless payment for the
damage to Customer's Property is approved by Customer's insurance company and accepted by Aspen. By signing this Agreement the Customer
authorizes Aspen to represent Customer's best interests for restoration and repair of damage to Customer's Property at a price agreed to by
Customer's insurer and Aspen ('Agreement Price") with no additional cost to the Customer except for Customer's deductible, and any and all
(non-rerecoverable) depreciation associated with an actual cash value (ACI) Policy. When the Agreement Price is determined it shall become
the final contract price. Customer authorizes Aspen to obtain and provide labor and material in accordance with the Agreement Price and
agrees to pay the Agreement Price to Aspen. If replacement of roof decking is required but not covered by Customer's insurance policy or
the Agreement Price it shall be replaced and Customer agrees to pay Aspen above and beyond the Agreement Price $2.50 per square foot
repaired for CDX decking and $6.50 per linear foot repaired for plank decking. Customer shall be responsible to pay Aspen for any items of
labor or materials that are required by applicable building codes and not covered by Customer's insurance policy.
PAYMENT SCHEDULE: The full applicable insurance ACV payment, deductible, and applicable out of pocket amounts shall be paid to
Aspen prior to the Work commencing. The full applicable insurance depreciation payments shall be due upon completion of each
construction trade. The remaining balance, which shall be the applicable insurance supplemental payments, unpaid depreciation amounts,
tax, base service charges, overhead & profit, and all outstanding balances owed to Aspen sha due upo total project completion.
Payments due to Aspen shall not be contingent upon city inspections and/or repair or punc i iss e s en will provide a warranty
certificate and lien waiver after final payment to Aspen.

This Agreement shall be subject to all applicable federal, state and local (city where Customer's property is located) laws, regulations and ordinances.
The work to be performed pursuant to this Agreement shall be collectively called the "Work." Any insurance estimates Aspen prepares in connection
with the Work shall be collectively called the "Estimates." Customer's property where the Work shall be performed shall be called the "Property."
Insurance proceeds and the Agreement Price for this project shall be defined as the full price for repairs allowed by Customer's insurance company,
including any and all supplements, and before any deductions for deductibles or depreciation. Aspen may order excess materials to complete the
project, which materials will not be charged above the Agreement Price. All excess materials shall remain Aspen's property.
If material and labor prices increase or additional Work is required beyond the scope of the Work covered by the Agreement Price, Aspen may
supplement its initial insurance Estimates and Customer shall cooperate in connection with such efforts. Supplemental amounts paid by Customer's
insurance company for additional labor and/or materials beyond the original Agreement Price shall result in a commensurate increase in the
Agreement Price and all such amounts shall be paid directly to Aspen.
Aspen shall have no responsibility for 1) any damage below the roof, due to leaks by excessive wind of 60 mph or more, ice dams, hail or preexisting
construction defects during the period of the warranty; 2) damages from fire, windstorm or other hazard, as is normally contemplated to be covered
by Customer's Insurance, unless a specific written agreement with Aspen has been made prior to commencement of the Work; 3) failure of
performance due to labor controversies, strikes, fires, weather, acts of nature, or inability to obtain materials from usual sources; or 4) any other
circumstances beyond Aspen's control whether of similar or dissimilar nature.
Aspen will provide the Customer with a three (3) year limited warranty covering material defects in workmanship. Any manufacturers' warranties
shall provide exclusive coverage for product defects, and Aspen shall not be responsible for any product defects. This Agreement and Aspen's
warranty may not be assigned and are non -transferable. If the Work includes roofing and sheet metal, it is understood and agreed that only Aspen's
standard roof warranty shall apply and that all terms and provisions therein shall prevail, unless otherwise specifically agreed to in writing prior to the
commencement of the Work. All warranties are null and void if Aspen is not paid in full in accordance with this Agreement.
If Customer does not allow Aspen to perform the Work or has a person or entity other than Aspen perform the Work, Aspen shall be entitled to
liquidated damages equal to 20% of the total amount of the most recent Estimate, including all supplements, plus costs incurred by Aspen in
preparation of the Estimate, any supplements, and the Work. These liquidated damages are not a penalty, but are instead agreed to be payment to
Aspen for its time and efforts in preparing Estimates and meeting with Customer's insurance company and/or insurance adjuster. If materials must be
reordered or restocked because of Customer's breach of this Agreement, in addition to any and all other damages to which Aspen is entitled,
Customer shall be liable for a restocking fee equal to fifteen percent (15%) of the price of the applicable materials. Customer shall be liable for and
agrees to pay Aspen for the entire cost of any materials that cannot be returned or restocked.
Customer shall pay a service charge of 8% per annum on all balances 30 days or more past due, including upon all agreed upon damages specified
herein: Customer also specifically agrees to pay for all collection costs, including employee time and expense and all attorneys' fees and costs, Aspen
incurs in either collection of and/or protection of its interests in Customer's past due account or performance of this Agreement.
Aspen accepts no liability to indemnify or hold Customer harmless for claims or damages to persons or property, except to the extent that such
damages occur during performance of Aspen's Work, and are the direct result of Aspen's error or omission. Aspen shall not be responsible for
damages for any area of the Property upon which Aspen's Work has not been completed. Customer understands and acknowledges that Aspen does
not warrant or guarantee previous workmanship or pre-existing materials, nor any materials or labor not originally provided by Aspen. Customer
understands and agrees that Aspen shall have no responsibility for damages of any kind to persons or property occurring after completion of the Work
or after Customer has breached this Agreement. Nothing contained in this Agreement shall be construed to require Aspen to determine the presence
or absence of any hazardous materials or asbestos -containing materials affecting Aspen's Work or Customer's Property or to require Aspen to remove
or protect such materials. If Aspen learns of the presence of such materials at the Property, Aspen reserves the right to immediatelv stop the Work

is not present, INSPECTION WILL NOT BE PERFORMED. 6 nails per shingle is
required. Ice dam membrane is required from eave edge to 2' inside exterior
walls. Board sheathing with any gap greater than 1/2" requires panel sheathing
overlay on entire roof. Sheathing inspection is required prior to covering.
Contractor shall provide ladder for inspections. Scheduled inspections that
cannot be performed due to inclement weather must be cancelled prior to 8:30
a.m. on the day of inspection or a re- inspection fee will be assessed. Subject
to field inspection.

1. This permit was issued based on the information provided in the permit application and accompanying plans and
specifications and is subject to the compliance with those documents, and all applicable statutes, ordinances,
regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must
be made in writing and received prior to the date of expiration. An extension of no more than 180 days made be granted
at the discretion of the Chief Building Official and may be subject to a fee equal to one- half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to
the standard requirements, fees and procedures for approval of any new permit. Re- issuance or extension of expired
permits is at the sole discretion of the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water . without prior
and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy
of all required inspections and shall not proceed or conceal work without written approval of such work from the
Building and Inspection Services Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of
any prov'sio any applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject

7. Permittee assumes all liability and holds the City harmless against all claims, damages, suites, penalties, fines
and liability for injury or death to persons or loss or damage to propeRy related to the placement of a
dumpster/confainer in tfie City rightof-way.
8. The repair of any damage to pavement, curb, gutter, landscaping resulting from the placement, use or removal of the
dumpster/container shall be the responsibility of the applicant and shall be perfortned to the satisfaction of the City.
